Building Operations: Tenant Insurance Requirements Prior to occupancy and updated from year to year, tenants must provide to the Building Management Office the proper certificate(s) of insurance. General Liability The required form for Certificate of Liability Insurance is Acord Form 25-S. The Certificate Holder should be Cushman & Wakefield Of California,Inc. Requirements for general liability coverage are in the amount of five million dollars ($5,000,000). Workers’ Compensation coverage is as required by law. Please click here to download a Tenant Emergency Contact Sheet Building Security: Building Access Building Hours Monday through Friday 5:00 a.m. to 6:00 p.m. Saturday, Sunday & Building Holidays Closed After- Hours Access 345 California Center's security staff is on duty 24 hours a day to monitor the security systems and life safety controls of the Building and to supervise procedures relating to access, deliveries and property removal. After-hours access is monitored by closed-circuit television surveillance and controlled by a computerized card access system which provides individual cardholders entry only to the floor(s) for which their cards are programmed. Tenant Authorization Form Each tenant is asked to designate on a Tenant Authorization Form employees who may authorize security matters pertaining to the premises such as keys, lock changes, after-hours access, property removal passes and access cards. Also designated are those who are authorized to request chargeable services such as janitorial or engineering services or work requiring an outside contractor, such as an electrician. We request that tenants notify the Building Management Office of any changes in authorized personnel (i.e. additions or deletions). Requests for restricted services are accepted only from authorized tenant employees. Please click here to download a Tenant Authorization Form After-hours Access Form An After-hours Access (AHA) Form will allow entry to the Building during restricted hours to contractors, vendors and tenant employees who have not been issued an access card. The form, which must be signed by a pre-authorized tenant representative, includes the date, time, floor(s) and the name of the person(s) or company for whom access is requested. The completed form should be submitted to the Building Management Office no later than 4:00 p.m. on the day of the requested access or 3:00 p.m. on the Friday prior to a request for weekend access. Once approved by the Building Management Office, the form will be given to Building Security. We regret that AHA forms cannot be accepted by Building Security directly without prior Building Management authorization. The employee or contractor should present himself to the lobby officer or Security Console Operator, who will then allow access to the area(s) specified on the AHA Form. Please click here to download an After-hours Access Form Building Security: Deliveries Loading Dock Routine small deliveries should be made to the loading dock on the Pine Street side of the Building between the hours of 6:00 a.m. and 6:00 p.m. Monday through Friday. Please call the Building Management Office to arrange for deliveries at other times or to schedule major moves. Short term parking (20-minute maximum) is available on the dock for loading and off-loading during regular delivery hours. The loading dock may be used for extended periods after 6:00 p.m. or anytime Saturday and Sunday by prior arrangement. Deliveries are taken from the loading dock to tenants' premises via the freight elevator. Please advise your vendors that because of potential damage to walls and carpeting, pallet jacks are not permitted beyond the loading dock into the freight elevator or onto tenant floors. Deliveries to the Building Management Office If you have correspondence to be delivered to the Building Management Office, please call Mail Services at 415-434-7742. Your correspondence will be picked up and delivered to us promptly. Bicycle Messengers California Center has a convenient receiving area on the Pine Street loading dock for bicycle messengers. Bicycle messengers are not permitted to make deliveries directly to tenants' premises. Between the hours of 11:30 a.m. and 6:00 p.m. Monday through Friday, the Mail Services staff of California Center's Security department will accept and log packages and deliver them to you promptly. If you have an outbound package to be picked up by bicycle messenger, please call Mail Services first at 415-434-7742, then notify your messenger service to pick up your package from the Pine Street loading dock. Between the hours of 8:30 a.m. and 5:15 p.m. Monday through Friday, the Building's Mail Services staff will collect your package and have it ready for pickup at the loading dock within 15 minutes of your call. Tenants with a high volume of outgoing packages can arrange for regular scheduled pickups by the Mail Services staff. Non-Bicycle Couriers Deliveries and pickups by uniformed non-bicycle couriers (DHL, UPS, Federal Express, etc.) may be made directly to and from tenants' premises during normal business hours. After-hours Deliveries Deliveries must be accepted by the consignee. We regret that Building employees cannot sign a shipment acceptance or escort vendors to tenant premises after hours. Tenants may arrange to receive a delivery in their premises after hours by submitting an authorized after-hours access form to the Building Management Office, providing the name of the vendor, the destination and the expected time of delivery. For after-hours outgoing packages, please arrange to meet messengers or couriers at the Pine Street loading dock. If this cannot be arranged, packages may be left for pickup at tenant risk at a location on the loading dock specified by Security staff. However, 345 California Center cannot assume responsibility for proper pickup or delivery of any incoming or outgoing packages left on the loading dock. Building Security: Property Removal Passes Property removal passes are issued through the Building Management Office to pre-authorized tenant employees only. Property removal from 345 California is monitored by Building Security. ALL PERSONS leaving the building with boxes, packages or equipment should exit via the freight elevator to the ground floor (loading dock). Security personnel will request the property removal pass at that location. At the loading dock the tenant must have a property removal pass describing the article(s) in their possession to give the security officer at the loading dock or security control center before leaving with the property. To obtain a property removal pass, please contact the Facility Manager or Authorized Personnel within your office. Building Security: Incident Reporting Do not assume that an unfamiliar person who looks and acts like a business person is a legitimate employee or visitor. If you notice any suspicious persons in your office, ask them their business, or to provide identification. If they cannot do so, contact Building Security immediately at 415-434-7745. To provide an accurate record of every incident, 345 California Center staff is required to write an incident report for any accident, theft, or other incident occurring on the property. Should any incident occur, we request your