Author Rebecca Skloot at Long Beach Public Library Award-winning writer Rebecca Skloot will discuss her new book, The Immortal Life of Henrietta Lacks on Saturday, February 6 at 2p.m. Henrietta Lacks was a 31-year-old mother of five in Baltimore when she died of cervical cancer in 1951. To help it do its job – and prevent heart disease – take stock of your diet. “What you eat and how you prepare food can strongly affect your blood cholesterol, your blood pressure and the propensity for plaque to build up in your arteries over the long run,” says Lori Mosca, M.D., director of preventive cardiology at New York-Presbyterian Hospital/Columbia Medical Center, in New York City. The good news is that eating “heart healthy” doesn’t require any special regimen; it’s just a balanced plan that follows the Dietary Guidelines for Americans and also helps ward off cancer, diabetes and hypertension, especially if you keep tabs on calories. But considering that plenty of notso-good-for-you foods are just a drive-thru away, we could all use a little inspiration and guidance. You’re never too young to take good care of your heart. Read on for the key nutrition rules that will help you and your family eat to beat heart disease. Women who consumed an average of twoand-one-half servings of whole grains daily had at least a 30 percent lower risk of cardiovascular disease than women who consumed less than one serving daily. Target Saturated and Trans Fats To keep your arteries clear, cut down on your intake of both saturated fat (the kind that’s solid at room temperature and found in many types of animal products) and trans fats (processed vegetable oil in which hydrogen has been added to the molecule to make it more solid and give it a longer shelf life). Both types raise your body’s level of “bad” LDL cholesterol – much more so than any cholesterol you get from food. When too much LDL cholesterol circulates in the bloodstream, it can slowly build up on the walls of arteries feeding your heart and brain, form- ing thick, hard plaque. Trans fats also lower “good” HDL cholesterol, making them doubly bad for your heart. HDL cholesterol is beneficial because it reduces plaque buildup by ushering excess LDL cholesterol from artery walls and back to the liver, where it’s passed from the body. Food Fix: Aim to have no more than 30 percent of your calories come from fat, with 10 percent or less from saturated and trans fat. “Limit butter, vegetable shortening and lard in cooking,” suggests Bethany Thayer, R.D., a spokesperson for the American Dietetic Association. Instead, use olive and canola oils, which both contain heart-healthy 12 • February 2010 • LONG ISLAND WOMAN monounsaturated fat. Because these healthy fats are still high in calories (120 calories per tablespoon), go easy to avoid weight gain, which is an independent risk factor for heart disease. Also, instead of butter or regular margarine, consider spreads such as Take Control or Benecol, which are enriched with plant sterols or stanols – compounds that can inhibit the absorption of cholesterol in the intestine, thereby lowering LDLs. (Products such as Yoplait Healthy Heart yogurt and Minute Maid Heart Wise orange juice also contain plant sterols.) Other ways to trim saturated fats from your diet: Drink skim or low-fat milk rather than 2 percent or whole. (Toddlers can switch to skim or low-fat milk starting at age 2.) “And choose lean meats and skinless poultry, keeping servings to about the size of your palm,” suggests Thayer. Finally, avoid processed cookies, crackers, chips and bakery products, which are often high in trans fats (also called “hydrogenated” or “partially hydrogenated” fat or oil in the ingredient list). Check the nutrition label to find snacks that are low in both saturated and trans fat. Eat More Whole Grains Whole-grain bread and cereals as well as beans, barley and lentils are good sources of vitamins A, B and E. These act as antioxidants, which may help neutralize free radicals – unstable oxygen molecules in the blood that may contribute to plaque buildup in the arteries. Whole grains are also “packed with fiber, which is potent in lowering LDL,” says Wahida Karmally, R.D., director of nutrition at the Irving Center for Clinical Research at Columbia University in New York City. Food Fix: Because most of us get only 15 grams of fiber per day, while 25 to 30 grams is ideal, it’s wise to choose whole grain cereal such as instant oatmeal and opt for whole grain bread instead of white as often as possible. Don’t worry: the other people in your home will get used to it. Aim for at least three, one-ounce servings of whole grains per day. The Nurses’ Health Study, which followed more than 75,000 women over 10 years, found that those who consumed an average of two-andone-half servings of whole grains daily (from whole grain breakfast cereal to popcorn) had at least a 30 percent lower risk of cardiovascular disease than women who consumed the least (less than one