Contiguous Property - Property owned or leased and occupied by a customer, which is not

1.1.
The rules and regulations contained herein apply to intrastate service and
facilities furnished within the State of Arkansas by Yelcot Video Group,
hereinafter referred to as the "Company", as may be subject to the jurisdiction of
the Arkansas Public Service Commission (the "Commission".)
1.2.
When services and facilities are provided in part by the Company and in part
by other companies, the rules and regulations of the Company apply to that
portion of the service or facilities furnished by it.
1.3.
In the event of a conflict between any rate, rule, regulation or provision
contained in this tariff and any rate, rule, regulation or provision contained in
the tariffs of another company or companies in which the Company concurs,
the rate, rule, regulation or provision contained in this tariff shall prevail.
UNDERTAKING OF THE COMPANY
2.1.
The Company does not undertake to transmit messages, but offers the use of
its facilities, where available, for communication between parties subject to the
terms and conditions specified in these tariffs.
2.2.
The Company's obligation to furnish, or continue to furnish telecommunications
service is dependent upon its ability to obtain, without unreasonable expense, retain,
and maintain, suitable rights-of-way and facilities, and to provide for the installation
of those facilities required to furnish and maintain that service.

3.1.
Applications for service, or requests from customers for additional service or
changes in the grade or class of service become contracts when received and
accepted by the Company and are subject to the minimum contract term,
which is one month unless specified otherwise.
3.2
Any general change in rates, rules or regulations prescribed by the Commission
modifies the terns and conditions of contracts to the extant of such change.
PROVISION OF EQUIPMENT
4.1.
Generally, all equipment necessary for the provision of any given service will be
furnished and owned by the customer, except where the Company continues to
lease equipment on a deregulated basis.
4.2.
Customer-owned terminal equipment and communications system may be connected
with the facilities furnished by the Company pursuant to the provisions of this tariff.
In these instances, the Company will take all reasonable precautions
to assure that the telecommunications network is not exposed to harmful or
hazardous voltages as a result of interconnection with customer-owned equipment.
RESPONSIBILITY FOR CHARGES
5.1.
The customer is responsible for all charges arising out of the use of or in conjunction
with any services furnished to him by the Company, including collect
toll messages which have been accepted at the customer’s telephone number.
5.2.
Monthly recurring charges will be billed in advance and toll charges are billed in
arrears.

CANCELLATION OF APPLICATION FOR BASIC SERVICE

Issued:
When the customer cancels an application for service prior to the start of
installation of service, or prior to the start of special construction, no charge
applies. When installation of service has been started prior to the cancellation, a
cancellation charge equal to the minimum service charge shall apply. Where special
construction has been started prior to the cancellation, a charge equal to the cost
incurred in the special construction, less net salvage applies. Installation
or special construction for a customer is considered to have started when the
Company incurs any expense in connection with the installation which would not
otherwise have been incurred.
MINIMUM CONTRACT PERIOD
7.1.
Except as specified elsewhere in this tariff, the minimum contract period is one
month from the date service or additions to service are established, and the
minimum charge is the authorized rate for one month. For purposes of rate
administration, each month is considered to have thirty (30) days.
7.2.
The Company may require a contract period longer than one month at the same
location in connection with unusual construction necessary to meet the specific
demands far service.
CUSTOMER TERMINATION OF SERVICE
8.1.
The customer may terminate service prior to the expiration of the initial contract
period after giving notice to the Company pursuant to Rule 6.20 of the
Commission's General Service Rules. The customer will be liable for all charges
for the full initial contract period, plus all charges due for toll or other services
which have been furnished.
8.2.
The customer may terminate service after the expiration of the initial contract period
after notifying the Company pursuant to the provisions of Rule 6.20.

COURT-ORDERED DISCONTINUANCE OF SERVICE

The Company will discontinue service upon receipt of an order from any court
directing the Company to discontinue such service. The Company shall give
notice to the customer at the time of the discontinuance of service of its reasons for
doing so, said notice to be accompanied by a copy of the order from the court.
The resale of any service provided by the Company is not permitted except as
provided elsewhere in this tariff or as specifically authorized by the Company.
DIRECTORIES
11.1
General
11.1.1. AT&T and/or CenturyLink, as the ILEC directory publisher, will
distribute one directory without charge to the Company’s customers.
11.1.2. The liability of the Company for any error or omission in these ILEC
directories shall not exceed the amount of actual damage suffered, and in
no event shall exceed an amount equal to the number listing service
charge to the customer for the period during which the directory contained
the error or omission in the last published directory of the exchange.

ALTERATIONS
12.1. The customer agrees to notify the Company promptly whenever alterations or new
construction on premises owned or leased by him necessitate changes in the
Company's facilities, and the customer agrees to pay the Company's current charges
for such changes.
13.
CUSTOMER SERVICE - USE OF
13.1.

Customer telephone service is furnished only for use by the customer. Business rates
apply whenever the use of the service is primarily or substantially of a commercial,
professional, institutional, or otherwise occupational nature, or where the listing
required is such as to indicate business use. Residential rates apply when the use of the
service is of a domestic nature, provided that service is not used substantially for
occupational purposes.
LIABILITY OF THE COMPANY
14.1. Given the customer's exclusive control of his communications over Company
provided facilities, and of the other uses for which Company facilities may be
furnished, and because errors incident to the service and the use of facilities are
unavoidable, the services and facilities furnished by the Company are subject to the
terms, conditions and limitations specified herein.
14.2
Issued:
The Company's failure to provide or maintain facilities under this tariff shall be
excused by labor difficulties, governmental orders, civil commotions, acts of God
and other circumstances beyond the Company's control, subject to the interruption
and outage allowance provisions of the Commission's General Service Rules.

