REFERRAL INFORMATION
Adequate and timely information exchange between treating doctors is essential for safe and effective patient care. However, studies of referral
letters have consistently reported that specialists are dissatisfied with their quality and content. The concerns most often expressed are the
frequent absence of an explanation for referral, medical history, clinical findings, test results and details of prior treatment.
Good quality referral helps to reduce the unnecessary repetition of diagnostic investigations, and helps to avoid patient dissatisfaction and loss of

ABORIGINAL INTERPRETER SERVICE
Aboriginal Interpreter Service
The Aboriginal Interpreter Service provides a 24 hours 7 days a week central booking service for government and non-government agencies that
require on-site Aboriginal languages interpreters. Bookings should be made with as much notice as possible – a week is preferred, especially if
the assignment will entail arranging travel for the interpreters.

AIS at Royal Darwin Hospital (RDH):
The Aboriginal Interpreter Service provides interpreters for five mornings per week at RDH. The interpreters undertaking this work are organised
on a rotating roster to provide a variety of language coverage in any given month. Wherever possible the interpreters are chosen to reflect the
dominant languages represented at the hospital in that week.
Payment
Payment for use of the AIS is made from a health budget provided specifically for the AIS (Other Government services and departments also
have their own budget). Payment for the service does not come out of existing health budgets.

Translating and Interpreting Service (TIS) National
The Department of Immigration and Citizenship (DIAC) provides the TIS National interpreting service with over 170 languages/ dialects
available. TIS National is available 24 hours a day, seven days a week.

Sending results to NT Hospitals - GPs are requested to ask for a copy of results to be sent to Jade Care Clinical when
requesting tests for patients they are referring to Royal Darwin Hospital (RDH). This will make it easier for the hospital clinicians to
obtain the results and improve the efficiency of results management internally within RDH. Addressing Referrals to RDH - For
Medicare purposes it is requested that GPs also nominate the Specialist by name when referring for hospital appointments.

healthdirect Australia is a FREE 24-hour telephone health advice line staffed
by Registered Nurses to provide expert health advice and direct patients, as
appropriate, to local health services. Afterhours, and in preference to contacting
the Emergency Department at their regional hospital, GPs should advise their
patients to call healthdirect Australia on 1800 022 222.
Use GP ACCESSLINE 8922 7900 to speak directly to the lead ED registrar or
consultant. Duty consultant available via Accessline til midnight weekdays and
1800hrs on weekends and via switchboard after those hours
NOTE: Please do not give Accessline number to patients; it is for doctor (or
doctor’s delegate) to doctor communication.
If patients are referred to RDH /Emergency Department please:
 Do not indicate to patients they will be admitted but rather that they will be
assessed at the Emergency Department. ED has access to range of
investigations and treatments and a short stay unit that may make
admission to the main hospital unnecessary. It is also possible for them to
be referred for admission to Hospital in the Home.
 Ask about whether patients have private insurance or are keen to be
assessed privately as this can save unnecessary delays and extend waits
in the Emergency Department.
 Remember other options such as the acute medical clinic or early
pregnancy clinic
On arrival, the triage nurse will priority triage patients according to the
assessment of time the patient can wait to be seen.

Medical Clinics are held at both RDH and Palmerston with a visiting service to remote and regional centres
RDH provides 3 acute medical clinics/week which are an alternative to ED referral and can provide “urgent appointments” in the registrar clinics.
Note: Where patients are referred privately a fee may be charged for private consultation. A reduced fee or bulkbilling may be negotiated for some patients.
It is therefore advisable for patients to have their Medicare card available.

The acute medical outpatient clinic provides fast access for patients who
need an urgent physician assessment but do not require admission.
Refer in circumstances where early intervention may alter the outcome and
prevent admission or reduce patient complications, morbidity and mortality.
Examples of suitable referral may include acute medical problems such as
TIA or acute or chronic conditions where the patient is not responding to GP
treatment- exacerbation of COPD, decompensating CCF.

Poisons & Dangerous Drugs Act & Regulations. The prescribing and supply of Schedule 8
substances must conform with these Schedule 8 and Restricted Schedule 4 Clinical Practice
Guidelines as required under section 31X of the Poisons and Dangerous Drugs Act.

