of transition planning and
services in the IEP. This
presentation and related
materials are intended to
support districts in developing transition plans designed to improve postsecondary outcomes for students with disabilities.
You will need to bring:
 1 student’s ETR/IEP age 14
or older from your district
 Any documents developed
in assisting the student &
family planning for postsecondary goals
 Middle or High School
course of study