Defacement of Premises - No liability shall attach to the Company by reason of any
defacement or damage to the customer's premises resulting from the existence of the
Company's facilities on such premises, or by the installation or removal thereof,
when such defacement or damage is not the result of the negligence or willful act of
the Company or its employees.
14.4
The Company's liability, if any, for its gross negligence or willful misconduct is not
limited by this tariff. With respect to any other claim or suit, by a customer or any
others, for damages arising from errors or omissions in accepting listings as
presented by customers or prospective customers, the Company's liability, if any,
shall not exceed the amount paid for number listing service during the period
covered by the directory in which the error or omission occurred as more specifically
addressed in the section of this tariff entitled “Directories”.
14.5
Indemnification - The Company's liability, if any, for its gross negligence or willful
misconduct is not limited by this tariff. With respect to any other claim or suit, by a
customer, the customer indemnifies and holds harmless the Company against claims,
losses or suit for injury, death, or damage to any property which arise from:
14.5.1.
The use of customer-provided premises equipment, voltages or
currents transmitted over the Company's facilities caused by
customer-provided premises equipment.
Further, the customer indemnifies and holds harmless the Company against claims
for libel, slander, or the infringement of copyright arising directly or indirectly from
the material transmitted over the Company's facilities or the use thereof by the
customer; against claims for infringement of patents arising from, combining with or
using in connection with, facilities furnished by the Company and apparatus,
equipment, and systems provided by the customer.

14.6. Liability - The Company's liability, if any, for its gross negligence or willful
misconduct is not limited by this tariff. With respect to any other claim or suit, by a
customer, for damages arising out of mistakes, omissions, interruptions, delays or
errors, or defects in-transmission occurring during provision of telephone service, the
Company's liability, if any, shall not exceed an amount equivalent to the
proportionate, charge to the customer for the period of service during which such
mistake, omission, interruption, delay, error or defect in transmission or service
occurs and continues. This liability shall be in addition to any amounts that may
otherwise be due to the customer under this tariff as an allowance for interruptions.

Telephone Number Retention (Number Portability)
Telephone Number Retention is an optional feature by which a Customer, who was
formally a Customer of another local exchange carrier at the same premises location,
may retain his/her telephone number(s) for use with the Company-provided
Exchange Services. Telephone Number Retention service is only available in areas
where the Company maintains some form of number retention arrangement with the
Customer’s former local exchange carrier.

16.
PROMOTIONAL RATES AND CHARGES
16.1.
The Company may, during certain specified periods, offer promotional rates for the
purpose of encouraging customer interest in and evaluation of a particular service
offering.
16.2.
A promotional rate is defined to be a temporary reduction or waiver of the monthly
recurring rate and/or nonrecurring charge of a tariffed service.
16.3.
The Company will notify the Commission 30 days in advance, by letter in Docket
No. 86-033-A, specifying the service involved, the terms and location of the
promotion, the dates of the promotion, and referencing Docket NO. 95-1 69-TF.
16.4. Promotional rates shall be applied only to new or additional units-of service ordered
during the promotional rate period.
17.
TERMS OF PAYMENT
17.1. A customer's bill is payable upon receipt and is due 22 days after the date the bill is
mailed. A late payment will be applied to payments made after the close of business
on the due date pursuant to Rule 5.06. These terms of payment will apply to all of the
Company's tariff schedules.

ARKANSAS TELECOMMUNICATIONS RELAY SERVICE SURCHARGE (TRS)
6.1.
On December 31, 1991, the Commission entered Order No. 4 of Docket No. 91-051U requiring all Arkansas Local Exchange Carriers ("LECs") to collect a surcharge
per month per access line from all customers effective April 1, 1992. The surcharge
is a source of funding for the design and implementation of Title IV of the
Americans With Disabilities Act ("ADA") through the establishment of a non-profit
corporation, Arkansas Relay Services, Inc., to select and oversee the operations of a
vendor to provide Telecommunications Relay Service ("TRS") for customers in
Arkansas.
6.2.
The Company concurs in the tariff rates and charges for TRS approved for
Southwestern Bell Telephone Company in their General Exchange Tariff and in any
future amendment or revision to such tariff as approved by the Commission.
6.3.
The rate applies for each exchange access line up to 100 lines per customer location.
TELECOMMUNICATIONS EQUIPMENT FUND SURCHARGE (TEF)
7.1.
Act 501 of 1995 requires all Arkansas Local Exchange Carriers (LECs) to collect a
surcharge per month per access line from all customers. The access line surcharge is
a source of funding to provide telecommunication devices for persons who are deaf,
blind or speech impaired. The access line surcharge levied under this Act shall be
collected by the LECs from their customers and deposited as special revenues in the
State Treasury to the credit of the Telecommunications Equipment Fund.
7.2.
The Company concurs in the tariff rates and charges for TEF approved for
Southwestern Bell Telephone Company in their General Exchange Tariff and in any
future amendment or revision to such tariff as approved by the Commission.

1.2.2. Listings will be limited to such information as is necessary for proper

1.2.3. Listings without address or without house number will be provided for
customers who desire such a listing.
1.2.4. The listing consists of one line. When use of abbreviations impairs clarity and
identification, a second line may be used with additional charge.
1.2.5. The Company may refuse to insert any listing which, in its judgment, does
not facilitate the use of the directory.
1.2.6. An additional listing must include the same address and telephone number as
the primary listing except that a different address may be shown for offpremises PBX or Centrex stations located on other premises occupied by the
customer.
1.2.7. Additional listings may be furnished with residence service for others who
are members of the customer's domestic establishment and who occupy the
same premises for a fee.
1.2.8. The rate for a foreign listing will be the rate of the local exchange carrier in
whose directory the listing appears. A foreign listing may be furnished for
customers requesting their listing be included in a white page directory alpha
section different than normal.
1.2.9. Non-published number service is an arrangement where a customer's listing
is omitted from both the telephone directory and information listing. A
monthly recurring fee applies.

1.2.10. The Company will take reasonable precautions not to publish the number
in the publicly distributed directories and, except when required by law,
will not disclose the number to any person other than representatives of
law enforcement agencies, its own employees or representatives or those
of other telephone companies responsible for publishing the directory.
1.2.11. Non-listed service is an arrangement whereby a customer's number is
omitted from the telephone directory but not from the information records.
1.2.12. When non-published or non-listed service is to be furnished, the customer
may be required to execute an agreement which holds the Company
harmless from any damages which might arise and which absolves the
Company from any responsibility for the failure of the customer to receive
calls because of the non-published or non-listed arrangement.
1.2.13. Non-published and non-listed service customers will be required to
maintain such service until the first issuance of a directory in which a
requested change may be published, or until telephone service is
discontinued.
1.2.14. Billing for such services will be discontinued with the last regular bill
dated before the issuance of the directory in which the change is made.
1.2.15. Non-published or non-listed service customers may not change from one
to the other without incurring a change charge.
1.2.16. The Company's liability arising from errors in or omission of directory
listings shall be limited to and satisfied by a refund not exceeding the
amount of the charges for such if the customer's service is affected during
the period covered by the directory in which the error or omission occurred.
In no event shall the Company be responsible for any consequential damages
arising out of any error or omission related to directory service.