cooperation in answering any questions the investigating Building Security Officer may have. Building Security: Key and Lock Policy The locks and keying sequences at 345 California Center are part of an overall master keying plan for the Building. Authorized tenant employees may request additional keys and lock changes and additions by contacting the Building Management Office. Please do not add or change locks or duplicate keys except by arrangement through the Building Management Office. Building Security: Lost and Found Please contact the Building Management Office at 415-434-7747 to report items lost in the Building or to claim items that have been found in the Building. Building Security: Solicitation As a matter of policy, 345 California Center does not permit solicitors in the Building. If you encounter a solicitor in the Building or require the assistance of a Building Security Officer, please call Building Security at 415-434-7745. Building Security: Tenant Precautions While Cushman & Wakefield of California, Inc. implements measures to maximize personal safety of 345 California Center tenants, individuals must remember that they, too, must take an active role to protect themselves. We offer a few recommendations: During the business day, do not leave purses, wallets, or loose change unattended. At the end of the business day, make sure all entrances and exits to your suite are locked. During regular business hours leave the reception door unlocked, but leave all other entrances and exits, especially those in back hallways, locked at all times. In addition, enforce a company policy stating the last person to leave for the day locks all the doors. Lock desks at night and over the weekend. Do not leave the office unattended during your regular business hours. Always have a company representative at the reception desk. If the entire office leaves for a function, lock the doors on your way out. Lock-up all laptop computers. Notice someone suspicious? Call Building Security immediately at Tel: 415-434-7745 and they will be dispatched to the scene. Building Services: Building Signage 345 California Center maintains a building standard sign through their contractor. Please contact the Building Management Office at 415-434-7747 with all of your building standard signage needs. Building Services: Cleaning Janitorial service is provided on weekday evenings beginning at approximately 5:30 p.m. Nightly janitorial services include trash removal, vacuuming, furniture dusting and restroom cleaning. Periodic services include low reach dusting, high reach dusting and floor buffing. 345 California Center also provides a daytime janitorial staff to maintain the level of cleanliness in the lobby, plaza, elevators, restrooms, etc. If you have any comments or questions regarding janitorial services, please call the Building Management Office. When collecting trash, 345 California Center's janitors are careful to collect only those items meant to be discarded. Trash labels are available from the Building Management Office for items which do not fit into waste baskets. If you require removal of large amounts of trash (due to a large move or shipment, etc.) please make advance arrangements with the Building Management Office at 415-434-7747 for the additional janitorial services. Cleaning (Continued) Tenants will be notified in advance of inside window cleaning so that files, plants and other items can be moved away from the windows to facilitate the cleaning process. Carpet and partition glass cleaning and other supplementary services may be arranged through the Building Management Office as chargeable services. Outside carpet cleaning contractors may be used with prior written approval from the Building Management Office. Recycling 345 California Center recycles mixed paper, glass, plastic, aluminum, newspaper and cardboard. Tenants are provided desk side recycling bins for employees and compost bins in kitchen areas as part of the regular janitorial service. Electronic waste is collected on a quarterly basis throughout the Building or upon need, it can be scheduled by Contacting Building Management Office at 415-434-7747. Batteries are part of the quarterly electronic waste collection or can be dropped off in the Building Management Office at Suite 475 at any time. Building Services: Elevators There are 14 elevators serving 345 California Center: twelve passenger elevators in three banks, one freight elevator and one shuttle elevator connecting the ground floor and the Garage. Please inform your contractors and employees that hand trucks and wheeled dollies are not permitted in passenger elevators. Deliveries that cannot be carried by hand must be made via the freight elevator. Pallet jacks must be left in the loading dock, as they may cause damage both to the freight elevator and to carpeting and walls in tenant areas. After-hours operation After normal business hours, elevators are locked off so that they will not respond to floor destinations selected from within the elevator without use of an access card. Access cards are not necessary to access the ground floor or to exit the Building. Freight Elevator The freight elevator is available for routine small deliveries on a first-come, first-served basis from 6:00 a.m. to 6:00 p.m. Monday through Friday. Moves should be scheduled between 6:00 p.m. and 10:00 p.m. weekday evenings or anytime on weekends. The freight elevator is available for priority use during these times. Please call the Building Management Office for reservations. Building Services: Cable Television Services Cable television service is provided to the building by Total Media Concepts, Inc. Total Media Concepts, Inc’s cable demarcation point is at the tap in the building’s telephone riser closet. The tenant should provide and maintain horizontal cable from the demarcation point to the locations(s) where service is desired in the tenant’s suite. There must be one separate cable for each receiver unit (television or other device).Total Media Concepts, Inc. will advise the tenant of the cable tap and the floor number of the riser closet nearest to the tenant’s location. Note: All horizontal cabling is the tenant’s responsibility to install and maintain. Building Services: Engineering Services Building Engineers repair and maintain all major Building systems and respond to tenant requests for temperature adjustment or lamp replacement. They can also provide miscellaneous chargeable services to tenants, such as re-keying locks, minor plumbing repairs, etc. We regret that 345 California Center staff is unavailable to assist with moving furniture. Please call the Building Management Office to request engineering services. Requests for chargeable services will be accepted only from authorized tenant employees. Building Engineers are on site from 6:00 a.m. to 6:00 p.m. Monday through Friday and 7:30 am to 4:00 p.m. Saturdays (schedule subject to change). Building Services: HVAC Seasonal heating and air conditioning are provided from 8:00 a.m. to 6:00 p.m. Monday through Friday, and at Tenant's request (but no additional cost) from 9:00 a.m. to 1:00 p.m. on Saturdays, except for Holidays generally recognized in San Francisco. Temperature is centrally controlled by the building's computerized energy