Driving and Accident Record Declaration for Foreign Licences I, ___________________________________________________, as part of an application for a Zipcar membership do declare under penalties of perjury, that I have provided a valid passport and a valid driving licence. Additionally, I meet the following driving, accident and use requirements: > I am at least 21 years old > I have a valid driving licence and have been licenced for at least 1 year > I have had no more than two ‘incidents’ (moving violations PLUS accidents) in the past three years > I have had no major endorsements or violations in the past three years > I have had no alcohol endorsements or violations in the past five years Major violations: > Excessive speed (20+ mph over the speed limit) > Operating to endanger, reckless driving, etc. > Driving under the influence of alcohol or drugs > Leaving the scene of an accident involving property damage > Operating a motor vehicle with a suspended or revoked licence People excluded from membership: > Carriage of Goods for hire or reward (couriers, parcel delivery services) > Primary business of transporting people or operating a taxi or cab > Professional sports/entertainers > Motor Trade (taxi) Attention Foreign Licence Holders (this is for applicants with licences issued outside of the United Kingdom and European Union): Please note that it is a mandatory requirement for you to exchange your driving licence for a UK DVLA licence after you have lived in the UK for more than 12 months. By failing to be honest in signing it, you will be breaching the contract making the insurance invalid. 3) Zipcar has the right to terminate your membership at any time if you are found to have been dishonest in the signing of the declaration.