DIRECTORY ASSISTANCE SERVICE
2.1.
GENERAL
2.1.1. The Company furnishes Directory Assistance Service whereby customers
may request assistance in determining directory information in the same local
calling area or in the same Long Distance Numbering Plan Area.
2.1.2. A customer request for directory assistance is any call to 411 or 870-5551212 directory assistance attendant.
2.1.3. A maximum of two telephone numbers may be requested per call to a
directory assistance attendant.
2.1.4. Charges for Directory Assistance Service are not applicable to calls
placed from customers who have physical, visual, mental or reading
handicaps that prevent them from using the telephone directory. The method
of exempting those handicapped customers shall be via the completion of an
exemption form supplied by the Company and the Company's acceptance of
that form.
2.2.
RATES
2.2.1
For customer direct dialed calls to a directory assistance attendant, a
charge of $ 2.19 per call is applicable.
2.2.2. For all customer requests for directory assistance which are placed to a
directory assistance attendant via an operator, a charge of $1.00 per call is
applicable (maximum of two requested numbers per call).
2.2.3. Directory Assistance charges billed to a telephone credit card, or a telephone
number other than the originating number via an operator, will be billed at
$ 2.25 per call rate.

2.3.1. DESCRIPTION
National Directory Assistance Service is provided to customers of the
Company for the purpose of requesting telephone numbers of individuals
Or businesses who are located outside the customer's local Directory
Assistance service area.
2.3.2. TERMS AND CONDITIONS
2.3.2.A. There are no cal1 allowances or exemptions for National
Assistance customers.
2.3.2.B. If a customer dials Directory Assistance for the purpose of
obtaining a National Directory Assistance listing, and also asks
for a listing within their local Directory Assistance service area,
the charge for National Directory Assistance applies.
2.3.2.C. A maximum of two requested telephone numbers are allowed
per call.
2.3.2.D. Charges apply to each call placed to National Directory
Assistance from a Public Access line.
2.3.2.E. In locations, including Public Access Lines, where the customer
has the capability to dial National Directory Assistance but
places a call to the National Directory Assistance service

and business customers.

3.1.
GENERAL
3.1.1. Operator Assisted Local Calling Service is provided upon customer
request in dialing a local number, requesting a local Person-to-Person call,
placing a local collect call, verifying the status of a line, and interrupting a
conversation already in progress.
3.1.2. Operator Assisted Local Calling Charges will not apply in the following
situations:
3.1 2.A. For calls to the Company for official telephone business.
3.7.2.B. Local calls from customers whose physical or visual handicaps
prevent them from making the call direct.
3.1.2.C. For emergency calls to agency type telephone numbers such as to
those agencies of the federal, state or local government which have
the capability and legal authority to provide aid in emergency
situations, i.e. 911.
3.1.2.D. When the caller advises he has had service trouble in reaching the
terminating number and the service trouble is verified by the
Company.

3.2.1 .C. The service charge in below applies to calls from customers who
request operator assistance in determining the status of a line,

3.3.1. The provision of Busy Interrupt involves an operator interrupting a
conversation in progress to ascertain willingness to establish conversation
with an alternate party.

1.
SERVICE CHARGE FOR CONNECTION, MOVE OR CHANGE OF SERVICE
1.1.
GENERAL
1.1.1. A service charge consisting of one or more charges specified herein is
applicable, as a non-recurring charge, to the ordering, installing, moving,
changing, rearranging and furnishing of telephone service. The term "service
charge" represents one of the following work functions necessary to perform
any of the activities described herein.
1.1.1.A. Service Ordering Charge - receiving, recording and processing
information in connection with a customer request for service or
equipment. One Service Ordering Charge is applicable per
customer request, per due date, per premises. Where both business
and residence service is furnished on the same premises, the "per
customer request" treatment is applicable separately for each service.
However, when multiple service orders are required for Company
reasons, only one Service Ordering Charge applies.
1.1.1.B. Central Office Line Connection Charge –work associated with
arranging an exchange line to provide service between the central
office and the customers premise and/or other premises where the
service is to be terminated.
1.1.1.C. Premise Visit Charge - when it is necessary to travel to the
customer's premises, one Premise Visit Charge is applicable per
customer request, per due date, per premises. However, in the
instance of a trouble report, the Company shall test the local loop
up to the standard network interface at no charge, pursuant to the
Special Rules - Telecommunications, Rule 4.05.

GENERAL
1.1.
Customer-provided circuitry and terminal equipment may be connected at the
customer’s premises to facilities furnished by the Company for use with individual
line exchange service in compliance with Part 68, Subpart B of the Federal
Communication Commission Rules and Regulations.
1.2.
The Rules and Regulations section of this Tariff, as well as the Rules adopted by the
Commission, also apply when the customer elects to provide his own
equipment.
RESPONSIBILITY OF CUSTOMER
2.1.
An access-line customer must subscribe to, and be capable of providing
operation for, sufficient quantities of access lines to provide adequate access to his
customer-provided equipment in accordance with accepted communications industry
standards.
2.2.
Upon notification from the Company that the customer-provided equipment is
causing or is likely to cause harm to any person or the system, or that such
equipment is causing or likely to cause interference with the system, the customer
shall make such change as is necessary to remove such harm or interference. Failure
to make such change will result in suspension of service in accordance with Rules
6.01 and 6.04 of the Commission's General Service Rules until such change is
completed to the satisfaction of the Company. Any connection of equipment by a
customer to the facilities of the Company that is not made in accordance with the
standards set forth in the most current issue of the American National Standard
Institute's National Electrical Safety Code and/or the National Electrical Code shall
be deemed likely to cause harm.

