Brooklyn native Barbara Cataletto was a gym teacher who later became a medical coder in the spine field. Today, she is a noted author, lecturer and recognized subject matter expert in spine coding and reimbursement. The Lloyd Neck resident founded her own company, Business Dynamics LLC, a Mineola-based business that has grown steadily over the years. To date, BDL has submitted more than 300,000 surgical spine claims, totaling close to 1 billion dollars in revenue, and spinning off three subsidiaries: Business Dynamics Consulting – customized consulting for the spine industry; The Business of Spine, advanced education for the coder who wants to specialize in spine, and Business Dynamics Online – webinars and online courses for the spine specialist. When Barbara started her career in this field, there was no footprint to follow, so she blazed her own trail. To ease the burden of spine coding, Barbara developed CaseCoder™ the only web-based “spine specific” coding program available today. Barbara is also a patient advocate who has taken appeals for payments to various Attorneys General offices and CEOs of health care companies when she felt a patient was wrongly denied coverage. With all this, Barbara still finds time to be philanthropic, including helping to build homes for returning servicemen. She believes in mentoring and supporting women of all ages, encouraging them to be the best they can be. “There is power to be harnessed when women work together,” says Barbara, and that is why she is so passionate about helping women succeed. These enlarged, twisted veins near the skin’s surface, occur most frequently in the legs, but can also appear in the ankles. While not a serious condition, it is often an uncomfortable condition and may lead to more serious health issues. According to Mark A. Schwartz MD, FACS, RPVI Board Certified in Vascular Surgery and Medical Director of The North Shore Vein Center in Lake Success (www.longislandvein.com), “Over 80 million people have varicose veins. Females are affected approximately four to one over males. These conditions are often hereditary, but hormonal factors including puberty, pregnancy, menopause and the use of birth control pills can also play a role. The development of varicose veins may occur at any age but usually occurs between the ages of 18 and 35 years, and peaks between 50 and 60 years. As one ages, the cumulative effects of advancing age will lead to persistence or worsening of the condition.” In some cases, varicose veins may be completely nonsymptomatic and still appear. In certain cases, varicose veins can lead to more serious health issues. Cause One-way valves found in veins keep blood flowing from the legs towards the heart. Issues in the valves, causing them to not function and blood to collect in the legs, creates pressure build up resulting in varicose veins. Veins become frail, bulky and distorted. A primary reason behind varicose veins is because the legs endure the most pressure from the body to carry blood back to heart. Most commonly caused by genetics and aging, 50 to 55 percent of women and 40 to 45 percent of men in the United States suffer from some type of vein problem, according to the Department of Health and Human Services. The valves in veins weaken and do not work as well as they used to, due to aging. Being born with weak valves may also increases the risk of contracting varicose veins. Varicose veins are also symptomatic of pregnancy due to the increased amount of blood in the body, enlarging legs. A growing uterus also adds pressure on leg veins. Three months after delivery, varicose veins may improve, but may return with each pregnancy. 14 • August 2011 • LONG ISLAND WOMAN A job that entails standing for long periods of time, consequently adding pressure on leg veins, can cause varicose veins to occur. Also, being dramatically overweight can also be grounds. Symptoms Typically varicose veins have a dark blue or red color, are swollen and have a coiled appearance underneath the skin. Mild symptoms range from burning, tenderness or leg pain. After standing or sitting for long periods, symptoms may worsen. They commonly develop on thighs, the back of the calves or inside the leg. During pregnancy they can form around the vagina and buttocks. In certain cases, they are symptomatic of a deep vein thrombosis, a blockage in the deeper veins, which may need treatment. “Initially, varicose veins may only appear as bumps or lumpy protrusions under the skin. Eventually, these veins may become quite tender and cause To advertise: 516-505-0555 x1 or ads@liwomanonline.com (!