Darwin Withdrawal Service
Outpatient and outreach withdrawal services are available for individuals who wish to
withdrawal from alcohol, opiates, cannabis, amphetamines and Benzodiazepines.
Clients are assessed by a medical team to identify the best withdrawal option. The ADS
Withdrawal Services team then provides medication (if needed), support and counselling.
Withdrawal may take place at the Royal Darwin Hospital (for high risk clients), in supported
accommodation (e.g. CAAPS, Banyan House or the Salvation Army) or include home visits.

RACGP guidelines on dementia recommend the following tests as part of
the work up of the demented patients and should be completed prior to
referral:
● Hb,WBC, ESR
● renal function/electrolytes
● liver function
● thyroid function
● blood sugar
● serum calcium and phosphate
● urine – WBC, protein, sugar (culture if delirium)
● serum B12, folate levels
● CT scan without contrast
● CXR (if delirium)
● syphilis serology (if indicated – usually indicated in NT)
● HIV testing (if specific indications).

Aged Care Assessment Team (ACAT)
ACATs provide information on suitable care options and can help arrange
access or referral to appropriate residential or community care services such
as Home and Community Care (HACC). An ACAT assessment and approval
is required before people can access residential aged care, Community Aged
Care Packages (CACPs) or Extended Aged Care at Home (EACH) Packages
If you wish Doctor Lowe to see a patient as part of their ACAT work up
please include a medical referral with your ACAT referral--including your
provider number, list of current medications etc. Clients may be referred to
the ACAT team by any concerned person – not necessarily by doctors. If
the ACAT team considers that Doctor Lowe needs to see such clients, they
will contact the GP for a referral if the GP sees appropriate.
Occasionally the ACAT team is called in situations where it is very difficult to
find out who the GP is. In these situations we will do our best to keep the
GP informed once we find out who they are.

Dementia Monitoring Program provides individualised practical help for
people with memory problems who live alone, or who have minimal family or
other social support. This assistance can be very flexible, and may include
transportation, help with budgeting and bill-paying, an occasional outing or
visit to the home. It also includes a weekly exercise group for those who are
physically active and keen to maintain their fitness. A major benefit of this
scheme is that it can maintain a regular check of the client's needs, and
report back to GP if there is a problem.
The Living With Memory Loss program is available to people dealing with
dementia or memory loss, and their family members and/or friends. The
program is conducted by Alzheimer’s Australia NT at various locations in the
Territory. For further information, or to find out when the next course will be

Dementia Behaviour Management Advisory Service (DBMAS) is a
nationwide network of services funded by the Australian Government that
provides clinical support for people caring for someone with dementia who is
demonstrating behavioural and psychological symptoms of dementia (BPSD)
which are impacting on their care.
The NT DBMAS office is staffed between 8.00am and 4.30pm (CST) on
weekdays.
After hours support and assistance is available on 1800 699 799, the
national after hours telephone service.

DBMAS is a government funded program and there is no cost to accessing
the service. DBMAS will make appropriate referrals to other support

GPs can now refer directly to RDH hospital-in-the-home (HITH) programme
( i.e. GPs no longer need to refer via ED) HITH treatment is safe and is
associated with high levels of patient satisfaction, with less disruption to
normal life and without a potentially prolonged wait in the emergency
department first.
Once a patient is admitted to HITH, the Hospital takes full responsibility for
all medical and nursing issues until the patient is discharged back to the
care of the GP. Patients that deteriorate whilst on the HITH programme will
be admitted to Royal Darwin Hospital.
GPs can refer during weekday working hours by telephoning the
Infectious Diseases Physician on call via Royal Darwin Hospital
switchboard Ph: 8922 8888. Afterhours referral via ED.
The Darwin and Palmerston metropolitan areas are covered as far as
Humpty Doo. (If your patient lives in the Darwin Rural Area, still consider a
referral as arrangements can often be made.)
Suitable Patients are those who are medically stable, have appropriate
social supports (including a designated support person, telephone, safe and
hygienic home environment and able to communicate effectively) and are
able to consent to home-based therapy may benefit from a direct referral.
Common indications for HITH include:
 Cellulitis and other skin and soft tissue infections
 Uncomplicated diabetic foot infections
 Deep venous thrombosis
 Others – call if you think you have a suitable candidate

