Foreign Object Damage (FOD)
Reason
As in the last few weeks, many FOD occurrences have been reported by some INACA members
in several airports. It caused some engine damages, fuselage or flap punctures and resulted an AOG
for a few days. Based on the event happened, Soekarno-Hatta airport has the highest number of
FOD occurrences, some because of loosen of runway concrete.
There are also many occurrences of tire burst and unscheduled removal (premature removal) due to
FOD reported by the member as well in last few months.
This FOD event is serious hazard to the safety of aircraft operation. Therefore as part of our safety
concern, we shall issue a safety notice to address it in order to eliminate or reduce the safety hazard
into an acceptable level.
INACA Recommendation
Airport Operator
Airport Operator shall review their existing Quality System and Procedures related to the runway
maintenance and operation in accordance to National Regulation, ICAO Guidelines and SARP
(Standard and Recommended Practices). Whereas it is also clearly stipulated in Undang-Undang
Penerbangan No 1, Tahun 2009 Republik Indonesia, article 219 that the Airport Operator must
comply and be certified in accordance with national regulation.
The highly probable event of FOD in several airports indicates that the Quality Assurance System
in those airports do not effectively implement the system in preventing such occurrences. The
frequency, method and procedure of runway inspection need to be reviewed as well as the
qualification of the personnel and the facilities to perform the inspection.
We Suggest Airport Operator to review the condition of the existing runways and performed a full
audit concerning the Quality System, Procedure, Equipment or Facilities and Personnel in order to
determine the gap analysis to see the deficiency. While the result have to be shared to the aircraft
operators and inform them when a hazard is admitted for safety concern.
The Airport Safety Department has to perform a Safety Audit of the airport operation and more
proactive to indicate a hazard among the runway operation in preventing such event to happen in
the future.
Airline
The airline must review their FOD event in several airports and make a mitigation in order to
eliminate or reduced the hazard. The occurrence shall be recorded as accurate as possible to
establish a good statistical data and will be used to mitigate the airline operation at the specific

the Airport to address our concern on Safety, especially in relation with the FOD event with more
pay attention to the Airports whose have high FOD statistical occurrences.