management system. Should the temperature level change abruptly or exceed a reasonable level in your office, please call the Management Office at 415-434-7747 and a Building Engineer will be immediately dispatched to investigate the problem. HVAC services are available at other times by prior arrangement with the Building Management Office. After-hours HVAC service can be arranged by written request to the Building Management Office by an Authorized Employee 24 hours in advance. Tenants will be invoiced for costs of after-hours HVAC, including costs for Building Engineers' overtime if required for chiller operation. Click here to download an After Hours Air / HVAC Request Form Building Services: Mail Service U.S. Mail Incoming Incoming U.S. mail is delivered to the Building Loading Dock. 345 California Center’s Mail Services staff then distributes the mail to the tenants’ premises. Outgoing Outgoing U.S. mail is picked up from tenants' premises between 4:00 p.m. and 5:00 p.m. weekdays by 345 California Center's Mail Services staff and delivered to the loading dock. Late Mail So that late correspondence can be posted the same day, 345 California Center has arranged for a private mail courier to pick up outgoing mail each weekday evening and deliver it to the U.S. Postal Service for posting that evening. Tenants who have mail which must be posted after the regular afternoon mail pickup may call the Mail Services staff until 5:30 p.m., or may bring it to the loading dock any time up until 6:30 p.m. Monday through Friday. Building Services: Telecommunications As a result of a ruling by the California Public Utilities Commission, building owners rather than the telephone company are responsible to maintain, repair and manage the telephone cable from the initial point of entry or demarcation line in the basement up through the telephone riser to the tenant floors. Under this requirement, Building Management must carefully control access to the basement telephone room, the telephone riser, and the telephone closets on each floor. 345 California Center has contracted with IMG Technologies, Inc. to maintain and repair the cable and keep track of cable use and capacity. Only IMG Technologies, Inc. can bring telephone lines and service from the basement telephone room to tenant floors. Tenants may hire other companies to provide telephone systems (handsets, etc.); however, only IMG can bring lines to the telephone closet on tenant floors. These procedures should be used in conjunction with any emergency procedures established by your company. You are in a 48-story, first-class office and hotel building with many advanced safety features designed for your protection. These safety features and emergency procedures are designed to minimize the loss of life and property during emergency situations. Primary responsibility for the safety of building occupants and compliance with fire codes rests with each tenant. Material in this site is supplied as general information to help you meet these responsibilities. Due to the fire resistant construction of the Building, total evacuation of the Building is very unlikely. Should evacuation become necessary, the authority and responsibility rests with the San Francisco Fire Department, San Francisco Police Department, local government officials, or in an extraordinary situation, Cushman & Wakefield of California, Inc. An announcement will be made from the Fireman’s Control Room directing tenants to evacuate the building. Neither the Building Management nor Building Ownership can assume responsibility for any consequences resulting from the decision to evacuate or not to evacuate. Please keep the telephone lines clear so that Building Management may attend to the situation as quickly and efficiently as possible. Emergency Procedures: Emergency Action Plan 345 California was built in accordance with code requirements pertaining to the life safety of its occupants. The plans and specifications were submitted to the City of San Francisco for review and approval prior to issuance of an occupancy permit. Alarms are located in the corridors adjacent to the stairwells and in the elevator lobbies. Call 911 from a safe location. Evacuate or relocate and assist all others in the immediate area. Close doors behind you to isolate fire. Proceed to stairwells and listen for instructions. NEVER use the elevators. Be cautious when opening doors so as not to spread the fire. Touch any door to see if it is hot before opening. If smoke is present, stay low and crawl with your body against the floor. The clearest air is near the floor. If forced to make a dash through smoke or flame, hold your breath and cover your nose and mouth with an article of clothing. Click here to view SafetyNet and learn more about Life Safety Training Tenant Evacuation If ordered to evacuate, all of the following steps should be observed: Everyone should proceed quickly, but calmly, to the nearest stairwell exits. DO NOT RUN! DO NOT USE ELEVATORS! The Tenant Safety Coordinator or Alternate should walk the suite to assist employees and make sure everyone is aware of the evacuation order. An after hours response team should exist for companies who frequently have after hours work activity. Once the evacuation has begun, no one should attempt to re-enter the evacuated area until it has been declared safe by the police department, fire department or Building Management/Security. Upon arrival of the designated area, the Tenant Safety Coordinator or Alternate should proceed to take a head count to determine if anyone is missing from his or her office. If someone is missing, this information should be relayed to building personnel immediately. To help ensure that all Building occupants understand their role in an emergency, we have prepared a convenient online life safety training program for our tenants. We encourage all building occupants to participate in this online training. The entire training process takes less than 10 minutes. Fire Prevention Do not accumulate large quantities of discarded files or other paper trash in your office or storage area. Pay special attention to housekeeping in those departments that produce quantities of debris, such as copy and mail rooms. Do not store large quantities of flammable solvents, duplicating fluids, or other combustible fluids. Keep electrical appliances in good repair. Report unsafe conditions to the building office. When furnishing an office, consider the fire potential of materials used in large amounts, like overstuffed chairs, settees, couches or anything that could become a combustible item. Such furnishings should be flame-retardant. Where potential for fire is especially high, such as supply rooms, tenants may wish to consider installing additional fire extinguishers. Emergency Procedures: Armed Intruder All tenant employees and Building staff share responsibility for the security of 345 California Center as a work place. The Building Management and Building Security staffs try to provide tenants with a secure work environment, yet there is no guarantee of safety. The unpredictable nature of any violent situation makes it impossible to create a detailed plan of response; however, there are a few precautions which should be taken to prepare for and minimize