The Alan Walker Cancer

The AWCCC provides a comprehensive range of services including:
radiation and medical oncology, haematology, chemotherapy nursing, and
allied health ( social workers, physiotherapy, dietetics, Aboriginal liaison
support)
Referral to the medical oncology should be addressed to AWCC.
Northern Territory Radiation Oncology (NTRO) provides the radiation
oncology services to the AWCCC. The NTRO operates as an outpatient
facility, Monday to Friday 8.00am to 4.30pm. NTRO is located on the
premises of the Royal Darwin Hospital - see site map on NTRO website.
NTRO is supported by the Royal Adelaide Hospital Cancer Centre and is
staffed by radiation oncologists, physicists and radiation therapists.
Treatments not supported at the AWCCC are paediatrics, stereotactic
radiosurgery, total body irradiation, brachytherapy and positronic emission
tomography. PATS services will still remain available for patients requiring,
and still receiving, treatment outside of the Territory.
Patients travelling from outside of the Darwin region for Radiation Oncology
treatment will be offered unit-style accommodation at the Mirambeena
Street Facility in Darwin. (Offers separate male / female accommodation
wings or family rooms; access to domestic cooking facilities or the option of
catered meals; and patient transport to the AWCCC.)

Hospice
The Hospice is a specialised 12 bed short-stay facility located on the
grounds of RDH in Darwin. The hospice accepts admissions for active
symptom management, as a transition to home from hospital, or for care in
the final stage of illness. There is limited access for respite care.

The Department of Renal Services is very happy to receive referrals from GPs who need help in the management of their renal patients. A referral with the
main immediate problem together with a list of prescribed medications and relevant past history would be helpful. This can be sent to the Royal Darwin
Hospital where appointments will be organised to see the patient at RDH or at the Palmerston campus.
We provide the following services:
1. Assessment of patients with haematuria and proteinuria.
2. Assessment of renal impairment.
3. Preparation for renal replacement therapy.
4. Donor assessment for potential live transplant donors.

GP Liaison Service


GP Liaison Service for GPs in the Darwin urban region offers review and advice to GPs in the management of non-urgent mental health matters
and associated medication management issues. Mental health issues may include; significant mood or anxiety disorders, trauma, or psychosis
The GP Liaison service does not provide initial assessment for clients that have not been commenced on treatment or do not have a Mental Health
Care Plan in place. These options should be trialled by the primary care provider prior to any referral. However, a referral for a review of client
under treatment may be arranged. General Practitioners are provided with written reports to assist in the ongoing management of complex
presentations.



8922 8399 or Amity house - 1800 684 372 for alcohol and drug issues
GP Liaison has limited resources, as such, before they any referrals, they ask that:
GPs have active consent from the patient for them to be referred to GP Liaison.
GP will have commenced a treatment regime, and monitored the efficacy of medications, prior to referring in for the GP Liaison Service.
GP have explored other intervention options such as a Mental Health Care Plan.
GP commits to remain as primary care provider for the patient and will implement management plan
GP is to provide a comprehensive history, including, presenting issue, past and current treatments, interventions tried and other services involved
in the referral
Please do not refer to GP Liaison for diagnostic clarification. This function can be discussed with TEMHS intake service (89994988) or via private
practitioners.

 For review referral your primary concern must be one around mental illness and the client should be on treatment that is not currently therapeutic
prior to referral. If this is not the case you will be provided with appropriate advice via phone or return letter.
 Other services and options exist to provide you with assistance, such as: AOD - 892 28399 or Amity house - 1800 684 372 for alcohol and drug issues
 Mental Health Care plan for psychotherapies and treatment for emotional issues, often the most appropriate first line intervention with high
prevalence disorders.