serving daily). According to the study, the more whole grains you consume, the greater your protection against heart disease is likely to be. To spot whole grain products, look for whole on the nutrition label, as in “whole wheat,” “whole corn” or “whole rye.” Also, check the fiber content. Look for foods with 2 or more grams of fiber per serving. Pack In Produce Like whole grains, fruits and vegetables are filled with fiber as well as beta-carotene and the antioxidant vitamins A and C. Ease Up On Eggs While it’s true that dietary cholesterol doesn’t affect your blood cholesterol to the extent that saturated and trans fats can, the dietary cholesterol in egg yolks can add up (a typical yolk contains 71 percent of the daily limit of 300 milligrams), and may contribute to high levels of LDL in your body. That’s why many nutritionists are conservative when it comes to recommending a daily limit for egg consumption. “Don’t have more than one egg yolk a day,” advises Melissa Ohlson, M.S., R.D., nutrition project coordinator for preventive cardiology at the Cleveland Clinic in Ohio. (If you already have high cholesterol, however, Ohlson recommends limiting eggs to no more than three per week.) There’s no need to buy eggs enriched with DHA and EPA, which can cost as much as $1.50 more per dozen than regular eggs. They contain relatively little heart-healthy DHA and EPA, and they’re only slightly lower in cholesterol, says Ohlson. Even though the Chief Medical Editor for NBC News is out the door by 6 A.M., she still manages to juggle three kids, a husband, teaching responsibilities at University of Pennsylvania and, of course, hosting “Dr. Nancy” weekdays at noon, eastern standard time, on MSNBC. Her secret? Lists. “I am a big believer in lists. Make two columns on a piece of paper: things I love about my life and things I hate about my life,” says the Fort Wayne, Indiana native (born in St. Louis, Missouri). “You’ll be surprised about how much you like yourself. But the things you don’t like, dump them. Get rid of them. If they don’t bring you value, and I mean spiritual nourishment and value to your soul, dump them.” Dr. Snyderman joined NBC News (the job she heads to before most of us even contemplate rolling out of bed) in September 2006. But that was not the doctor’s first foray into television news. She is a 17-year veteran health correspondent for news outlets on ABC network, including 20/20, Primetime and Good Morning America. Her assignments brought her around the globe covering the challenges of women and their health. “Nancy has always been an outspoken advocate for women’s health with insights as a physician, wife and mother,” according to Ami Schmitz, Dr. Snyderman’s senior medical producer for 15 years. “She will always stand up for the truth, even if it’s not popular, because that’s what our viewers have come to count on from her: that she will tell it to them straight.” Currently, her reports appear on Today, NBC Nightly News with Brian Williams, Dateline NBC, MSNBC and MSNBC.com. Before she became “Dr. Nancy,” the Indiana University undergrad, 1977 University of Nebraska School of Medicine grad was a practicing otolaryngologist – an ear, nose and throat doctor or ENT. “I’ve always had patients. I’ve always maintained either an active or private practice along with being a medical correspondent. The reason it was very important to me is that I grew up to be a doctor, I didn’t grow up to be on TV,” she quips. Dr. Snyderman’s father was a surgical ENT. Prior to becoming a surgical ENT herself, the author of Medical Myths That Can Kill You: And the 101 Truths That Will Save, Extend, and Improve Your Life, considered becoming a pediatrician as her own “mini-rebellion.” While she soon realized pediatrics was not her career path, she never wavered in her desire to be an educator, whether in front of the camera or while wearing a physician’s jacket. “I found that the skill set of speaking to people bedside is the same skill set as talking to people on television. It’s taking a complicated thing and condensing it to make it simpler without being condescending,” recalls Dr. Snyderman. “And I’ve always maintained that I am a better correspondent if I am seeing patients and being a practicing physician. I am a much smarter doctor because I’ve traveled the world and interviewed people and seen things outside of my own field. So, to me, they have been phenomenally complimentary.” It was during her residency at the University of Pittsburgh that she began to work in broadcasting. The dual career path continued, after becoming a staff surgeon in Little Rock, Arkansas in 1983. Ever the multi-tasker, she has always been able to weave the two callings together. Dr. Snyderman, however, made the decision to stop seeing patients in 2004, to focus on her other passions, including teaching in the Department of Otolaryngology-Head and Neck Surgery at the University of Pennsylvania. “I have come to some proverbial forks in the road where I’ve had to make decisions. When I moved to the East Coast five years ago, I closed my private practice. So, now I teach medical students and residents, but I’m not taking new patients because the job I’m doing at NBC is so big. I had to think, ‘Can I do it all well?’ And I made that quiet decision on my own to leave at the top of my game as a surgeon but to still keep my feet in medicine,” reflects the former Vice President of Consumer Education for Johnson & Johnson. “So that is how I’ve balanced things. But we all make decisions about what we can have on our plate and what we can’t. For me, I can balance medicine and broadcasting as long as I limit my medicine a little bit.” Taking off one hat to put on another is how Dr. Snyderman, like the majority of her female audience, juggles life’s challenges and the stresses that ensue. While many people live by their Blackberry, Dr. Snyderman still carries a paper calendar . Every year, she takes the time to write down important birthdays and school vacation schedules, school plays, etc., and then begins to schedule her life around all of that. “My oldest child is 23, and I’m proud to say that I’ve never missed a kid’s birthday.” In a profession that keeps her on the road and traveling to where the news is, Dr. Snyderman can recall taking her three children, Kate, Rachel and Charlie, on work trips, to the hospital while she was seeing patients, and even to the studio. Realizing this may not be an option for every woman, she believes allowing her children to witness her at work, has helped her teach them. “I think it’s OK for kids to see that you work hard and struggle and make hard choices, not always the popular choice. But they have to think that at least you’re trying. Overall, I’ve been blessed with having clarity of what is important and what’s not. Everyone has known that I’ve done the best that I possibly can.” Still, the question for millions of women over 40 remains, “How can I do it all?” when “help” feels like another four-letter word. Disproportionate caregiving responsibilities are among the risk factors that can spark disorders such as depression and anxiety in women, according to the World Health Organization. Dr. Snyderman reported on this in “A Woman’s Nation: Are Women Making Themselves Sick?,” a series which aired on NBC News in October 2009 in conjunction with the findings from a study conducted in partnership with The Center for American Progress, The Shriver Report: A Woman’s Nation Changes Everything. Schmitz explains that Maria Shriver’s group put together a dialogue-starter in the truest sense. “They assembled the data on a wide variety of topics and presented NBC News with the opportunity to tell the story of America’s women – what was working and what was not. In the case of caregiving, many of us find ourselves sandwiched between generations: caring for our young as well as our aging parents and relatives, often while working a full-time job. And it’s exacting a terrible toll on our health and well-being,” she adds. Exhaustion, depression, high blood pressure, dehydration and heart disease – these are all illnesses stemming from stress, which negatively affects every organ system in the body. Many of these illnesses are common in caregivers, who tend to be women over the age of 40 that habitually put their own health needs last. Stress, Dr. Snyderman feels, has become the silent killer of women. “We know that women are under more stress than ever. Stress leads to higher blood pressure, stress hormones and obesity. We reach for fat and carbs when we are stressed. Stress leads to depression. Stress is not only psychologically damaging when it’s unrelenting but it is physically damaging,” according to Dr. Snyderman. “Women are nurturers, but we put ourselves on the back burner since we are biologically wired to nurture.” The solution is that women acting as caregivers need to put their own health needs first and manage their health as thoroughly as they manage their careers and families. Her article “Taking Care of the Caregiver,” posted on BeWell.com, discusses the higher death rate in women once becoming the primary caregiver for her spouse. The increased health risks are due to basic overextension. The physicality of helping an ailing family member with their daily hygiene, transportation and monitoring new dietary needs, which are difficult even for a trained professional, can prove overwhelming. These factors are compounded by the emotional hardship of seeing a loved one in need. By maintaining an annual schedule, there is less likelihood of pivotal tests and checkups falling by the wayside. “My birthday is March 26, so I look at the month of March as my gift to myself. I know about this time of year to start booking things – my colonoscopy, my mammogram,” Dr. Snyderman suggests. “I tell women to pick a significant day and that is what you strive for because when you say ‘Oh, I’ll get it in the spring,’ then it’s the summer