CUSTOMER-INSTALLED PREMISE WIRE
3.1.
Premise inside wire and jacks associated with residence and business line exchange
access service will be provided by the customer.
3.2.
Premises inside wire provided by the customer is connected to exchange access
service at the protector or the standard network interface as such devices are
commonly known within the industry.
3.3.
The protector or standard network interface is provided as part of the exchange
access service provided by the Company and will be installed at a location
determined by the Company which is accessible to the customer.
3.4.
The customer assumes the risk of loss of service, damage to property or death to or
injury to the customer or anyone acting on behalf of the customer when the customer
provides, maintains, or attempts to provide or maintain inside wire.
3.5.
The Company will promptly notify the customer when the service difficulty or
trouble results from customer provided insider-wire or jacks.

The Company is responsible for providing telecommunication service that
complies with standards of service set forth in the Commission's Special
Rules-Telecommunications and the Commission's General Service Rules. The
Company is not required, and does not warrant or guarantee, that any or all
types of customer-provided equipment is compatible and/or will operate or
properly function on the Company's facilities. It shall be the sole responsibility
of the customer to ensure that customer-provided equipment is compatible with the
Company's facilities that provide service to the customer, and the Company shall not
be responsible for any loss or damage due to or caused by the failure of customerprovided equipment to function on the Company's facilities.

The Company shall not be responsible to the customer for changes in the technical
criteria or in any of the facilities, operations or procedures initiated by the Company
or appropriate regulatory agencies which might render any customer-provided
equipment obsolete or otherwise affect its use or performance.
4.3.
The Company shalt not be responsible for the installation, operation or maintenance
of any customer-provided communications systems or equipment.
CONNECTION TO COMPANY FACILITIES VIA PROTECTIVE CIRCUITRY
5.1
Customer-Provided terminal equipment not conforming with Part 68 of the
Federal Communications Commission Rules and Regulations may be connected to
Company facilities for telecommunication service via protective circuitry provided
by the Company. The charges for such protective circuitry will be based upon costs,
including a return, and will be subject to approval by the Commission.
5.2.
The utility shall not be responsible for the through transmission of signals generated
by customer-provided equipment or systems or for the quality of, or defects in such
transmission, or the reception of signals by customer provided equipment or systems.

SECTION X- UNIVERSAL EMERGENCY NUMBER SERVICE 911
1.
GENERAL
Where requested by local government authorities, and subject to the availability of facilities,
the Company will provide a universal number “911” dialing to its Customers for simplified
emergency access police, fire and other emergency services. 911 calls will be routed for
answering to a Public Safety Answering Point (PSAP) as designated by the local
government unit authorized to establish and operate such systems. The Company does not
undertake to answer and forward 911 calls, but furnishes the use of its facilities to enable the
local government unit or its designee to answer and respond to such calls.
The 911 Calling Party, by calling 911 Service, gives consent for the Company to provide
911 information consisting of the name, address, telephone number, and other calling party
information when available, to Law Enforcement Agencies and other emergency service
providers on a call-by-call basis for the purpose of enabling those agencies or service
providers to respond to emergency calls for assistance. Database inquiries for 911
information consisting of name, address, telephone number and other information when
available, will only be allowed for purposes of dispatching or responding to 911 emergency
calls or integrity verification as prescribed by the applicable state codes, rules or legislation.
Customers with Unlisted or Non-published numbers as well as those customers who have
been requested per line blocking forfeits the privacy afforded by these services on calls
made to 911.
The provision of 911 Service by the Company shall not be interpreted, construed, or
regarded as being for the benefit or creating any Company obligation, either express or
implied, toward any third person or legal entity other than the customer. The Company’s
entire liability to any person for interruption or failure of 911 Service shall be limited to the
terms specified in this tariff by statute.

The Company will charge a fee of five dollars ($5.00) when a customer or any
authorized party requests a statement of the customer's account record as described
by the Commission's General Service Rules.
DEPOSIT FROM APPLICANT
3.1.
Under certain circumstances the Company may require a deposit from an applicant
pursuant to the provisions of the Commission's General Service Rules.
3.2.
The general amount of the deposit shall be equal to one month's bill for local
exchange service, plus two average bills for toll as defined in the Commission's
General Service Rules. Certain exceptions such as for unauthorized use, tampering,
bankruptcy or prior unpaid bills, may also be applicable.
DEPOSIT FROM CUSTOMER
4.1.
Under certain circumstances the Company may require a new deposit or an increase
in the amount of a deposit from a customer pursuant to the provisions of the
Commission's General Service Rules.
4.2.
When the Company charges a new or additional deposit, the total amount of deposit
at any time shall not be more than the total of the customer's two highest bills during
the last twelve (12) months, except in instances of unauthorized use, tampering, or
bankruptcy.

LATE PAYMENT CHARGE
9.1.
The Company will charge a late payment charge not to exceed ten percent (10%) of
the first thirty dollars ($30.00) of the customer's bill and two percent (2%) of the
remainder.
9.2.
The late payment charge will apply on the overdue portion of a customer's bill for
service. The due date for the Company's bills shall not be less than 22 calendar days
after the date the bill is mailed.
RECONNECTION FEE
10.1.

The Company will charge a reconnection fee of fifteen dollars ($15.00) when a
customer or other authorized party requests reconnection during normal working
hours (Monday through Friday 8:00 a.m. to 12:00 p.m. and to 1:00 p.m. to 5:00

the Company's close of business on the same day.

1.
GENERAL
1.1.
Digital Centrex Service is a Flat-rate business service with a 2 line minimum
requirement. It is a central office based service provided from suitably equipped
Telephone Company digital central office facilities. DTMF station signalling is
provided as a standard feature with this service.
1.2.
The equipment permits the direct dialing between lines connected to the service and
the direct dialing of outgoing calls. Incoming calls are received by direct inward
dialing from the calling party to the station line or through an attendant console.
1.3.
Digital Centrex Service consists of basic lines equipped with standard features, plus
a number of optional features. The standard features are included in Digital Centrex
Service. Optional features are offered subject to availability. Attendant consoles and
station equipment are to be provided by the customer, or can optionally be leased
from the Company.
1.4.
Digital Centrex Service will be offered under two basic packages:
Multiline Variety Package (MVP) for 2-6 lines;
Integrated Business Services (16s) for 2-2999 lines

The rates and charges shall apply for a minimum period of time of twelve (12)
months. The customer shall be obligated for the service selected for this minimum
period of time. Rates and charges for Digital Centrex Service are subject to
modification or change as authorized or directed by the Commission. If the customer
decides to discontinue service before the end of the twelve (12) month minimum
service period, a termination charge will apply as specified in paragraph 4.5.