 Should your patient fail to attend their appointment, the referral will be closed. You will be notified and are able to re-refer if you wish.
The Mental Health and Related Services Act is the legislation that governs the delivery of mental health services in the Northern Territory and the
treatment of people with mental illness in hospitals and the community.
Voluntary admission - a patient is not subject to any order under the Mental Health and Related Services Act. The patient may be managed as a
voluntary patient in the community or in a hospital. Referral may be made by GP, mental health worker, self or other e.g. a parent or family member.
Patient must consent to treatment and admission.
Involuntary admission is an admission because of mental illness or mental disturbance. Patients must accept any necessary psychiatric treatment that is
ordered. The patient may be admitted as:
1. an involuntary client in the community under a Community Management Order, or
2. an involuntary patient in the hospital. Whilst admitted involuntarily, the patient cannot leave the hospital without permission.
Mental Illness - the doctor must be satisfied that the patient has a mental illness and because of that illness:
 needs treatment that is available at the hospital; and
 without treatment, they are likely to seriously harm themself /someone else or become more unwell.
Mental Disturbance
 Patient does not fulfil the criteria for involuntary admission on the grounds of mental illness; and
 their behaviour is, or has over the past 48 hours, been so irrational as to lead to the conclusion that:
o for some reason they are not functioning normally; and
o their behaviour has been so abnormally aggressive or irresponsible to justify the need for psychiatric assessment or care in hospital.

GP Referrals – ring and discuss your referral with GP Liaison team on 1800 682 288, this is
best done when you have the client with you. The triage clinician will discuss presentation
and available options with you. Then send your referral or a completed GP Liaison referral
form by fax to (08) 8942 6568
CATT team can then advise on:
 best location for patient to be assessed:
o Depending on patient location and CATT workload , CATT team may be able to attend
patient or will see in Building 9 on RDH campus – opposite Cowdy ward
o Tamarind Centre during working day hours
o If patient is likely for admission, the usual practice will be to refer patient to ED where
facility/infrastructure is better setup for this process

Police should be considered last resort and only if patient is dangerous, also needs
escort.
If GP believes the patient requires an urgent psychiatric assessment, please contact
NTCATT.
GP can complete Entry to Mental Health Services Form 09. This enables patient to be
detained for 6 hours and requires the patient to be assessed by an Authorised Psychiatric
Practitioner (APP) or a Designated Mental Health Provider DMHP. For remote patients the
assessment may be done by telephone and/or via a DMO who can liaise with APP.
Where a medical officer refers for a psychiatric assessment it may be that the patient is
assessed by either an APP or DMHP. While there is no mandated requirement to inform the
referring medical officer of the outcome of this process it is considered best practice that the
referring MO should be informed of the outcome especially should the situation arise that
admission was not deemed necessary.

Indicate for colposcopy, and attach PAP/ HPV results.
Referrals that indicate "for continuing care and management” will be valid for
the entire treatment of the patient’s condition and the patient will not need to
get another referral for treatment and follow up that continues beyond 12
months.
Provide a Patient Information Sheet
Results are given to patient by the Colposcopist at a three week follow up
appointment when further follow up is also organised.
A letter to the GP will be written on all new patients with a referral letter and for
previous patients if further problems occur. The letter will provide information
as to the colposcopy findings and the continuing care plan.
If a pap smear only was performed, a copy will be sent to the referring GP and
the patient advised to contact their GP for the result.
GPs can obtain recent results by faxing a signed patient information release
form to Pathology for recent results (or to the Community Liaison on 8922 7865
for older results). Remember to request specific results (e.g. PAP smear result
and biopsy result) otherwise only one result may be sent.

mandatory reporting obligations
The Toolkit contains information and resources regarding partial confidentiality, risk
assessment, safety planning, and staff safety. The Toolkit provides suggestions and
ideas to assist organisations implement processes and policies to meet the new
reporting requirements

Amendments were made to Section 26
of the Act on 20 August 2009 to change
the reporting obligations regarding

Fact Sheet – REPORTING HARM AND SEXUAL OFFENCES INVOLVING CHILDREN
In the Northern Territory every person is required to report suspected child abuse and
neglect. The purpose of imposing a legal obligation on people to report suspected child
abuse and neglect to the department, is to protect children/young people from further
harm. Child abuse can be prevented. Identification and reporting of child abuse
are the beginnings of intervention and prevention of such abuse. By protecting the
child/young person and helping the family, future abuse can be reduced, even
eliminated.