and before you know it it’s the fall.” Caregivers are nearly twice as likely as non-caregivers to report a chronic condition, and they are also less likely to seek out medical attention. Such a factor makes annual tests and doctor visits, all the more crucial for this group of high risk individuals. Because caregivers become so consumed with the care of the patient/loved one, they do not see, or ignore, the warning signs of their own declining health. “Women almost like permission to say we matter. If you put yourself first, it’s not selfish, it’s self-preservation,” Dr. Snyderman emphasizes. “A lot of people depend on us, so the best thing we can do is take care of ourselves. Do not assume you can abuse yourself and that it is going to be someone else’s problem later in life. For all of us who like control of our lives, taking care of yourself is the best thing you can possibly do.” Dr. Snyderman believes that her cravings for a balanced diet and healthy foods help her maintain her schedule. “We have sit-down family dinners on the nights that I’m not on the nightly news. It’s been the only sort of rule in our family for years. We eat simply;. I’m a good cook, not a fancy cook.” As for physical activity, when she is not horseback riding, the doctor tries to walk or hike for exercise. Dr. Snyderman truly credits her equestrian hobby with helping keep life in perspective. “I usually try to get out three or four times a week,” says the longtime riding enthusiast. “The horse is good for me physically, and the great thing about riding is that you can’t think about the outside world. You really have an hour where you have to purge your brain of all the other junk that infiltrates it.” For many women over 40, another stressor is the aging process and its physical as well as mental toll – which leads many women to consider plastic surgery. In the nearly 11.7 million surgical and nonsurgical cosmetic procedures performed in the United States in 2007, as reported by the American Society for Aesthetic Plastic Surgery (ASAPS), women accounted for 91 percent of that total. Plastic surgery is a subject Dr. Snyderman covered extensively as a correspondent for Good Morning America. She presented a special five-part series, which was honored with the 1990 Journalism Award of Excellence by the American Academy of Facial, Plastic and Reconstructive Surgery. “Probably the number one thing people never bother to ask their doctor is, ‘Do you have privileges at a local hospital?’ A lot of doctors just have a surgical room in their office. If there is an emergency, they don’t have any way of taking care of you at a local hospital,” cautions Dr. Snyderman. Statistics from the ASAPS illustrate the need for this type of inquiry: in 2007, over 54 percent of cosmetic procedures were performed in an office facility; only 17 percent in a hospital. The remaining 29 percent were completed at free-standing surgicenters. Alan Matarasso, M.D., Clinical Professor at Albert Einstein College of Medicine concurs, women need to make sure they recognize all the details of a procedure before going under the knife. When considering any form of plastic surgery, potential patients should always realize the pre- and post-operative minutiae about possible complications and long-term consequences. Patients, in many cases, also do not grasp the length of time necessary for healing and should be clear with their physician about when pre-operative daily routines can resume. “You don’t want to be sorting out these details after the fact,” emphasizes Dr. Matarasso. “Carefully select your surgeon. It’s important to do thorough research in selecting a board certified plastic surgeon.” Dr. Snyderman also wonders about women’s motivation for getting plastic surgery. According to the American Academy of Cosmetic Surgery (AACS), the average age of patients receiving cosmetic surgery is 41. Dr. Snyderman encourages women to find out what is really important to them as they age, before rushing to the operating table. “Those are very personal decisions. I think the biggest concern I have about women in plastic surgery is if they are competing with their teenage daughter rather than wanting to look their best,” she says. As much as she preaches self-care, Dr. Snyderman must work at taking care of her own health. Despite advising patients and humanizing critical illnesses, she admits she is not immune to being less than perfect, at times, when it comes to managing her own health. “Some weeks are better than others. Now with the economy the way it is, it feels nearly impossible to divide the household because supporting one household has gotten even tougher. Our investments have been shrinking and so has the equity in our homes. Emotions are flaring. Not to mention the huge cost of attorneys fees for separation and divorce. The time for alternate means of obtaining a divorce or separation is even more crucial. Breaking up is surely hard to do, especially in these times of