exposure to a dangerous intruder: NEVER prop exit stairwell doors open. Under normal circumstances, access to tenant floors is impossible from inside the exit stairwells if the doors are closed. Know ahead of time how to communicate with Building Security. Security Office phone number: 415-434-7745. Red call boxes in exit stairwells every 4 floors (4, 8, 12, etc.) connect to Building Security Office. Alarm buttons in all elevators enable two-way communication with the Building Security Office. Armed Intruded (Continued) If instructed to lock your doors, do so immediately for your own safety. Do not explore to see what is happening or attempt to leave the Building. Follow any instructions given over the public address system by Building Management staff or the local authorities. Information and instructions will be updated as appropriate. Do not attempt to exit via the Building stairwells. The exit stairwells may be perilous if a dangerous intruder is present. Do not attempt to exit via the elevators. Once the presence of an armed intruder is reported to Building Security, elevators will be recalled to the ground floor lobby where they are available for police and rescue operations and unavailable to an intruder. Emergency Procedures: Bomb Threat Typically there are two reasons for a caller to report that a bomb is to go off at a particular location; the caller knows that an explosive or incendiary device has been or will be placed in the building and wants to minimize personal injury, or the caller wants to create an atmosphere that spreads panic and disrupts normal business activity. In either case, it is extremely important that the appropriate precautions be taken when dealing with any suspicious items. Suspicious Items Letters that are unusually bulky or weighty. Parcels or envelopes with chemical or oily stains. Parcels or envelopes without a return address. Parcels or envelopes with foreign postmarks. Parcels or envelopes that simply do not look or feel ordinary. Bomb threats demand a quick, clear response. The person receiving the call must be prepared to deal with the caller, record accurate information, and assist authorities in identifying the perpetrator. Precautions for Suspicious Items DO NOT handle the item. DO NOT attempt to open the parcel. DO NOT place the parcel in water. DO NOT remove any binding material. DO NOT pull or cut any protruding material. Notify building security and/or 911 immediately. Bomb Threat Procedures Attempt to obtain the following information: Attempt to obtain the following information: Who is calling and their phone number Where the bomb is located and when it is set to explode. Get the exact words of the person who made the threat. Immediately call 911 and Building Management at 415-434-7747. Building Security will dispatch an officer to the threat area to monitor the area and to give assistance as needed. Bomb Threat Procedures (Continued) San Francisco Police Department will be notified immediately. The Tenant Safety Coordinator in the affected area will be informed of the situation. Tenants should be alert for any unfamiliar people or objects, which will need to be pointed out to the police or building staff upon their arrival. DO NOT touch or handle any suspected objects. The Tenant Safety Coordinator, accompanied by the police and building staff, will make a complete search of the suspected area. The Tenant Safety Coordinator will be responsible for identifying any suspicious items or packages, which do not belong in their area. Who received the actual notification or threat. If the bomb threat is received against the building and not for a specific floor, all public access areas, beginning with the most accessible will be searched. An order to evacuate or the “ALL CLEAR” may be given by building management, the bomb squad unit or the Safety Coordinator. This responsibility rests solely with these personnel and their decision will be based on information given to them by emergency search party teams. The Tenant Safety Coordinator will only make evacuation decisions for their respective company. Emergency Procedures: Civil Disturbance Should a riot or civil disturbance start outside the Building, the security officers will immediately lock all entrances to the building. The San Francisco Police Department will be notified. We will keep you informed. If a disturbance should occur in the main lobby, all elevators will be turned off at the ground floor and lower level floors and the police will be summoned. Emergency Procedures: Earthquake Earthquake Preparedness Your company should prepare for the next major earthquake. It is important not to expect that help will be immediately available from the City or from the Building. In a major earthquake, San Francisco's downtown streets may be dangerous. Aftershocks will make the streets even more dangerous due to falling debris. Major routes to the city, particularly the bridges, could be impassable. The Police and Fire Departments will be in demand by everyone. Access and emergency assistance resources will be limited, and priority will be given to schools and hospitals. It could be days before outside help is available. A three day supply of stores is recommended, which should include the following: Food and water – at minimum, a 3 day supply (a 7 day supply is ideal). First Aid kit and First Aid manual. Large and small heavy-duty plastic bags, duct tape and scissors. Extra prescriptions and eyewear. Heavy-duty shoes and gloves. Rescue tools. Portable TV and/or radio with extra batteries. Flashlights and extra batteries. Cash (ATMs and credit cards may not work). Emergency Phone List, including family at work, school, daycare, etc. Cell phone. During an Earthquake During an earthquake, please observe the following: Remain calm, do not panic. Stay in the office area. Take cover under tables, desks, or strong doorways. Keep away from partitions and doors. Keep clear of filing cabinets, shelves and tall stacks of materials. Check for any injured personnel and administer first aid where necessary. Floor Wardens should assess damage and injuries and be prepared for aftershocks. In the event of a fire resulting from an earthquake, follow the fire emergency procedures as able. Elevators are equipped with seismic detectors, which when activated, will automatically stop the elevator at the nearest floor and activate the earthquake Public Address message. Please follow instructions issued by the Floor Warden and/or Building Staff. Communication during an earthquake Communications may be a significant problem in an earthquake. Outside telephone lines may be down, or so overloaded that connections cannot be made. Security will communicate with all floors via the public address system if it is operable. If the public address system should fail, runners will be designated, personnel permitting, to disseminate information. It is very important that you report your condition to Building Security. Your condition report should include: the number of people on your floor, the number of injured people on your floor with a brief description of the injuries, a brief description of any apparent structural damage or hazardous conditions on your floor. Try using your telephone to report to Building Security at 415-434-7745. If the