*-/&/(*$%*) (%/'%!%/'!# North Shore Medical Spa ##/*&/ +#/ Wave goodbye to your former self! &(/(*!!/ -)!!%) !%)/)* *!!%) (( !(("(!!((!(( ($!(( (!(#(((( !((%"( ((( !(! #(( ( $( "&(( !((!((!(( "((!# (("" '( ! When patients develop swelling, skin changes or leg ulceration, Dr. Antonios P. Gasparis, MD, FACS and Director of Stony Brook Vein Center recommends they see a doctor. “The skin is an organ, prolonged swelling leads to organ damage over time and advanced disease presents with skin damage, pigmentation or ulceration,” according to Dr. Gasparis. Veins can also become swollen, red, tender and hot to the touch. Sores or a rash on the leg or ankles can develop. Side Affects In some cases, varicose veins may be completely non-symptomatic and still appear. In certain cases, varicose veins can lead to more serious health issues. Skin sores or ulcers may develop due to chronic blood back up. These sores are slow to heal and sometimes cannot ever fully heal, until the backward blood flow in the vein is repaired. “Left untreated these symptoms may progress to more severe problems such as chronic leg swelling, eczema-type symptoms, skin thickening and discoloration, as well as ulcerations,” warns Dr. Schwartz. “The early signs of pain, swelling and heaviness must be recognized so that the condition can be treated before irreversible skin changes occur.” Superficial thrombophlebitis, which is a blood clot formation in veins below the skin, is also common. Skin will become scarlet, firm but tender and feel hot around the vein. Deep vein thrombosis is a blood clot deep in the vein that can cause a pulling sensation in the calf, pain, warmth, swelling and discoloration. In cases where, deep vein thrombosis causes no symptoms, the blood clot can travel to the lungs and be fatal. While not all varicose veins can be prevented, there are steps that can reduce their occurrence. Diagnosis An examination by a doctor will allow proper diagnosis. Patients usually stand up and then sit with legs dangling in order for the doctor to spot the varicose veins, which are evident due to the physical symptoms of darker veins, dry thinning skin and inflammation of the area. In order to rule out other disorders, tests may be performed. Ultrasounds are used to see the structure of the vein, investigate blood flow and check for blood clots. Sound waves are used create images of the structures in the body. There are several types of doctors that can treat varicose veins. A phlebologist specializes in vein treatment, a vascular doctor focuses on the blood system, an interventional radiologist specializes in using imaging tools to see inside the body and can often treat issues with little or no surgery. Dermatologists are also capable of treating varicose veins. Many of these doctors perform some of the procedures used for treating varicose 16 • August 2011 • LONG ISLAND WOMAN veins. A good starting point is inquiring with your general physician regarding which specialist he or she recommends. Inquiring with your insurance provider about which type of doctor is covered under your policy may also be beneficial. Treatment Lifestyle changes, in addition to medical treatment can relieve symptoms, prevent complications and improve the appearance of varicose veins. A doctor is likely to propose lifestyle modifications if the varicose veins are not causing many symptoms. Should symptoms be more severe a doctor may suggest a variety of management options. Many treatments of varicose veins can be done in the home. For immediate treatment to decompress varicose veins, Dr. Eli Anker, a vascular surgeon with a practice in West Islip, recommends patients wear prescription surgical compression stockings. “They are best worn throughout the day. Obviously this treatment is only temporary since the veins return to their usual size and extent once the stockings are removed,” describes Dr. Anker. Many patients complain the stockings are difficult to put on and may be too hot to wear during the summer months. “In the past, patients have avoided treatment as they are most familiar with out-dated stripping procedures that were painful and came with a prolonged convalescence,” explains Dr. Schwartz. “Recent advances in vein treatment now offer patients an easier alternative. Procedures are now being performed in an office setting.” Sclerotherapy is the most common treatment for varicose veins. A liquid chemical is injected into the vein causing the walls to seal together and remain closed. This stops the blood flow, converting the vein into scar tissue. The