How to Complete The HealthQuest Profile
The HealthQuest Profile is one component of the CBFit4Life wellness program. Taking the HealthQuest Profile (HQP)
is an important step towards leading a healthier lifestyle. The HealthQuest Profile is completely confidential and your
answers will not be shared with your employer nor change your health insurance coverage in any way. Completing
the HealthQuest Profile is one requirement to receive preferred rates on medical contributions. For complete details
on qualifying for preferred rates, please see the CBFit4Life program FAQ’s found on the HR4U Portal.
Log In
Go to www.healthplus.org.
A. If you have not registered on the
HealthPlus website, click “Register Now!”
found in the Log-In box. Then follow the
registration instructions.
Or …
B. If you have already registered on the
HealthPlus website, login using your
User Name and Password. (If you have
forgotten your User Name and Password
call HealthPlus Customer Service at
1-800-332-9161.)
Or …
C. If you do not have HealthPlus medical
coverage and would like to complete the HQP, please send an email requesting a User Name and Password to
cbfit4life@citizensbanking.com.

A. To register, you will need your HealthPlus
ID card. Begin by entering your HealthPlus
ID#. This number is 11 characters long,
begins with H and ends with a two digit
code* such as 01.
n Fill in the remainder of the required
information.
n You must enter an email address.

Access Your Wellness Program
A. After you log in, click “Your Wellness
Program”.

Begin Your HealthQuest Profile
A. Click on “Get Started” in the Take Your
HealthQuest Profile Box on your home
page.
A
Once you have completed your questions,
click on “Finish Now” to receive your
report with your wellness score. You are
able to print a copy of this report for your
records.
Once you have successfully completed
your HealthQuest Profile, your home page
will indicate you have earned 5 credits and
under the “Track Wellness Credits” page,
the HealthQuest Profile will be listed at the
bottom as a completed activity.