OBSTETRIC CARE - PUBLIC

FOR ALL PATIENTS
GPs should:
 Confirm pregnancy and offer healthy pregnancy advice.
 Provide information on options of care – shared, private and public. Assess suitability and interest in GP shared care.
 GPs will need to provide first trimester care for all public patients (and the majority of private patients).
 All public referrals should initially be to first antenatal clinic.
 The orange hand held record should be completed, and provides a useful prompt and checklist to assist with comprehensive assessment.
 The pregnancy information kit (available from the NTML) should be provided to all women in their first pregnancy and as appropriate
 Forwarding results to RDH or other NT Hospital - please request a copy of results to be sent to Jade Care Clinicals. This will make it easier for the
hospital clinicians to obtain the results and improve the efficiency of results management internally within RDH.
At the first antenal clinic visit at RDH, a women’s suitability for models of antenatal care will be assessed. If a woman is assessed as low risk, the woman has
the choice of continuing Shared Care with GP, or attending the Midwives Clinic at RDH, the Community Midwives Program, or continuing with the antenatal
clinic. See pamphlet “Pregnancy Care at RDH-Birthing Option “
Note: Women will not be offered these options for maternity care until her routine ANC is complete and no problems are identified.
For public patients ANC will also organise the fetal morphology scan for 18 weeks. If the woman has uncertain dates and no previous ultrasound RDH
usually do a dating scan at the first antenatal clinic but it is preferable for the GP to organise this.
Amniocentesis is available at the public hospital-performed from 15 weeks GA, or consider private referral for Amniocentesis: (Referral to Dr S O’Callaghan
-CVS and Amniocentesis available or Dr A Miller - Amniocentesis, note private fee would apply)
Second Trimester screening: 14w - 20w6d for maternal bloods only.

1 Midwives Clinic (MWC)
Referral for MWC required before 22 weeks gestation. Midwife ANC based at
RDH OPD Clinic located on 8th floor, Main ward block, RDH. Delivery is at RDH
Delivery Suite 6th floor Main ward block RDH. Complete relevant sections of
orange hand held record.
2 Community Midwives Program (CMP)
CMP care is available only for low risk women. Strict entry criteria apply. Places
are limited. The model of care is very popular. Women can register their interest
directly to the Birth Centre from 14 weeks but will require a first visit in the
Antenatal Clinic. GPs should refer to ANC and indicate that the women would like
to be considered for CMP. GPs should complete relevant sections of orange hand
held record. CMP provides continuity of care through antenatal, intrapartum and
postnatal with care provided by midwives. Some places are reserved for women
with high social needs. Care is provided from Birth Centre at RDH but may
involve home visits. Delivery is either in the Birth Centre or at RDH Delivery Suite
6th floor. Early transfer home within 4 hours after birth if all is well otherwise
transfer to RDH. Postnatal follow up with initial daily home visits, 24-hour on-call
support and postnatal care for up to three weeks following birth.
3 Midwifery Group Practice (MGP)
The MGP offers Darwin-based care for remote Indigenous women, from
consultation rooms in Wanguri. Under the Midwifery Group Practice, women
are assigned a Darwin-based midwife who works in conjunction with an
Aboriginal Health Worker. The midwife accompanies the mother to any
specialist appointments, conducts their antenatal care and education, and
assists at the birth of the baby, either in the Delivery Suite or the Birth Centre
at Royal Darwin Hospital. The MGP began in September 2009 for mothers
from Wadeye, Maningrida, Palumpa and Peppimenarti. The communities of
Warruwi, Minjilang and Nauiyu Nambiyu (Daly River) have recently been
added to the program. The Practice is funded by the NT Government’s
Closing the Gap initiative with supplementary funding from the COAG
Indigenous Early Child Development Partnership.
Medical Obstetric

