At CCDPH, interns have a unique opportunity to build on knowledge learned in school and gain real-world experience at one of the largest nationally-accredited public health departments in the nation. CCDPH serves suburban Cook County: a complex jurisdiction comprised of 129 municipalities and a diverse population of 2.4 million residents. Our interns obtain experience working in the field of public health under the close supervision of leading public health professionals with specialties in the following areas. Click on your area(s) of interest to see what types of internships are available within each department: In this manual, you will find step‐by‐step instructions for how to apply for an internship online. Interns are placed based on skills, coursework and essay quality. For full consideration, all applicants must complete an online application form, and attach an essay, resume/curriculum vitae (CV) and list of references. Please read and follow all instructions in the step-by-step guide. If you have any questions, please call the Office of Administration, 708-633-4016. Upon acceptance, applicants will be required to complete the CCHHS Human Resources Infection Control Form and a background check. Internships are available year-round and applicants can select from summer, fall and/or spring trimesters. Eligibility Requirements All applicants must complete all parts of the application process and be: • • • Enrolled (currently) as a matriculated undergraduate, graduate, or professional student in a degree program at an accredited U.S. college or university. In good academic standing, students must have a minimum GPA of 3.0 and above. A Cook County resident. Allow at least 1 month to prepare/apply, and 2 to 3 weeks to find out if you have been accepted. You must complete the following tasks before you can begin your internship. ¨ 250-word essay (single-spaced, 12 pt. Times New Roman font) describing your area of study and career goals and how you think an internship at Cook County Department of Public Health will help you achieve those goals. Please also describe your area(s) of interest and how you think your education and any previous work/internship experience will benefit CCDPH. (See descriptions listed on the website at www.cookcountypublichealth.org/internships and talk with your preceptor about which department would be most appropriate). This essay is required and can be uploaded with your application form via the website. ¨ Resume/curriculum vitae (CV) and references. These documents can be uploaded with your application form via the website. ¨ Consider what schedule you would like based on other work, school and family commitments. Online Internship Application Form (see pp. 4 and 5 of this document for instructions). Include essay, resume/CV and references. ¨ ¨ IMPORTANT NOTE TO INTERNATIONAL STUDENTS. It is important that you consult with the International Student and Faculty Office (ISFO) before you begin your internship. The federal laws governing your eligibility to participate in internship are complicated and change from-time-to-time. Your immigration status is your responsibility. Go to http://www.cookcountypublichealth.org/internships 2. Review areas of interest. 3. At the bottom, you will see the section “On-line Application Form” and click on the link. Complete form and then click “submit” button. Using the “upload” feature, please be sure to attach your resume or any other supporting documentation to the application, such as references. 5. You will receive an email confirmation that your application has been received. 6. Please allow 2 to 3 weeks for your application to be reviewed. CCDPH may be contacting your references and request a phone or in-person interview with you to discuss your interests and placement in an internship position. Ø CCHHS Infection Control Form (available at: www.cookcountypublichealth.org/files/forms/cchhs-hrinfection-control-form.pdf) CCDPH is a professional workplace. All staff and interns are expected to dress and behave professionally at all times, whether in the office or the field. • Be on-time and present for all scheduled work days. • Follow your pre-determined schedule unless otherwise discussed with your site supervisor. • If you need time off because of illness or other reason such as vacation or emergency, you must get advanced approval or call your internship supervisor by 8:30 a.m. the day you will be absent. An email is not appropriate notification that you are not coming in on a previously scheduled day. • Each day you work in the office, you are required to sign in and out. • Due to the nature of programming, some evening or weekend hours may be required. Depending on the nature of your project, you may work outside of normal business hours. Please consider other school and/or work schedules when determining your internship project.