phone is busy, keep trying. If you don't get a dial tone, the lines may just be overloaded; the phone may not really be dead. Wait a minute to see if you get a dial tone. If you succeed in getting through, keep your call short. Many others will also be trying to contact Security. If your telephones are not working, use the red emergency phones located at every fourth floor in the stairwells (floors 4, 8, 12, etc.). These connect directly with the Security office. Please keep your report brief. If you cannot get through, designate a runner to deliver your conditions report to the Building Security Office, which is located next to the loading dock on the Pine Street side of the Building. Choose an able-bodied person to use the stairs. Following an Earthquake Be prepared for aftershocks. While aftershocks are generally smaller than the earthquake that generates them, they can cause additional damage. Unless you see immediate danger, stay inside the building. DO NOT use the telephone except for extreme emergencies. DO NOT use the elevators under any circumstances. Follow instructions given over the public address system or by your floor's Emergency Response Team. There are no gas lines on tenant floors in the office building. Check for injuries and apply first aid as needed. Be prepared for an absence of immediate emergency services, and be prepared to help yourself and others. Extinguish any fires. Do not light matches. Do not smoke. Listen for news or pertinent instructions on Public Address, the radio or television. Conserve and responsibly ration food and water. Emergency Procedures: Flooding In the event of a flood that may cause damage to tenant property or affect the normal operation of the building, designated tenant representatives will be contacted by Building Management personnel, regardless of the time of day. The first priority is to ensure that no personal injury occurs as the result of a flood. The second priority is to discover the cause and prevent or minimize additional flooding. Once the flooding has been contained, clean-up operations will be commenced. Tenants will need to contact their insurance carrier for any damage to their property. Emergency Procedures: Homeland Security Building Management recommends that each tenant have an emergency action plan in place to help their employees prepare for, and react quickly to, a regional emergency, including terrorist attacks. Emergency Procedures: Medical Emergency In case of a medical emergency: Call either 911 or the San Francisco Emergency Ambulance at 415-431-2000. Give the exact location, and describe as fully as possible the apparent nature of the emergency and condition of the victim. Wait for the dispatcher to end the call. You must be the last person to hang up. Report to Building Security at 415-434-7745. Building Security will place an elevator on standby service for immediate access by ambulance personnel with their equipment, and will escort them directly to the emergency scene. If possible, position someone in the elevator lobby on your floor to direct the responding emergency teams to the victim(s). For optimum preparedness, 345 California Center recommends that tenants advise employees to wear medic alert identification or to inform their employers of chronic medical problems and allergic reactions to medications. Emergency Procedures: Pandemic Preparedness What you Need to Know An influenza (flu) pandemic is a worldwide outbreak of flu disease that occurs when a new type of influenza virus appears that people have not been exposed to before or have not been exposed to in a long time. The pandemic virus can cause serious illness because people do not have immunity to the new virus. Pandemics are different from seasonal outbreaks of influenza that we see every year. Seasonal influenza is caused by influenza virus types to which people have already been exposed. Its impact on society is less severe than a pandemic, and influenza vaccines are available to help prevent widespread illness from seasonal flu. Influenza pandemics are different from many of the other major public health and health care threats facing our country and the world. A pandemic will last much longer than most flu outbreaks and may include "waves" of influenza activity that last 6-8 weeks separated by months. The number of health care workers and first responders able to work may be reduced. Public health officials will not know how severe a pandemic will be until it begins. Pandemic Flu Resources There are many publicly available resources in place to help communities, companies, and individuals plan for a possible pandemic flu outbreak. Department of Homeland Security (DHS) DHS is working on a “Business Planning Guide,” which will be posted on the DHS home page and on Pandemicflu.gov as soon as it is completed. Also, for business-specific questions, the DHS has created an e-mailbox— DHSPandemic@dhs.gov. Emergency Procedures: Power Failure California Center is equipped with an emergency generator which will start automatically in the event of commercial power failure. In the event of power failure, this generator will provide emergency power for certain basic building functions. Those functions include: 1. Activating emergency lights on each floor throughout the building, including all Exit signs. 2. Activating all stairwell lighting. 3. Activating the building’s emergency Fire, Life and Safety Systems as well as the building’s communication systems. 4. Recalling all elevators to the ground floor lobby. (One elevator per bank will remain operative for use by security to assist handicapped persons or to take service crews and equipment into the building, as needed.) It is seldom necessary to evacuate the building during a power failure. Unless you are directed to do so through the emergency communication system, please remain in your offices. Power Failure (Continued) 345 California Center is designed to minimize the risk of a general power failure resulting from causes within the building. Typically, should a power failure occur, it will affect either an isolated area of the building or some larger portion of the surrounding geographic area. All suites and public areas are equipped with independently powered exit signs and emergency lights. These will remain lit in a general power failure. If an electrical failure does occur, the following guidelines should be observed: Contact the Building Management Office at 415-434-7747. Open the draperies and raise blinds to let in outside light. If there is adequate lighting from windows, continue performing assignments as well as possible. If you are instructed to evacuate, lock all areas. Do not congregate in lobby areas or in the street. If you are trapped in an elevator during a power failure, wait for assistance. Your elevator will cease operation, but WILL NOT FALL. Do not force the doors open. DO NOT PANIC! The Building Management Office will attempt to advise you regarding the length and cause of the power failure, as soon as possible. Turn off all electrical equipment in your area to reduce the load on the building when power is restored. Emergency Procedures: Toxic Hazards If there is a toxic spill or exposure, proceed immediately to an area where you are no longer exposed. Call 911. Provide the