Community Pharmacy Humber is proud to announce the inaugural Community Pharmacy Humber
Awards, to be held for the first year on Wednesday 9th September 2015 at the Village Hotel, Hessle.
The event will follow our AGM at 19:00.
What is it?
Our Awards event is to recognise the excellent contribution to patient care, public health and
community involvement that community pharmacies in the Humber area provide. We will be
presenting awards across five categories giving opportunity for everyone in the pharmacy team to
be recognised.
The awards will be presented at a celebration dinner with invited local stakeholders from the local
NHS commissioners and providers. We will also be having a special guest keynote speaker.
Who is eligible?
All NHS community pharmacies in the area of Community Pharmacy Humber LPC are eligible to
enter the awards, and there are different award categories for different members of the pharmacy
team, along with a category for the whole pharmacy and a special patient recognition award.
How do we enter?
Read the summary of the categories below and then complete the application form included.
Applications should be no more than 250 words, but you can refer to posters and/or photos or other
resources in your application.
When do we have to apply by?
The deadline for submission is 20th July 2015.
What do we win?
The award winners will receive the recognition of their peers, a unique trophy and a certificate.
How much does it cost?
Entry is free and there awards dinner is free for shortlisted nominees. If you wish to attend the

Categories
Pharmacy Team of the Year
A pharmacy’s biggest asset is its staff – its pharmacy team. And a good pharmacy team is greater
than the sum of its parts, with every member using their individual talents to contribute to the
group’s overall objective – great patient care. Tell us how your team puts your pharmacy or
pharmacies at the heart of the community/ies it serves, how it has overcome obstacles by working
together and how it has used individual skills and strengths to create an even more powerful whole
– all to the benefit of patients.




What has your team done to deserve this award and why?

How has your team improved your pharmacy’s/ies’ links with other healthcare
professionals?
Community Pharmacist of the Year
A great community pharmacist has many strings to his or her bow. Whether you are a branch
manager, run your own pharmacy, or act as superintendent, you strive to be the best. You are a
leader in your field, combining the day-to-day running of your pharmacy or pharmacies with cutting
edge practice, be it clinical, business or management. Tell us how you have maximised delivery of
your contract, developed clinical services and built links with patients and peers. An all-rounder who
doesn’t compromise one aspect of the job for another, you should be able to show how you have
engaged the whole pharmacy team to help your patients.






Pharmacy Technician / A.C.T of the Year
Pharmacy technician may be the new professional title on the healthcare block, but the role is
rapidly expanding – and this is your chance to show how you are making a difference to your
patients. With an eye for detail, an ability to multitask and a willingness to go the extra mile, the
winner of this award will make everything tick in their pharmacy. Whether you have improved safety
or efficiency in the dispensing process, taken on delivery of clinical services or management
responsibilities, or mentored colleagues, your entry should demonstrate what you’ve achieved, how
you did it, and the measures of success.


Pharmacy Team Member of the Year
Assistants are the frontline of pharmacy and deal with millions of patient requests every year. The
winner of this award will be a friendly and familiar face to their customers and an example to their
colleagues. Simply doing the job well isn’t enough – the winner needs to go a step further. Whether
you have helped recruit patients to your pharmacy’s services, delivered health promotion activities,
or forged links with your local community, the C+D Awards is your opportunity to show how
important you are to the pharmacy team. Entry can be by nomination as well as by self-entry.




Patient nomination for special recognition
A special award recognising any individual or group of individuals that have gone the extra mile to
really make a difference for their patients or their community. This category is open to nominations
from the general public as well as directly from pharmacies and we will be working with patient
representative groups to judge this category.
