Registered Nurse - FACT Peel + Program The Canadian Mental Health Association/Peel Branch is recruiting for a Registered Nurse to provide nursing support in a case management role to a caseload of young adults (16 to 35 years old) experiencing mental health and addiction issues. The FACT PEEL PLUS Program supports the recovery of young adults experiencing mental health and addictions issues and their families. The early phase of these issues is a critical period which provides an opportunity to minimize disruption of school, social, and employment goals. There is a primary focus on individuals who are experiencing psychosis. Service is provided in partnership with the client, family, and relevant community agencies. Limited nursing coverage will be provided to ACT clients. This position is a full time permanent position (37.5 hours per week) and located at our 60 West Drive office in Brampton. Key Responsibilities: To provide nursing and case management support: 1) Providing case management support for up to 20 clients and their families. 2) Delivering psycho-educational groups for clients, families and other professionals re: Mental health concerns around physical health, medications, etc. 3) As part of multidisciplinary team, providing the team, client, family, and others with specialized consultation/education related to nursing issues within a recovery model of intervention. 4) Working effectively and collaboratively with the EPI team and other services, internal and external to CMHA/Peel. 5) Provide limited nursing coverage to ACT clients to support holiday coverage (medication monitoring, injections, health teaching). 6) Collaborating with appropriate partner agencies as required. 7) Maintaining appropriate record-keeping/documentation and client files, statistical and outcome measurement tools as required by CMHA/Peel policies and procedures. 8) Participating on agency and community committees and workgroups as requested by manager. Qualifications: 1. 2. 3. 4. 5. 6. 7. 8. Current registration as a Registered Nurse with the College of Nurses of Ontario. Bachelor’s degree in Nursing (BScN) or Social Sciences is preferred. Certification in psychiatric/mental health nursing CPMHN (c) is an asset. A minimum of 2 years’ experience with young adults experiencing mental health and addictions issues. Community nursing experience is required. Working knowledge of Recovery Model. Excellent oral, written communication, organizational and problem solving skills. Demonstrated ability to work independently and as part of a multi-disciplinary team. 9. Ability to work with the diverse client group, considering issues of equity and from an LGBQT positive framework. 10. Proficient computer skills including MS Office, Outlook and Internet. 11. A satisfactory Criminal Record check, valid “G” Driver’s license, relevant insurance and access to a reliable vehicle is required.