building’s address, your floor and phone number, and also what type of spill has occurred. Take appropriate action to contain the hazard; close doors behind you, and always follow all safety procedures when working with toxic materials. Policies and Procedures: Contractors Notice of proposed construction impacting Building Systems or the Building structure must be submitted to the Building Management Office for approval prior to commencement of the work. Building Systems include heating, ventilating, air-conditioning, plumbing, electrical, fire protection, life safety, security and other mechanical, electrical and communications systems of the Building. The Building Office requests that a written scope of work and a drawing of the proposed changes be submitted for review. Building or specialized trade permits are required by the Building Management Office in the event such permits are required by the City and County of San Francisco. This procedure is essential to protect critical Building Systems that serve and affect all of the tenants in the Building. The Building Management Office offers a handbook of Construction Rules and Regulations to assist tenants and contractors working at 345 California Center in completing tenant improvement and alteration projects efficiently. Copies are available upon request from the Building Management Office. It is our goal that these guidelines will facilitate communications and promote a positive relationship between Building management, contractors and tenants. We urge tenants to become thoroughly familiar with the Construction Rules and Regulations and to provide copies to contractors working in the Building. Contractor Insurance Certificate Requirements Prior to any work performed in the building, contractors must provide to the Building Management Office the proper certificate(s) of insurance. General Liability The required form for Certificate of Liability Insurance is Acord Form 25-S. The Certificate Holder should be Cushman and Wakefield of California, Inc. Requirements for general liability coverage per occurrence are in the amount of five million dollars ($5,000,000). Workers’ Compensation coverage is as required by law. Additional Insureds The required form indicating Additional Insureds is form CG 20 26 11 85. The certificate(s) must name 345 California L.P., Cushman & Wakefield of California, Inc. and its Affiliates, Metropolis Investment Holdings Inc, 345 California Street Commercial Condominium Owners Association and Bayerische Landesbank Girozentrale as additional insureds. The certificate(s) should be faxed to 415-434-7703 prior to the scheduled work. A hard copy of the certificate(s) should be mailed to the certificate holder - Cushman & Wakefield, 345 California Street, Suite 475, San Francisco, CA 94104. Please direct any questions to the Building Management Office at 415-434-7747. Policies and Procedures: Office Building General Rules and Regulations 1. The sidewalks, halls, passages, exits, entrances, elevators and stairways of the Project shall not be obstructed by Tenant or used by it for any purpose other than for ingress to and egress from the Premises. The halls, passages, exits, entrances, elevators, escalators and stairways are not for the use of the general public, and Landlord shall in all cases retain the right to control and prevent access thereto of all persons whose presence in the judgment of the Landlord would be prejudicial to the safety, character, reputation and interests of the Project and its tenants, provided that nothing herein contained shall be construed to prevent such access to persons with whom Tenant normally deals in the ordinary course of its business, unless such persons are engaged in illegal activities. Tenant shall not go upon the roof of the Project. 2. No sign, placard, picture, name, advertisement or notice visible from the exterior of the Premises shall be inscribed, painted, affixed or otherwise displayed by Tenant on any part of the Project without the prior written consent of Landlord. Landlord will adopt and furnish to Tenant general guidelines relating to signs inside the Project on the office floors, and Tenant agrees to conform to such guidelines. All approved signs or lettering on doors shall be printed, painted, affixed or inscribed at the expense of Tenant by a person approved by Landlord, which approval will not be unreasonably withheld. Material visible from outside the Project will not be permitted. 3. The Premises shall not be used for the storage of merchandise held for sale to the general public or for lodging. Tenant shall not use or permit the use of any premises in any manner which involves an unusual risk of injury to any person. No cooking shall be done or permitted by Tenant on the Premises, except that use by Tenant of Underwriters' Laboratory-approved portable equipment for brewing coffee, tea, hot chocolate and similar beverages shall be permitted, provided that such use is in accordance with all applicable federal, state and local laws, codes, ordinances, rules and regulations. 4. Tenant shall not employ any person or persons other than the janitor of Landlord for the purpose of cleaning the Premises, unless otherwise agreed to by Landlord in writing. Except with the written consent of Landlord, no person or persons other than those approved by Landlord shall be permitted to enter the Project for the purpose of cleaning same. Tenant shall not cause any unnecessary labor by reason of Tenant's carelessness or indifference in the preservation of good order and cleanliness. Janitor service will not be furnished on nights when rooms are occupied after 9:00 p.m. unless, by agreement in writing, service is extended to a later hour for specifically designated rooms. 5. Landlord will furnish Tenant with two (2) keys to the Premises, free of charge. 6. Additional keys shall be obtained only from Landlord and Landlord may make a reasonable charge for such additional keys. No additional locking devices shall be installed in the Premises by Tenant, nor shall any locking device be changed or altered in any respect without the prior written consent of Landlord. All locks installed in the Premises, excluding Tenant's vaults and safes, or special security areas (which shall be designated by Tenant in a written notice to Landlord), shall be keyed to the Building master key system. Landlord may make reasonable charge for any additional lock or any bolt (including labor) installed on any door of the Premises. Tenant, upon the termination of its tenancy, shall deliver to Landlord all keys to doors in the Premises. 