Contractual Employment Opportunity
The Fountain of Health Initiative seeks a coordinator or coordinators for two related projects
using the Fountain of Health materials in two community settings. One project involves
supporting a pilot group of family physicians in using the FOH materials to better the mental
health of their patients. The second project will work with a small group of seniors in a peer
education model to promote mental wellness and positive ageing with seniors in community.
Both projects will operate between March 2015 and March 2016. Projects may be stand alone or
combined into one contract, depending on candidate qualifications and interest. To find out

Coordinator –Fountain of Health Family Physician Project
Main Responsibilities:






Develop a physician guide for use with already existing Fountain of Health materials in
consultation with family physicians and project advisory group.
Assist in the development of an evaluation framework and evaluation tools
Coordinate the production of a short role play video as part of the physician guide materials
Recruit 8-10 family physicians to take part in the pilot with assistance from the project advisory
team and Fountain of Health members
Implement and monitor the project in cooperation with the family physicians involved.
Coordinate project evaluation activities and prepare a project report
Skills required:






Project Management experience
Excellent communication skills
Background in health promotion, health education or related skills
Knowledge of seniors mental health issues and the Fountain of Health messaging
Experience working in a primary care setting
Valid driver’s license and ability to travel
Term of Contract:
This is a contract position to be carried out from a home office.
Project is to be completed between March 15, 2015 and February 28, 2016
Compensation: To be negotiated. Total value of coordinator contract $7,000-8000
Note: This contract may be combined with a related contract within the same time frame.
Coordinator –Fountain of Health Peer Leadership Project
Main Responsibilities:







Assist in the completion of peer facilitator packages using the existing Fountain of Health
materials as well as Powerpoint Presentations and speakers notes currently being developed.
Assist in the development of an evaluation framework and evaluation tools
Recruit and train senior peer leaders in the Fountain of Health presentation with assistance from
the project advisory team and Fountain of Health members
Coordinate several pilot sessions with a small selection of senior peer leaders and participants to
gain feedback on content and process. Make appropriate changes to the presentation package.
Coordinate the promotion of FOH sessions with senior clubs, senior serving agencies, and
community organizations’
Coordinate the delivery of 10-12 peer led sessions (Fall 2015)
Coordinate project evaluation activities and prepare a project report
Skills required:







Project Management and community development experience
demonstrated application of adult education principles
direct experience working with older adults in community setting
Background in health promotion, health education, or related fields
Knowledge of seniors mental health issues and the Fountain of Health messaging
Background in volunteer recruitment and management
Valid driver’s license and ability to travel
Term of Contract:
This is a contract position to be carried out from a home office
Project is to be completed between March 23, 2015 and March 18, 2016
Compensation: To be negotiated Total value of coordinator contract $16,000-18,000
Note: This contract may be combined with a related contract within the same time frame.