Optional Lump Sum Payments
3.2.1. The customer has the option of making a partial lump sum payment (PLSP) or
total lump sum payment (TLSP) at the time of installation or at any other
time during the term of the service period selected.
3.2.2. The partial payment will reduce the Digital Centrex Service monthly rate per
line by the monthly annuity amount per line of the lump sum payment. The
monthly annuity amount is the annual amount divided by 12.
3.3.
Subsequent Additions
3.3.1. After initially subscribing to Digital Centrex Service with a particular number
of lines and set of features for the minimum service period of twelve (12)
months, a customer may add lines or features by placing an order with the
Company.

3.3.2. A customer ordering additional lines must specify the number of lines and the
features. The service period for the additional lines and features, if such
additional lines and features are ordered during the initial minimum twelve
(12) month service period, shall be for a minimum period of time equal to the
time remaining on the initial service period. The service period for the
additional lines may not extend beyond the end of the service period for the
lines in the initial subscription. Customers who add less than 100 lines shall
pay appropriate Package MVP or IBS I-IV rates. Customers who add 100 or
more lines shall pay the rates for those additional lines using the IBS V-IX
rates.
3.3.3. A customer ordering additional features for existing lines must specify the
features. The service period for the additional features, if such additional
features are ordered during the initial minimum twelve (12) month service
period, shall be for a minimum period of time equal to the time remaining on
the initial service period. The service period for the additional lines may not
extend beyond the end of the service period for the lines in the initial
subscription. The rates and charges for the additional features shall be those
in effect at the time the order is placed.
3.4.
Renewal Option
3.4.1. The customer has the following renewal option:
3.4.1.A. If the customer does not request discontinuance of service,
subsequent to the expiration of the minimum twelve (12) month
service period, service will be continued at the monthly rate currently
in effect and will be subject to all prevailing rules and regulations of
the offering.

REGULATIONS
4.1.
Digital Centrex Service is provided to customers with two or more lines. This
service is furnished subject to availability. In those cases where the Company
determines that additional equipment must be provided on the customers' premises,
the customer will be required to furnish the suitable space and environmental
conditions as determined by the Company.
4.2.
The Digital Centrex Service line rates for customers with 2-99 lines apply as
specified. Customers who subscribe to Digital Centrex Service for 100 or more lines,
and/or are more than 2 miles from their servicing central office, may at the telephone
company's option, be offered customer specific pricing on a contract basis, This rate
will be offered to the customer for its acceptance in writing, subject to Commission
approval. An individual service agreement will specify length of service and
applicable rate. With the exception of the customer specific rates, all other rates,
charges, rules and regulations, specified herein, continue to apply.
4.3.
One directory listing is provided without charge for each Digital Centrex Service.
4.4.
The monthly rate for Digital Centrex Service lines and features covered under a
monthly service period plan is guaranteed against Company-initiated change and will
apply for the selected service period. The minimum service period is 12 months.
4.5.
If the service is canceled in whole or in part by the customer after installation of the
service but prior to the completion of the service period, the customer shall be
obligated to pay the remaining payments, as contracted.
4.6.
The prepayment of charges in no way constitutes a purchase of the equipment, and
the Company retains full ownership of all equipment covered by the prepayment.
4.7.
Digital Centrex Service station lines may be terminated in a customer-provided PBX
system. Such lines may be either ground start or loop start and may have any
standard treatment. Transmission quality over Digital Centrex Service lines of this
type is not guaranteed. Additional transmission improvements requested by the
customer will be provided by the Company at charges based on cost.

4.8.
Station Message Detail Recording (MDR) is not represented to be a provision of
billing detail.
4.9.
When using Call Forwarding and Call Transfer, the Digital Centrex Service
customer is responsible for the payment of the tariff station-to-station charge for
each call connected over the exchange system between the Digital Centrex Service
and the telephone at which the call is answered.
4.10.
Unless specifically exempted, this service is subject to all general regulations
applicable to the provision of service by the company as stated in the general tariff.
4.11.
Trunk verification from a station line requires the establishment of a supplemental
line treatment.
DEFINITIONS
5.1.
Automatic Line - Provides an automatic connection between a calling station that
goes off-hook and a preassigned DN. This DN can consist of 1 to 16 digits.
5.2.
Call Forwarding, All Calls - Allows incoming calls (intragroup and DID) directed to
a station line to be routed to a user-defined line inside or outside the customer group
or the attendant.
5.3.
Call Forward, Busy - Permits incoming calls (originating from an outside group)
attempting to terminate to a busy station line to be redirected to a predetermined line
inside the customer group.
5.4.
Call Forward, No Answer - Provides for forwarding of incoming calls to a
predetermined line inside the customer group when the called station line does not
answer within a predefined ringing cycle.

5.5.
Consultation Hold - Permits a customer on an active call to retrieve a waiting call or
perform Three-way conference/Call Transfer just by flashing the switchhook. The
original call is placed in a temporary hold state.
5.6.
Call Hold - Allows the user to hold one call for any length of time provided that
neither party goes on-hook.
5.7.
Call Pick-Up - Allows a station line to answer incoming calls to another station line
within a defined call pick-up group. Call pickup is provided on individual station
lines within a customer group.
5.8.
Call Waiting - Allows an incoming call (within or outside the customer group) to
apply a call waiting tone on a busy station line which has been assigned the call
waiting feature.
5.9.
Cancel Call Waiting - A line option that allows a user, by dialing a code, to prevent,
on a per-call basis, any incoming calls from call waiting on his/her line.
5.10.
Class of Service - Provides the capability to allow or deny individual station line
features. The treatments can be arranged to control all calls originating or
terminating on station lines.
5.11.
Common Control Switching Arrangement (CCSA) Access - Enables station line
users in the customer group to gain access to the CCSA by using special access
codes and dialing reserved number exchange (RNX) patterns.
5.12.
Convenience Dialing - Allows MVP subscribers to place calls by dialing a speed
calling code from a shared list of up to 30 stored numbers (normally 20-49) instead
of dialing all digits of the desired number. A control station will add, change, or
delete telephone numbers from the list for the group.