7. The elevators to be used for the loading of freight shall be available to Tenant in accordance with such reasonable scheduling as Landlord shall deem appropriate. Tenant shall schedule with Landlord, by written notice given no less than forty-eight (48) hours in advance, its move into or out of the Project, which moving shall occur only on weekend days if required by Landlord; and Tenant shall reimburse Landlord upon demand for any additional security or other charges incurred by Landlord as a consequence of such moving. The persons employed by Tenant to move equipment or other items in or out of the Project must be acceptable to Landlord. The floors, corners and walls of elevators and corridors used for the moving of equipment or other items in or out of the Project must be adequately covered, padded and protected, and Landlord may provide such padding and protection at Tenant's expense if Landlord determines that such measures undertaken by Tenant or Tenant's movers are inadequate. Landlord shall have the right to prescribe the weight, size and position of all equipment, materials, supplies, furniture or other property brought into the Project. Heavy objects shall, if considered necessary by Landlord, stand on wood strips of such thickness as is necessary to properly distribute the weight of such objects. Landlord will not be responsible for loss of or damage to any such property from any cause, and all damage done to the Project by moving or maintaining Tenant's property shall be repaired at the expense of Tenant. 8. Tenant shall not use or keep in Premises or the Project any kerosene, gasoline or flammable, combustible or noxious fluid or materials or use any method of heating or air conditioning other than that supplied by Landlord. Tenant shall not use, keep or permit or suffer the Premises to be occupied or used in a manner offensive or objectionable to Landlord or other occupants of the Project by reason of noise, odors, and/or vibrations, or interfere in any way with other tenants or those having business in the Project. 9. Landlord reserves the right to exclude from the Project between the hours of 6:00 p.m. and 6:00 a.m. and at all hours on Saturdays, Sundays, and legal holidays all persons who do not present a pass to the Project signed by Landlord and properly in the possession of the person presenting such pass. Landlord will furnish passes to persons for whom Tenant requests same in writing. Tenant shall be responsible for all persons for whom it requests passes and shall be liable to Landlord for all acts of such persons. Landlord shall in no case be liable for damages for any error with regard to the admission to or exclusion from the Project of any person. In the case of invasion, mob, riot, public excitement or other circumstances rendering such action advisable in Landlord's opinion, Landlord reserves the right to prevent access to the Project during the continuance of same by such action as Landlord may deem appropriate, including closing any doors in the Project. 10. The directory of the Project will be provided for the display of the name and location of tenants and a reasonable number of the principal officers and employees of tenants, and Landlord reserves the right to exclude any other names therefrom. Any additional name that Tenant shall desire to place upon the directory must first be approved by Landlord and, if so approved, a charge will be made for each such name. 11. No curtains, draperies, blinds, shutters, shades, screens or other coverings, hangings or decorations shall be attached to, hung or placed in, or used in connection with any window of the Project without the prior written consent of the Landlord. In any event, with the prior written consent of the Landlord, such items shall be installed on the office side of Landlord's standard window covering and shall in no way be visible from the exterior of the Project. 12. Tenant shall not obtain for use in the Premises ice, drinking water, food, beverage, towel, barbering, shoe polishing or other similar services, without Landlord's prior written consent. The provision of any such goods and services shall be subject to such reasonable regulations and at such reasonable hours as may be fixed by Landlord. The suppliers of such services shall be subject to Landlord's approval. 13. Tenant shall see that the doors of the Premises are closed and locked and that all water faucets, water apparatus and utilities are shut off before Tenant or Tenant's employees leave the Premises, so as to prevent waste or damage, and for any default or carelessness in this regard Tenant shall make good all injuries sustained by other tenants or occupants of the Project or Landlord. On multiple-tenancy floors, all tenants shall keep the doors to the Project corridors closed at all times except for ingress and egress, and all tenants shall at all times comply with any rules or orders of the fire department with respect to ingress and egress. 14. The toilet rooms, toilets, urinals, wash bowls and other apparatus shall not be used for any purpose other than that for which they were constructed, no foreign substance of any kind whatsoever shall be deposited therein, and any damage resulting to same from Tenant's misuse thereof shall be paid for by Tenant. 15. Except with the prior written consent of Landlord, Tenant shall not sell, or permit the sale from the Premises of, or use or permit the use of any sidewalk or mall area adjacent to the Premises for the sale of newspapers, magazines, periodicals, theater tickets or any other goods, merchandise or service, nor shall Tenant carry on, or permit or allow any employee or other person to carry on, business in or from the Premises for the service or accommodation of occupants of any other portion of the Project, nor shall the Premises be used for manufacturing of any kind, or for any business or activity other than that specifically provided for in Tenant's lease. 16. Tenant shall not install any radio or television antenna, loudspeaker, or other device on or about the roof area or exterior walls of the Project. 17. Tenant shall not use in any space, or in the common areas of the Project, any hand trucks except those equipped with rubber tires and side guards or such other material-handling equipment as Landlord may approve. No other vehicles of any kind shall be brought by Tenant into the Project or kept in or about the Premises. 18. Tenant shall store all its trash and garbage within the Premises until removal of same to such location in the Project as may be designated from time to time by Landlord. No material shall be placed in the Project trash boxes or receptacle if such material is of such nature that it may not be disposed of in the ordinary and customary manner of removing and disposing of trash and garbage in the City of San Francisco without being in violation of any law or ordinance governing such disposal. 19. All loading and unloading of merchandise, supplies, materials, garbage and refuse shall be made only through such entryways and freight elevators and at such times as Landlord shall designate. In its use of the loading areas of the Project, Tenant shall not obstruct or permit the obstruction of said loading areas, and at no time shall Tenant park vehicles therein except for immediate loading and unloading purposes. 20. Canvassing, soliciting, peddling or distribution of handbills or any other written material in the Project is prohibited and Tenant shall cooperate to prevent same. 21. Tenant shall immediately, upon request from Landlord (which request need not be in writing), reduce its lighting in the Premises for temporary periods designated by Landlord, when required in Landlord's judgment to prevent overloads of the mechanical or electrical systems of the Project. 