1.
Introduction
1.1
The National Kidney Foundation (“NKF”) wishes to invite vendor (the “Vendor”) for the
provision of cleaning services with the provision of toiletries consumables & sanitary items,
such service as described in Scope of Service (the “Services”) to its five (5) new dialysis
centres in Singapore.
2.
Scope of Service
2.1
The Vendor must be registered with the Building and Construction Authority (BCA) under
work head “MW02” – Housekeeping, Cleansing, Desilting & conservancy Services.
2.2
The scope of service shall conform to the detail service requirement in Annex A, Annex A1A4.
3.
Contractual period
3.1
Contractual period: One (1) year period from July 2015 to June 2016.
4.
Submission of ITQ
4.1
The quotation submitted by the Vendor shall be as in “Price Schedule Part 1 and Part 2” Annex B. Full set of quotation must be submitted with vendor’s stamp on all pages stipulated
in the ITQ. The quotation may be submitted by hand or post in a sealed envelope and
endorsed with the words “Invitation to Quote Ref No NKF/CL/2015/005 – For The
Provision of Contractual Cleaning Services with the Provision of Toiletries
Consumables & Sanitary Items for 5 New Dialysis Centres”. All submission should be no
later than 27 April 2015, Monday, 3pm (the “Closing Date”) and delivered by:

The submitted quotation shall be irrevocable and open for acceptance by NKF for 90 days
from the Closing Date.

5. Terms and Conditions
5.1
The terms and conditions set out in Annex C1 - C2 shall form part of the binding contract
between the successful Vendor and the NKF. In the event of any inconsistencies between
the terms and conditions set out in Annex C1 - C2 and the terms and conditions set out in
this Invitation to Quote and/or the Scope of Services, Annex C1 - C2 shall prevail.

Price Quotations
6.1
All prices quoted by the vendor shall be in the lawful currency of the Republic of Singapore.
6.2
All prices quoted by the vendor shall represent the total cost to NKF.

Annex A
SCOPE OF WORK FOR NKF DIALYSIS CENTRE
1.0
FREQUENCY OF CLEANING
(Daily)
(a) To clean NKF plaque at the main entrance.
(b)
To sweep and mop all floor within the centre (dialysis areas required three times a day)
using 0.1% bleach solution.
(c)
To sweep the external areas, including corridor & aluminium grille area.
(d)
To mop each cubicle after each patient’s dialysis treatment using 0.1% bleach solution
with designated mop.
(e)
To empty all bins and clear all refuse, especially food refuse & Biohazard waste from
the dialysis centre. To put on Personal Protection Equipment (PPE) (ie. gloves, gowns,
proper foot-wear etc.) when clearing the waste.
(f)
To dust and clean floor mats.
(g)
To wipe and clean all tables, counters, furniture & office fixtures including door handles
(h)
To spot clean and remove all finger marks and smudges on wall and fixtures.
(i)
To descale toilets (including walls and floors) with approved cleaning agent and clear
the toilets twice a day.
(j)
To boil water and refill all kettles, flasks and dispensers.
(k)
To wash all cups and plates in pantry, keep pantry dry and clean.
(l)
To dispose carton cardboards into Bin Centre.
(m)
To clean & scrub the wash basin with bleach at Patient Area & Re-processing Room,
as & when necessary.
(n)
To refrain from sweeping of floor or any form of cleaning that generates dust (earth or
fine/dry particles) during patients’ meal times & during clinical procedures namely
starting and ending cycles of each dialysis treatment at Patient Treatment Area. Meal
times (approx. 30 to 45 mins per session) are defined as when snacks and beverages
are being served and consumed.

To damp dust all cupboards, shelves, edges, glass panels, etc.
(b)
To damp dust outside windows, ledges and cornices.
(c)
To clean all internal glass surfaces.
(d)
To clean store room.
(e)
To wipe and clean all painting frames, artifacts & etc.

To wipe and disinfect all telephones.
To maintain the water feature and courtyard debris.

To mop/ wash the external areas including corridor.

To clean fridge at Tea Room area.

To clean & dust all display items (eg. wooden artifact, glass artifact etc.).
(Monthly)
(a)
To carry out highdusting of the centre within one (1) day.
(b)
To damp dust all high unreachable areas, including air-conditioning units, televisions,
beam lights, ledges, window blinds, all external exhaust air grilles etc.
(c)
Thorough cleaning of all washrooms including wall tiles and wash basins.
(d)
To clean and wipe all window panels and ledges (internal and external).
(e)
To scrub and high jet all perimeter tiled area (external).
(f)
To clean main signages and any other signages, ceiling tiles & exhaust fans.
(g)
To clean and wash rear of block area including waste storage & CU area especially
CU door & door handle.
(h)
Wiping of all furniture and fixtures, including cabinets, lockers, wall fixtures, television,
etc.
(i)
Wiping & polishing of stainless steel cabinets / compartments and wash basins.
(j)
Thorough scrubbing of floor tiles with appropriate cleaning chemicals (internal).
(k)
To remove lamp cover and clean fitting.
(l)
To clean & dust all light holder corners.
(m)
To clean & high jet the Bio-Hazard Waste Room and other vent rooms.
(n)
To clean/flush dialysis machines’ drain lines.
(o)
To clean shoe rack and General & biohazard bins in the DCs.
Note : All works for monthly highdusting are to be carried out on Sunday and to complete
within a day.