DTMF Station Signaling - Permits station equipment to utilize dual tone
rnultifrequency signaling tones, instead of dial pulses to transmit called numbers,

Option 2 - Directed Call Pick-Up (DCPU)
(a) DCPU Non Barge-In - Permits a station line user to answer a call
that is ringing any other line within the same customer group by
dialing a code followed by the station number of the ringing line. If
the called station line has already been answered, the initiating station
line will be connected to a reorder tone.
(b) DCPU Barge-In - Permits a station line user to answer a call that
is ringing any other line within the same customer group by dialing a
code followed by the station number of the ringing line. If the called
station line has already been answered, the initiating station line may
barge-in to the answered call and be connected into a three-way call.
(c) DCPU Any Station - This is a terminating line option. A call to a
station assigned the DCPUA option can be picked up by any other
member, from any phone within the defined customer group.
(d) DCPU Barge-In Exempt - This is a terminating line option that
blocks any attempt by another station to barge-in.
(e) DCPU Exempt - This is a terminating line option that blocks any
attempt by another station to pick up a call by means of DCPU, either
barge-in or non barge-in.
Option 3 - Ring Again
(a) Ring Again - Allows a station line user calling a busy station line
to be automatically connected to the called line when the line
becomes idle.

Option 5 - Call Park
(a) Call Park (hardware arrangement only) - Allows a station line to
park a call against a specific number, attendant paging or at the

Option 6 - Call Forward Remote Access
(a) This feature allows customers to activate or deactivate the Call
Forward option on their telephone from a remote station. This will be
accomplished by dialing a dedicated directory number and personal

1.1.
Custom Calling Service consists of features which provide special kinds of
customer-controlled communications features on individual service lines. Custom
Calling Service is available only through central offices which, at the Company's
option, are appropriately equipped. Operating specifics may vary by central office
equipment type.

Custom Local Area Signaling Services (CLASS) is a group of capabilities that use
industry-standard protocols to effectively manage call flow. This is accomplished by
the central office deployment of a technology which routes the calling party’s
telephone number from the central office originating the call to the terminating
central office serving the called party. That telephone number is held in network
memory giving the called party options, including answering the call and calling
back to the originator. CLASS can be provided on a stand done basis or as an
enhancement to existing custom calling features to residential and business
customers in central office switches having the generic capability to offer CLASS,
The CCS7 signaling standard is used to extend CLASS services across multiple
nodes.
FEATURE DESCRIPTIONS

Auto Origination - Allows a customer a specific amount of time to dial a number

telephone normally, but to call a designated number by keeping the phone off hook
for the timeout period.

Telemarketer Do Not Disturb - When activated, callers are informed that the TDND
subscriber does not want to receive telemarketing calls. When the calling party
attempts to call a line with TDND, the caller receives a recorded announcement
indicating that non-telemarketing callers must dial the digit "1" to complete the call.
Once the “1” is dialed, the call is completed. If any other digit is entered the calling
party receives a busy tone.

Automatic Call Back - When activated, this feature automatically redials the last
number the customer attempted to call. If the called line is not busy, the call will be
placed.
2.10. Automatic Recall - This feature enables a customer to place a call to the telephone
number associated with the most recent call received, whether or not the call was
answered or the number is known.

Customer Originated Trace - This feature enables the customer to initiate an
automatic trace of the last call received. Upon activation by the customer, the
network automatically sends a message to a predetermined location (never to the
customer) indicating the calling and called numbers, the time the call was received,
and the time the trace was activated. Under no circumstances will the customer be
given the traced number. The results of the trace will be furnished only to legally
constituted authorities upon proper request by them.
2.12.
Selective Call Acceptance - This feature screens incoming calls against a list of
customer-specified directory numbers (DNs) and then accepts any calls from those
numbers. Maximum of 31 numbers screened.
2.13
Selective Call Forwarding - This is a call management feature that allows the
customer to transfer selected incoming calls to another telephone number. Maximum
of 31 numbers screened.
2.14.
Selective Call Rejection - This incoming call management feature allows the
customer to define a list of calling directory numbers to be screened. Maximum of 31
numbers screened.

2.15.
Distinctive Ringing - Call Waiting - This is an incoming call management feature
that provides special treatment for calls received from special telephone numbers.
When a call is received from one of the predetermined telephone numbers, the
customer is alerted with a distinctive ringing pattern. Calls from telephone numbers
not included on the screening list will produce a normal ringing pattern. If the
customer also subscribes to Call Waiting and a call is received from a telephone
number on the screening list while the line is in use, the Call Waiting tone will be
distinctive. Calls from directory numbers not included in the screening list will
produce a normal call waiting tone. Maximum of 31 screened numbers.
2.16.
Call Number Delivery (CND) - This is a feature that enables the customer to receive
the calling directory number on incoming calls. The calling number will be delivered
to the called party’s customer premise equipment. When CND is activated on a
customer’s line, the directory number of an incoming call will be sent to the display
of the customer premises equipment during the first long silent interval of the
ringing.
2.17.
Calling Name and Number Delivery (CNND) -This is a feature that enables the
customer to receive the calling directory name and number on incoming calls. The
calling name and number will be delivered to the called party’s customer premise
equipment. When CNND is activated on a customer’s line, the directory name and
number of an incoming call will be sent to the display of the customer premises
equipment during the first long silent interval of the ringing.
2.18.
Anonymous Call Rejection (ACR) – When activated, incoming calls are examined to
determine whether presentation of the calling party’s directory number is allowed or
restricted. If presentation is allowed or if the presentation status is unavailable, the
call is completed to the ACR subscriber. If presentation is restricted, the incoming
call is routed to the ACR denial recorded announcement or tone. The ACR
subscriber does not receive alerting tones for incoming calls that are rejected.

Installation charges apply to all new custom calling service connects.

When more than one feature is ordered on each exchange line, the combined rates of
the two or more features in the "2 or More Features" column is the monthly rate.
3.4.
The installation charge does not apply to outside moves.

The grade of transmission on calls forwarded and three-way calling may vary upon
the distance and routing required to complete such calls; therefore, the normal
grade of end-to-end transmission cannot be guaranteed on such calls.

Custom Calling Service may be provided on individual lines, Private Branch
Exchange (PBX) trunks and key business lines and may not be provided in
conjunction with coin telephone services.
3.7.
Remote Call Forwarding may not be terminated in a coin telephone.
3.8.
Identification of the originating telephone number will not be provided to Remote
Call Forwarding customers.
3.9.
Transmission quality may vary depending on the routing required, thus Remote Call
Forwarding is not represented as suitable for transmission of data or for being
forwarded again at the distant location.

