Indicate your approved courses on the bottom of your consent form. • Fill in your full legal name and high school name, and obtain all required signatures including school counselor, parent or guardian, and your own. • Create an electronic copy of your consent form by scanning it or taking a digital photograph. Save this copy for use later in the application process.