uncertainty. But there are solutions. Divorce Mediation and Collaborative Divorce are out there now and are proven to be successful. “Divorce mediation” allows for a less expensive means of separating or divorcing and allows for both spouses to be able to start over peacefully. “Collaborative divorce” allows for a couple to be able to obtain a separation or divorce with the additional help of legal and financial professionals to guide couples in this time of financial uncertainty. In either of the litigation alternatives, “mediation” or “collaboration,” flexibility and creativity is the key. With mediation or collaborative divorce the parties can agree on a figure to start with and then agree to adjust the amount of support paid at intervals throughout the economic times of recession and recovery. With collaboration, the process includes in most cases a Certified Divorce Financial Analyst (CDFA) or a Certified Financial Planner (CFP) on the team to assist in the calculations and generating options. With mediation, if the parties and the mediator find that one would be helpful, a CDFA or CFP can be called in. Both of these approaches help the couple to accomplish the separation or divorce they are seeking now, rather than waiting until the financial situation improves. The same types of creativity can be established with the division of property including the marital residence and retirement plans. A destination worthy of its own trip is Vancouver Island, where nature’s bounty awaits with an abundance of wildlife and scenic wonders. We began our adventure with a quick 30-minute drive from the Vancouver airport to Nainamo where we boarded a ferry (courtesy of BC Ferries) to Tsawassen. The ferry had the feel of a large cruise ship, with a spacious sun deck, lots of comfortable seating and tables, a playroom for the kids, a cafeteria with a Starbucks café, gift shops and even a pet area. Despite the fact that our ferry was sold out, there was plenty of room on board. (It is advised to book in advance as ferry rides usually sell out during peak season.) We headed west to Tofino to explore the Pacific Rim. The picturesque drive took us past majestic mountains and bright blue lakes that demanded we break for hikes and photo ops. In Tofino, we stayed at Long Beach Lodge, a rustic and comfortable oceanfront resort on Cox Bay. The bay is a haven for surfers, and the lodge’s Great Room has an expansive view of the bay from its comfy couches and plush sitting chairs. It also doubles as a restaurant/bar. On the Northeast side of the Island, we visited the harbor town of Port McNeill. It’s a sleepy little town surrounded by abundant wildlife. There are a few bed and breakfasts available along with The Black Bear Resort, where we stayed overnight. Not quite a resort, but the rooms were acceptable and there was an indoor pool and spa services available. Instead of booking space on a large whale-watching boat, we were fortunate to find Ocean Rose Coastal Adventures. Our guide, Angela (she’s also the proprietor), was a delight, and she shared her wealth of information as we skimmed the water on her Zodiac. The Zodiac is a great way to view the coastal offerings. It’s an intimate motorized raft that is swift yet remarkably comfortable and seasick proof. Angela has an innate sonar capability for marine and wildlife with plenty of experience in the surrounding waters. During our thrilling tour, we had upclose and personal viewings of dolphins, humpback, orca and minke whales, sea lions and bald eagles. Angela supplies everything you need, from lunch (her homemade cookies were a nice touch) to the gear to keep you warm and safe. From Port McNeill, we were on our way – via helicopter – to the highpoint of the vacation: Nimmo Bay. Nimmo Bay lived up to its promise and then some! The helicopter ride was jaw dropping as we weaved around the mountains, over the water and above the clouds where we landed atop a glistening snow-topped glacier. We spent a few minutes taking in the sights from what seemed like the top of the world, and then got back in the helicopter to get to Nimmo Bay. We descended from the clouds into a clearing that opened like curtains onto this little Shangri-La. It’s a Fantasy-Island-like resort, only accessible by helicopter, that accommodates up to 18 guests. For those who enjoy fishing (catch and release), it doesn’t get better than this. Heli-fishing is the big lure here; helicopters provide access to remote streams and rivers where the fish are plentiful, and the scenery and tranquility are breathtaking. Nimmo is also known for pampering its guests and individualized attention. The staff anticipates your needs before you do. This is a family-run operation where the cheery staff act and feel like part of the family as well. The atmosphere of caring and attention to detail starts from the top; owner and founder Craig Murray creates the inimitable atmosphere, and his true love for his natural surroundings is contagious.