Public hospital patients who meet inclusion criteria for Medical Obstetric Clinic
(see below) should be referred to Medical Obstetrics Clinic and addressed to
Endocrinologist, Dr Sridhar Chitturi, and Obstetrician, Dr Sujatha Thomas Fax
referrals to 8922 6750.
(High Risk Antenatal Clinic)
This clinic is designed to manage the care of women with high risk pregnancies
and with concurrent or pre-existing medical illnesses. Examples of co-existing
medical conditions warranting referral to this clinic include:
 Pre-gestational T1 and T2 diabetes,
 Thyroid disease: Graves’ disease, Hashimoto’s thyroiditis and
hypothyroidism,
 Pituitary dysfunction,
 Adrenal disorders,
 Severe hypertension,
 Active hepatic/renal/haematological/rheumatological/Infectious disease
needing joint care with sub-speciality teams,
 Cardiac disease – including mild valvular stenosis, moderate to severe
degrees of valvular dysfunction, prosthetic heart valves, history of heart
failure or prior surgery, myocardial dysfunction, congenital heart disease,
pulmonary hypertension, Marfans syndrome,
 Current or past history of venous or arterial thromboembolosim.

Pregnant women who prefer to be seen by private obstetrician who have coexisting medical conditions warranting referral to an Endocrinologist privately,

Homebirth Service

Homebirth offers a woman and her family one-to-one midwifery care in her home
throughout antenatal, intrapartum and postnatal period.
The current geographical boundary for homebirths in Darwin region is Howard
Springs Road. However, women outside the designated boundary area can still
have a homebirth by arranging accommodation in the Darwin or Palmerston
regions.
GPs can support care though arranging routine screening.
Backup care is provided through RDH ANC and Delivery Suite. Patients may also
be supported to deliver in the Birth Centre at RDH.

Prenatal diagnosis services are available including referred ultrasound scanning,
chorion villus sampling, amniocentesis and nuchal translucency screening.

Dr O'Callaghan performs nuchal translucency scans at 11-14 weeks +or- CVS or
amniocentesis, and routine scans at 17-22 weeks. GPs should include referral for
both these scans at the time of their initial referral.

The Northern Territory’s Medical
Services Act (section 11) states that
abortion is lawful;
a) up to 14 weeks gestation, when
performed in a hospital by an
obstetrician or gynaecologist, where
that practitioner and another
practitioner are both of the opinion
that
i) The continuance of the
pregnancy would involve greater
risk to a woman’s life or greater risk
of injury to her physical or mental
health than if the pregnancy were
terminated; or
ii) there is a substantial risk that, if
the pregnancy were not terminated
and the child were to be born, the
child would have or suffer from
such physical or mental
abnormalities as to be seriously
handicapped.

Information on referrals to Ophthalmology at Royal Darwin Hospital
Given the imbalance between high volume of referrals and limited
appointments at the Eye Clinic at Royal Darwin Hospital, prioritisation of the
referrals depending on the nature of the eye condition has become necessary.
Adequate information on the referral to the Ophthalmologist is required for
meaningful prioritisation. This should ideally include:
Mandatory information:
 Main visual complaint (in which eye)
 Duration of symptoms
 Visual acuity in each eye (with distance glasses if patient has them)
formally checked with a Snellen chart or equivalent.
Depending on the clinical situation and severity, referrals without the above
mandatory information (especially visual acuity) may be
- Resent to the referrer as incomplete, and further action on the referral will be
taken only after receiving the missing information
- Prioritised without a urgency category, hence patients might have a long
waiting time to get an appointment

2. Past medical history
3. Current medications
It should be noted that patients who need screening for the following
conditions should be ideally sent to an optometrist who can follow up yearly
and refer to the Ophthalmologist when appropriate.
 screening for eye involvement in chronic diseases like diabetes and
hypertension
 screening for refractive error and need for spectacles
 screening for glaucoma
Please call the Oncall Ophthalmology Registrar through RDH Switchboard to
discuss very urgent referrals.
Appointment waiting times will depend on the time of referral and more
importantly on the severity and urgency of the clinical situation. The Eye clinic
at RDH appreciates your understanding in this matter.

patients would receive in Adelaide. In particular they can offer transrectal
prostate biopsy through Darwin Private Hospital under sedation for men with
an abnormal PSA or abnormal prostate examination.
Most urological surgery can be offered in Darwin, including radical
prostatectomy for prostate cancer. Some complex laparoscopic surgery and
minimally invasive laser surgery for renal stones cannot be offered in Darwin
at present.