vein fades in a few weeks, but may need treatment more than once. This in-office procedure may cause red or raised skin, bruises at the injection site and groups of fine lines around the treated area, which will also disappear in a few weeks. Radiofrequency and laser treatments treat deeper leg veins, called the saphenous veins, and can replace surgery for severe cases of varicose veins. This technique can also be done in a physician’s office and requires a catheter, a very small tube, to be placed into the vein. A probe travels through the tube that heats the inside of the vein to close it via radio frequency or a laser. Local anesthesia is required and slight bruising is common after treatment. Prevention While not all varicose veins can be prevented, there are steps that can reduce their occurrence. Apply sunscreen to protect the skin on the legs, exercise regularly to increase leg potency and circulation in veins, especially since this can help keep weight under control, as excess weight adds stress on leg veins. Wearing lowered heeled shoes tones calf muscles to keep blood flowing through the veins. While resting, keep legs in an elevated position as much as possible. ◆ To advertise: 516-505-0555 x1 or ads@liwomanonline.com Cancer Treatment: Specializing in Insulin Potentiation Therapy A Targeted Low Dose Chemotherapy IPT targets cancer cells using the powerful cell-killing effects of standard chemotherapy. And if you’re actress Meredith Baxter, you can fault stinking thinking for three failed marriages and a bout with alcoholism while enduring years of verbal and physical abuse and decades of self-abnegation. Yes, that Meredith Baxter: the perky, blonde, golden girl who embodied the shikshe goddess of Bridget Loves Bernie, the determined, divorced young mother of Family and, most memorably, the nurturing post-hippie mom of Family Ties. In her just-released autobiography, Untied, Baxter not only rips the cover-girl surface off those seemingly ideal years with her tales of spousal abuse and marital woe, but she also recounts her bout with breast cancer and decision, in 2009, to come out publicly as a lesbian. “I thought (the publisher’s) the initial request for the book had been about the whole coming-out process, and that was a little disturbing,” Baxter says in our late-winter phone conversation, “because I thought, `that does not warrant a book.’ It’s not all that interesting, and I’d like to think I’m more than that. Although, if you Googled me, you’d think that’s all I’ve ever done.” When I counter that sex sells books and that when a woman who was straight for decades suddenly declares she prefers ladies, prurient curiosity is likely to follow, Baxter laughs, “You, too? But seriously, “As it happens, I like men much better now that the whole sexual component is out of the way.” there were no conditions on the book. I simply had to write what I knew. If I had to write this book 20 years ago, or even 10, I wouldn’t have known what to write because I really hadn’t learned anything. I’d never done any self-examination. I was just trying to keep my head above water. It wasn’t until I decided, “Okay, let me take a look at what I’ve learned about myself in my (then) 62 years. It turns out, everything I’ve learned I’ve learned in therapy and in a 12-step program.” The main thing Baxter says she’s gleaned from digging deep is taking responsibility for her role in her own history. Whatever the circumstances of your victim-hood, throwing yourself a pity party doesn’t cut it. “When I got out of my third marriage [to late actor and screenwriter Michael Blodgett],” Baxter recalls, “I had a kind of breakdown. And it was not because of the loss of the relationship because I knew that had been a disaster. All I could think was, `How did I find myself in the same place yet again?’ I remember howling in my living room in despair because I had created such disaster. It was horrible. I don’t mean the marriage was all horrible, because he wasn’t a bad guy, and sometimes there was a lot of fun to be had. But there was dissociative behavior on his part, and there was a terrible price to pay at the end because of stuff that went on. So it was the first time since I started my 12-step program that I realized I had no answer for my next step. I was broken. I had hit bottom. And that’s when I started asking for help.” Continues Baxter, “I was guided to a therapist I’ve been with all this time who has been remarkable for me. I redoubled my efforts in the program and started doing the work I had never done before. Well, some of that was taking