I have thoroughly read the enclosed Lifest 2015 Vendor/Exhibitor Policies and understand the guidelines are part of
this agreement
I have listed the items and services to be sold and/or promoted on the bottom of this page
I understand that I need to send a proof of insurance by 5/23/2015 with Life Promotions listed as additional insured
I understand that locations are reserved on a first-come-first-serve basis and that BOTH the completed application
and payment are needed to ensure my reservation
The applicant and individual agrees that all our personal property, supplies, equipment, and merchandise in or
around our exhibition/vendor booth area shall be at our sole risk and hazard of our operation. As a booth operator,
we agree to indemnify and hold harmless, Life Promotions, Inc., and Lifest, from any and all liability, damage,
expenses of any kind, cause of action, suits, claims or judgments arising from injury to persons or property within and
on the site of Lifest, which arises out of any act, or failure to act, or any alleged negligence of the booth operator, its
guests, employees, staff, volunteers or customers.
Life Promotions Inc, and Lifest require proof of the following Insurance Coverage & Limits, from each
applicant prior to admission to Lifest gounds. A Certificate of your Liability Insurance will be required.

I hereby accept these terms and conditions, and assume full responsibility for our booth, property & operations.
Further, I will fully comply with all points mentioned both in the above and the guidelines set forth by Life Promotions
Inc., and Lifest, for booth operation, and do hereby apply for a booth(s) at Lifest.
