Audience
Health care providers committed to preventing influenza, pneumococcal disease and other vaccine -preventable diseases primary care, managed care, long term care, public health, occupational health, insurers and others are welcome.
Purpose
The purpose of the conference is to increase knowledge about the impact of influenza, pneumococcal disease and other
vaccine-preventable diseases and how to develop, implement and evaluate related prevention programs.

The Minnesota Landscape Arboretum is located about 4 miles west of the intersection of Hwy 5 and Hwy 41. Take I-494
to Hwy 5 (exit 11c). Follow Hwy 5 about 9.25 miles past Chanhassen, past the intersection of Hwy 41. Watch for the pale
gray entrance sign on the left. Be careful as you turn left from Hwy 5 onto Arboretum Drive. Parking is included in the
registration fee so proceed to the Oswald Visitor Center. The parking lot is located to the right of the building.

The Minnesota Coalition for Adult Immunization has reserved a block of rooms for Thursday, April 9, at the rate
of $110.00 plus tax at the Holiday Inn Express Hotel & Suites in Chanhassen. Call the hotel directly at 952-401-8850, by
Friday, March 27, to reserve a room. The block name is MCAI and includes both 1 King and 2 Queen rooms.
Please note that check-in time is 3:00 pm on the day of arrival and check-out time is 11:00 am on the day of
departure. Guests can enjoy free hot continental breakfast each morning, free Wi-Fi, free local and national newspapers
and an indoor pool, hot tub and 90-foot waterslide! There are also free guest passes to the Lifetime Fitness. A
complimentary shuttle is available daily from 5:00pm - 9:30pm.
The hotel is located on the southeast corner of Hwy 5 and Century Boulevard, east of the Arboretum. Take Hwy 5 to
Century Boulevard and turn south. Take a left into the parking lot.

Registration Deadline
Monday, April 6. A refund, less $30, is available prior to Friday, April 3. Final counts must be submitted to the Arboretum
on Friday, April 3. Substitutions are encouraged.