An indicative Daily Task List for Cleaner (AM & PM Shift) is enclosed at Annex A1. This is for
the purpose of reference only and varies among dialysis centres.
5.0

The list of provision of washroom items & dispensers for each NKF Dialysis Centre is
enclosed at Annex A2. The estimated toiletries indicator for reference is enclosed at Annex
A3.

Annex C1
TERMS & CONDITIONS
The terms and conditions set out in Annex C1-C2 shall form part of the binding contract between the
successful Contractor and the NKF. In the event of any inconsistencies between the terms and
conditions set out in Annex C1-C2 and the terms and conditions set out in the ITQ, Annex C1-C2
shall prevail.

INSURANCE COVERAGE

The Contractor is responsible to ensure that all foreign workers employed to work in
the NKF premises are in possession of valid work permits. Failing which the
Contractor is liable of all cost including legal charges or whatsoever imposed by
MOM.
SERVICE CONTRACT BETWEEN CONTRACTOR AND ITS WORKERS
3.1
The Contractor’s service contract with its cleaners must be in compliance with
Singapore’s employment laws such as the Central Provident Fund Act, Employment
Act and Workmen’s Compensation Act.
3.2
The Contractor shall declare whether they comply with the Singapore employment
laws, and declare any fines or convictions by the Ministry of Manpower over the last
2 years. Where a breach in employment laws relating to this service contract is found
to occur, monetary penalty shall be imposed on the Contractor. NKF reserves the
right to terminate the service contract if the Contractor had repeated breaches in
employment laws.
WORKPLACE SAFETY AND HEALTH

Contractor is required to provide the necessary insurance coverage such as
Workmen Compensation and Public Liability.
The Contractor shall at all times observe and comply with the Workplace Safety &
Health Act, all other applicable laws, regulations and requirements of any
government or regulatory authority relating to safety, and shall bear all costs and
expenses connected with the observance and compliance of the same.
TERMINATION OF CONTRACT
Both parties may terminate the contract by giving three (3) months’ written notice.
6.
PAYMENT
Monthly payment for services rendered will be paid upon satisfactory completion of the
cleaning works. If the Contractor fails to perform the Services or provide any Deliverables by
the date(s) specified in this Contract or otherwise in accordance with the requirements of this
Contract, NKF shall, in addition to any other remedies which it may have under this Contract
or otherwise, have the right to:
i. cancel all or any such items of Services from this Contract without being
liable therefore in damages, obtain them from other sources and deduct all
increased costs thereby incurred from any moneys due or to become due to
the Contractor or recover such increased costs from the Contractor as
damages; or

ii. deduct from any moneys due or to become due to the Contractor or require
the Contractor to pay, as and for liquidated damages a sum $200/- per
missing cleaner per day and/or an amount of $350/- per missing supervisor
per day.
7.
LABOUR DEPLOYMENT
MONTHLY HIGHDUSTING
The once a month periodic cleaning (highdusting) on a Sunday at Dialysis Centre has to be carried
out by a team of at least four (4) cleaners and guided by a cleaning supervisor.
DAILY DEPLOYMENT
Monday to Saturday (inclusive of Sunday/ Public Holidays or Six day per week where applicable)
:
1st Cleaner
7.00am to 3.00pm (Lunch - 1 hour)
:
2nd Cleaner
3.00pm to 11.30pm (Dinner - 1 hour)
:
Sundays
Rest Day
NOTE
:
You may be requested to supply labour on Sunday (from 7.00 am to
11.30 pm) in the event that the Centre operates on Sunday.

HEPATITIS SCREENING
All Cleaners would need Hepatitis screening on a yearly basis and if the
antibodies are less than 10 mIU/ml, they would require immunisation. This is
MOH guidelines. The blood test and immunisation charges will be borne by
the cleaning contractor. The Hepatitis screening report and the cleaners’
Identity Cards have to be submitted to the Nurse Manager for verification.
(a)
The Cleaner is advised to work for 1 shift only in the dialysis centre.
(b)
Deployment of cleaners on Public Holidays is at no additional charge.
(c)
The Client reserves the right to change the working hours, in instances like the Chinese Lunar
New Year period where public transport claims (ie. taxi claims) will be borne by the Contractor.
(d)
Cleaners are to report to the Nurse Manager of the Dialysis Centre and take all instructions from
the Nurse Manager.
Contractor is to inform the Nurse Manager of any replacement of cleaner.

The replacement shall be briefed by the cleaning supervisor on his/her duties.
(For any new replacement, the cleaning supervisor shall be on-site to guide and train the new
cleaner.
The cleaning supervisor is to report to Nurse Manager at least on a weekly basis to ensure all
instructions are adhered.
The cleaning supervisor is to maintain record of DC visit and follow up action to ensure quality &
compliance to requirement.
