2. Rationale for the module and its links with other modules
This module is the compulsory module for the BSc (Hons) in Long-Term Conditions as the key
concepts discussed within it are fundamental to the care of people living with long-term
conditions. It is underpinned by the principles of evidence based healthcare.
Its focus on developing skills to support person, family and community centred approaches to
the care of and development of services for people living with long term conditions and their
families firmly underpins the content of the disease specific modules The subjects covered
within this module draw together key concepts of the disease specific modules, linking optional
modules, and as such, enables students to study a cohesive programme.
3. Aims of the module
This module is suitable for all practitioners who work with people with one or more long term
condition(s) who wish to develop skills in supporting people to live with these conditions in line
with current health and social care policy based on national and international evidence
This module aims to equip health professionals with the knowledge and skills required to
underpin effective long term conditions management. It explores health and social care needs
and identifies different care delivery methods that can be used to meet the healthcare needs
of individuals with a long term condition and their families and carers.
Successful students will be able to demonstrate an understanding of the evidence base and
policy initiatives informing care of people with long term conditions. They will be able to
identify how health and social care practitioners can influence health outcomes at an
individual, family and community level. Students will be able to work with people with long
term conditions to plan and facilitate supported self care in response to assessment of
individual need.

6. Indicative content.
The subjects covered include:
• Definition and perceptions of long term conditions
• Methods of identification of people with long term conditions
• The social, psychological and political influences on long term condition management
• Issues for people living with chronic disease and for their carers- patient
autonomy/supported self care, cultural issues/diverse belief systems
• Impact of co morbidities, poly-pharmacy,
• Issues for end of life care, ethics, access to care
• Organisational issues: the role of case management, pathways of care, local systems,
audit, expert patient programme
• Professional issues- interagency working, accountability and clinical governance
• Developing and implementing individual care plans
7. Assessment strategy, assessment methods and their relative weightings
Students are assessed by written assignment.
Assignment
Students are required to submit a fully referenced case study of 3,000 words. Drawing on their
learning from the optional disease focus modules students must demonstrate an understanding
of complex case management and illustrate an in-depth evaluation of the issues for patient,
carer and multidisciplinary team.
The assignment component comprises 100% of the overall module mark.
Criteria for a pass
The pass mark for the module is 40%.
Reassessment is as per the regulations in the Student Guide.