The Remote Call Forwarding terminating telephone may not be equipped with the
Call Forwarding feature.
3.11. The originating station is responsible for all charges on calls placed to the Remote
Call Forwarding number as specified elsewhere in these tariffs.
3.12.
The calling party is responsible for toll charges associated with calls between the
originating telephone and the call forwarding location. The Remote Call Forwarding
customer is responsible for the applicable customer dialed station-to-station charges
associated with messages between the call forwarding location and the terminating
telephone.
3.13.
Auto Origination designated number will be administrated by the Company.
3.14.
CLASS features are provided subject to the availability of facilities. These features
will operate only on calls originating and terminating within a CLASS serving office,
or similarly equipped offices of interconnecting local telephone companies.
3.15. CLASS features will be offered on a subscription basis only, except for CustomerOriginated Trace which will be billed on a per-successful-activation basis.

CLASS will be available to customers having premises equipment which is
technically compatible with the features.
3.17. No charges contained within this tariff for CLASS features include the supply of
Customer Premises Equipment (CPE). CPE must be purchased separately by the
customer.

CLASS information will not be forwarded for calls originating from public and
semi-public coin stations.
3.19.
The existence of some Digital Loop Carrier systems on the customer loop could
inhibit the delivery of CLASS display features to customers served by these systems.
3.20.
The Customer Originated Trace (COT) feature will be based on the standard
annoyance call procedure, and its use will conform with existing regulations
concerning how to deal with annoying and harassing calls. COT will not replace
existing procedures. Customers will not be provided with the traced number. The
results of a customer originated trace will only be released to legally constituted
authorities upon proper request by them.
3.21
Any customer subscribing to CND or CNND is responsible for the provision of a
display device which will be located on the customer’s premises. The installation,
repair, and technical capability of that customer premises equipment to function in
conjunction with the feature specified herein will be the responsibility of the
customer. The Company assumes no liability and will be held harmless for any
incompatibility with the network features provided by the company and described
herein.

3.24
All Company customers will be provided Calling Number Delivery Blocking
(CNDB) in conjunction with the offering of CND or CNND at no charge. CNDB is a
service that enables the calling party to suppress his/her directory number so that the
called party with CND or CNND does not receive the information. The called party
will receive a “private’’ message instead of the calling party’s directory name or
number message. All numbers which are not blocked will be delivered subject to the
technical limitations of the equipment.
3.25.
CNDB will be available to customers only on a per-call basis. CNDB, on a per-line
basis, is available upon request, at no charge, to the following entities for lines over
which the official business of the agency is conducted, where an executive office of
the agency registers a need for blocking and provides the required certification to
Company that the entity is: (a) a private, nonprofit, tax exempt, domestic violence
intervention agency, or b) a federal, state or local law enforcement agency. The
directory number will not be transmitted from a line equipped with this capability.

GENERAL DESCRIPTION
1.1
Advanced Digital Services is a public network-based set of communications services
that make it possible to send, receive, and modify information using regular
telephone lines. These services are based on the National Integrated Services Digital
Network (ISDN) standards. ISDN provides end-to-end digital communications and
gives the ability to transmit data and voice over the same telephone line
simultaneously ( a user can send information from a personal computer and talk to
the person on the other end of the line at the same the). This functionality is provided
via channelized transport facilities. The ISDN architecture consists of digital central
office switching systems which connect Basic Rate Interface (BRI) lines to
customers' premises.
1.2.
Advanced Digital Services BRI is an optional service arrangement that can be used
in conjunction with a customer's residential service, individual business line or
Centrex service. It uses the ISDN architecture to provide the customer with the
capability to transmit voice and data simultaneously over the same digital line. Under
various optional arrangements, BRI provides the customer with access to CircuitSwitched Voice Services and Circuit-Switched Data Services.
1.3
An Advanced Digital Services BRI arrangement obtains its capabilities from an
ISDN-capable, Telephone Company central office switch. The BRI ISDN
arrangement provides two communications channels (but using only one physical
line) between a telephone (or computer, fax machine or other equipment) and the
digital central office. These channels are called Bearer, or B Channels. Another
channel, called the Delta, or the D channel, is used for signaling purposes. The
complete BRI ISDN line is known as 2B+D.

1.3.1. B ChanneI-The B Channel is a bi-directional synchronous channel capable of
supporting digital transmission speeds of 64 kilobits per second (kbps). Each
B Channel may be configured in one of the following ways:

All Advanced Digital Services lines consist of central office facilities (including
outside plant facilities) extended from the Company's switching equipment to the
customer's demarcation point.
1.5.
Directory Numbers - On a given 2+D Advanced Digital Services line, calls are
routed to the appropriate terminal device (voice telephone or computer/data terminal)
based on the directory number and the type of call {voice or data) presented to the
Advanced Digital Services line.
1.5.1. Primary Directory Number - Each Advanced Digital Services Line includes a
single primary telephone directory number, which may originate or receive
calls.
1.5.2. Secondary Directory Numbers - An Advanced Digital Services line may
have additional telephone directory numbers. The additional telephone
number(s) may originate or receive calls independent of the user’s Primary
Directory Number; however, each B channel is allowed only one
simultaneous circuit connection at a time.

CIRCUIT SWITCHING SERVICE DESCRIPTIONS
Circuit Switching is a switching arrangement in which an entire circuit or, in a digital switch
equipped for ISDN, a specific selection of channels is dedicated to a given call.
Circuit-Switched Service provides the ability to originate and receive circuit-switched voice
and/or data calls over a 64 kbps B Channel, The customer may choose among the following
Circuit-Switched features based upon application needs:

Clear Channel Capability - A characteristic of the transmission paths on the B
Channels that allows the fill bandwidth of 64 kbps to be available to the customer.
Using appropriate terminals or terminal adapters, it is also possible to bridge two B
Channels together to achieve data transmission speeds of 128 kbps. However, ISDN
interconnection to or through non-ISDN equipped central offices will potentially be
subjected to analog transmission or sub-rated to 56 kbps per channel.
2.2.
Additional Call Offering - This feature allows the user to be notified of additional
call when the telephone set is busy. Similar to conventional Call Waiting, multiple
incoming calls to a directory number or secondary telephone number (if purchased)
can be terminated to the telephone.
2.3.
Multiline Hunt Service - This feature allows incoming calls to a busy directory
number to search though a predetermined list of directory numbers. This may be an
Advanced Digital Services directory number on a different BRI or (for voice calls)
an analog he. The hunting arrangement may be series completion, linear, uniform
distribution, or circular.
2.4.
Directed Call Pick-Up - This feature allows the user to dial a special code plus an
extension number to answer calls directed to other stations.