22. Landlord reserves the right to select the name of the Project and to make such change or changes of name as it may deem appropriate from time to time, and Tenant shall not refer to the Project by any name other than: (I) the name as selected by Landlord (as same may be changed from time to time), or (ii) the postal address, approved by the United States Post Office. Tenant shall not use the name of the Project in any respect other than as an address of its operation in the Project without the prior written consent of Landlord. 23. Tenant assumes all responsibility for protecting its Premises from theft, robbery and pilferage, which includes keeping doors locked and other means of entry closed. 24. No vending machine or equipment relating to ice, drinking water, food or beverages shall be maintained or operated within the Premises or the Project without the prior written consent of Landlord. Any food or beverages permitted in writing by Landlord to be brought onto the Premises must be properly stored and maintained at all times. 25. All incoming mail and package deliveries shall be received at the area in the Building designated by Landlord for such purposes and distributed through means established by Landlord. No messenger or other delivery personnel shall be permitted to enter any area of the Project other than the area designated by Landlord for the pick-up and receipt of such deliveries. 26. Landlord reserves the right to exclude or expel from the Project any person who is, in the judgment of Landlord, intoxicated or under the influence of alcohol or other drug or who is in violation of any of the Rules and Regulations of the Project. 27. No animal or bird shall be permitted in the Premises or the Project. 28. The requirements of Tenant will be attended to only upon application by telephone or writing or in person at the Management Office of the Project. Employees of Landlord shall not perform any work or do anything outside of their regular duties unless under special instructions from Landlord. 29. Landlord may waive any one or more of these Rules and Regulations for the benefit of any particular tenant or tenants, but no such waive by Landlord shall be construed as a waiver of these Rules and Regulations in favor of any other tenant or tenants, nor prevent Landlord from thereafter enforcing any such Rules and Regulations against any or all of the tenants of the Project. 30. Wherever the word "Tenant" occurs in these Rules and Regulations, it is understood and agreed that it shall mean Tenant's associates, agents, clerks, employees and visitors. Wherever the word "Landlord" occurs in these Rules and Regulations, it is understood and agreed that it shall mean Landlord's assigns, agents, clerks, employees and visitors. 31. Landlord reserves the right to make such other and reasonable rules and regulations as in its judgment may from time to time be needed for the safety, care and cleanliness of the Project, and for the preservation of good order therein. 32. Tenant shall be responsible for the observance of all of the foregoing rules and regulations by Tenant's employees, agents, clients, customers, invitees and guests. Policies and Procedures: Tenant Insurance Requirements Current certificate(s) of insurance are required to be on file with the Building Management Office evidencing appropriate coverage for tenants and their contractors. Requirements are as follows: The required form for Certificate of Liability Insurance is Acord Form 25-S. The Certificate Holder should be 345 California L.P. Requirements for general liability coverage are the amount specified in the tenant’s lease. Workers’ Compensation coverage is as required by law. The required form indicating Additional Insureds is form CG 20 26 11 85. The certificate(s) must name 345 California L.P., Cushman & Wakefield of California, Inc and its Affiliates, Metropolis Investment Holdings, Inc, 345 California Street Commercial Condominium Owners Association and Bayerische Landesbank Girozentrale as additional insureds. The certificate(s) should be submitted to the Building Management Office prior to occupancy. Certificate(s) should be updated as needed to remain current. Certificate(s) may be mailed to 345 California L.P., 345 California Street, Suite 475, San Francisco, CA 94104. Policies and Procedures: Moving Procedures It is necessary to make special arrangements with the Building Management Office before a move or shipment. An After Hours / Weekend Access Request should be completed by the tenant and sent to the Building Management Office at least 48 hours prior to the scheduled move. This form identifies all vendors that will be on site during the move. An authorized person as designated on the Tenant Authorization Form must sign this form. Reservation of the freight elevator may occur at this time. Only the freight elevator is available for moves and deliveries. Reservations of the freight elevator are on a first come, first serve basis. All moves must be scheduled after business hours (Monday through Friday, 1:00 AM – 5:00 AM and 6:00 PM – 10:00 PM or anytime on weekends. Loading dock The freight elevator is accessible via the loading dock. Pallet jacks are not permitted beyond the loading dock. Delivery/moving trucks may be present in the loading dock when loading or off-loading only. Parking is not permitted on the loading dock. Aisleways and emergency exits in the loading dock may not be obstructed at any time. Building security staff may direct the move or delivery at the loading dock as necessary to ensure normal building and hotel operations are not impacted. Dimensions of the freight elevator Door: 4’0” W x 8’7” H Cab: 5’5” W x 10’8” L x 127" H Capacity: 4,500 lbs Insurance requirements for moving vendors Prior to any move the tenant should arrange for the moving contractor to provide the Building Management Office with proper certificate(s) of insurance. Workers’ Compensation coverage is as required by law. The required form indicating Additional Insureds is form CG 20 26 11 85. The certificate(s) must name 345 California L.P., Cushman & Wakefield of California, Inc. and its Affiliates, Bayerische Landesbank Girozentrale, Metropolis Investment Holdings Inc, and 345 California Commercial Condominium Owners Association as additional insureds. The certificate(s) should be faxed to (415) 434-7703 prior to the scheduled move. A hardcopy of the certificate(s) should be mailed to 345 California L.P., 345 California Street, Suite 475, San Francisco, CA 94104. Care and protection Please advise the moving contractor that corridors and doorways should not be obstructed, and that corridors and public areas of the Building must be protected from damage. Masonite or plywood and corner boards should be used in corridors and lobbies leading from the freight elevator to the tenant area. Walk-off mats should also be used to protect door thresholds. An inspection of public areas will be conducted prior to the move. If damage occurs, required repairs will be made at the tenant’s expense. Please remove protective materials and any empty containers or debris the same day as the move. You may take these materials to the compactor on the loading dock, or make arrangements with the Building Management Office in advance for supplemental janitorial services. Policies and Procedures: Smoking San Francisco Ordinance No. 359-93 prohibits smoking in places of employment. Smoking is not permitted at 345 California Center.