Custom Calling Services - Applicable Custom Calling Services (except for those
superseded by an Advanced Digital Services counterparts) are available at rates
and charges specified in the Custom Calling Services section of the Company's
tariff. The following Custom Calling features found specifically in this tariff will
be charged at rates shown on the Advanced Digital Services Rates Schedule.
2.6.
Call Hold - This feature allows the user to place a call on hold by pressing the
appropriate button on the telephone set. As an option, multiple independent calls
may be retained on hold with this feature.
2.7.
Three-way Calling - This features allows the user to add a third party to an existing
voice call by pressing a feature key, and thus enables a conference between parties at
multiple locations. The user may also disconnect the last party added by depressing
the appropriate button on the telephone set.
2.8.
Call Transfer - This feature allows the user to transfer a voice call to another
directory number.
2.9.
Call Forwarding - This feature allows voice calls to be redirected from one ISDN
telephone (based on DN and call type) to another telephone (either an analog or an
ISDN telephone).
2.10.
Advanced Calling Services - Advanced Calling Services (also called CLASS
services) are available at rates and charges specified in the Advanced Calling
Services section of the Company's tariff. The following Advanced Calling Service
features are included specifically in this Advanced Digital Services tariff.

2.11.
Automatic Callback/Monitoring - Also called Repeat Dialing, this feature extends
the concept of the Automatic Callback feature to calls originated at Advanced Digital
Services lines. When the calling party encounters a busy condition with the called
party, the calling party can dial an access code to cause the switch to monitor the
called party. When the called party is no longer busy, the switch will notify the
calling party of this condition.
2.12.
Caller ID - Number - This feature allows h e calling party's directory number to be
displayed on a properly equipped telephone set.
2.13.
Caller ID - Name - This feature (where available) allows the listed name of the
calling party (along with the caller's number) to be displayed on a properly equipped
telephone set.
2.14.
Display Capability - This feature allows an appropriately equipped telephone set to
display a variety of information. For example, when idle, the time and date are
displayed. When the user is making a call, call progress information is displayed.
The following information is also provided.
2.14.1. Caller ID - As described under Advanced Calling Services in this tariff,
Caller ID - Number is provided. Caller ID - Name is also displayed if
subscribed to.
2.14.2. Called Number Display - This feature displays the called number (dialed
digits) on the telephone set when an outgoing call is made.
2.14.3. Message Waiting Indication - For users who also subscribe to Voice Mail
Service (de-tariffed service), this feature provides a visua1 message waiting
indicator (via a lamp/feature button or other message indicator on the
telephone set) to indicate that the user has received a voice message.

TECHNICAL SPECIFICATIONS
3.1.
Transmission Specifications
The standard transmission parameters for an Advanced Digital Services line utilizing
an ISDN Basic Rate Interface (BRI) consists of: A maximum of 38.5 db loop loss at
a 40kHZ test tone terminated into a 135 ohm impedance. The 38.5db loss includes
all central office facilities, outside plant facilities and inside wiring.
3.2.
Customer Premise Equipment and Facilities
Compatible customer premise equipment is required to utilize Advanced Digital
Services. All equipment used to interface with these services is required to conform
with National ISDN guidelines as referenced in the following Bellcore
specifications.

REGULATIONS AND CONDITIONS
4.1.
Unless specifically exempted, Advanced Digital Services shall be subject to all
general regulations applicable to the provision of service by the Company as stated
in the general tariff.
4.2.
Advanced Digital Services are provided at the option of the Company. These
services are furnished subject to the central office switching capacity, capability, and
the availability of outside plant facilities.
4.2.1
The availability, functionality, and capabilities of Advanced Digital Services
may vary, or may not be available, dependent upon type of serving central
office switch, related software controlling that switch, and associated outside
plant.
4.2.2. Where facilities are not available, or unused expenditures are involved in
making them available, the customer may be required to pay additional
charges to cover the unusual expenditure, or to contract for services beyond
the normal service term, or both.
4.2.3

Payment for Service
4.3.1
Issued:
Distance Extension Service: Advanced Digital Services may be provided
to a customer's location served beyond the normal transmission range of
the serving central office. In such cases, in addition to the charges and rates
for Advanced Digital Services, Distance Extension Service rates and charges
are applicable. These rates and charges will be determined on an individual
case basis.
The minimum charge period for services provided under this tariff is for
one month.

4.4.
Directory Listings
4.4.1. One directory listing is provided without charge for each Advanced Digital
Service customer. Additional listings may be provided as specified for
Additional Listing Service in the Directory Listings section of the Company's
tariff.
4.5.
Billable Call Treatment
4.5.1.
Normal toll charges shall apply to calls that are made outside of the Local
Service Area.
4.5.2. Advanced Digital Services customers who use the Call Forwarding or Call
Transfer features are responsible for the payment of any applicable
charges for each billable call connected via these features over the public
network. The charge is applicable to each call answered, including the
Call Forwarding setup call. It also applies to collect and person-to-person
calls, which may be refused at the answering station.
4.6.
Customer Premise Equipment
4.6.1. This tariff for Advanced Digital Service does not include terminal equipment
on the customer's premises. Terminal equipment may be covered under a
separate tariff, sold or leased by the Company (under a separate contract), or
may be provided by the customer.
4.6.2. The customer is responsible for providing the power required for any and
all customer premise equipment connected to an Advanced Digital Services
line.

The Company shall not be liable for any loss or damages arising out of error,
interruptions, defects, failures or malfunctions of Advanced Digital Services or
associated equipment. Damages arising out of such interruptions, defects, failures, or
malfunctions of the services after the Company has been notified, and has reasonable
time for repair, shall in no event exceed an amount equivalent to the charges made
for the service affected for the period following notice from the customer until
service is restored.