a look at what my part was in things. If you’re a victim, you don’t have a part; it all happens to you. To illustrate my thinking, I was at a meeting, and I heard a woman talk about how we’re trained as children to go to the parental well for food, nurturing, sustenance, love – but some parents’ wells are empty. They’re not empty at us; they’re just empty. “I was blown away,” Baxter says of this revelation about her unhappy childhood as the daughter of sitcom creator Whitney Blake and radio announcer Tom Baxter. “I had always thought it was empty at me, that I had been a target. I had lain all my troubles at my mother’s feet, so I had to go back and say, `Who was my mother? What did she want out of her life that she didn’t get, that put her in a situation where she had to look only to take care of herself and maybe not take care of me so much?’ And I came to understand her. But I couldn’t forgive her yet. So I had to look further and say, `What about me? How did I handle my children?’ I looked at all the times when I wasn’t able to show up for them. When I was afraid and didn’t protect them because I was taking care of myself instead. Was it shabby? Sometimes. Was I doing the best I could? I had to allow that yes, I was. And if I could allow that for myself, I had to give that to my mother, which totally changed the way I saw her.” Asked if she was beaten or molested as a child, Baxter allows that she had a stepfather “who would switch us, but I would not call him an abuser. He was a very rough guy, and he did sexualize me very early, just in the way he treated me, but no, I was not abused. My childhood wasn’t bad; it’s just that stuff happened that was very painful. I lived my life as if it had been a tragedy. Kids who had grown up in foster homes with lots of brutality and cigarettes put out on them – somehow, because they didn’t have the same kind of self-perpetuated thinking, came out and went, `Boy, I grew up with some motherf**kers. But I’m okay!’ Me, I imploded. I didn’t want to let an opportunity for pain and despair pass by.” And so, her first youthful marriage crash landed, followed by marriage number two, for 15 years, to Baxter’s Bridget Loves Bernie co-star, David Birney. And that’s where things get ugly. “I never called David any names in my book,” Baxter says, but she stands by her public statements that he was both verbally and physically abusive. “Yes, he hit me before we were even married, and yes, he’d been rough with the kids. But I pretended not to see that. I pretended not to call it what it was. But this is not about him; I’m not trying to demonize him. It’s about the narration I had in my head that allowed me to go there. And I had to continue this narration as more violence happened. And there wasn’t a slew of violence. You don’t have to get hit every day to know that’s in the deal; it can come out at any time. It was the daily derision, the mockery, the put-downs. That’s the stuff that’s so corrosive. “As for why I didn’t just leave,” continues Baxter, “talk with women who have been in abusive relationships. Your sense of choice is just not there. There’s a story I cited in my book that I remembered so profoundly: Hedda Nussbaum and Joel Steinberg. Remember, the couple in New York about 20 years ago? He abused her for four years and then killed their six-year-old daughter. I’m not saying my life was on a par with them. However, everyone was saying to (Nussbaum), who was a pariah and blamed almost as much as he was, `Why did ya stay?’ WHY DID YA STAY?’ And I remember her sobbing and saying, `I didn’t know I could leave.’ I sat there sobbing watching this, `cause I felt the same way. I had a car, I had a job, I went out in the world, but still, I didn’t know I could leave. That choice never occurred to me.” (On his official website, Birney contests Baxter’s portrayal of events in “Untied.” While not denying any specific allegations, the actor accuses his ex-wife of constructing a mean-spirited “fairy tale,” one which hurls accusations that tend to crop up suspiciously often in bitter custody battles.) Asked how she finally got the gumption to exit the marriage, Baxter recalls, “We had a Thanksgiving dinner that was no different from other Thanksgivings. I assume people were at the house; I don’t remember who was there. And it was after the dinner, and I was cleaning up. And David came and sat down and said, `Well, what are we gonna do next year so it’s not a disaster like this year?’ And I remember thinking, `I don’t know. What did I do this year?’ And I’m just standing there crying over the stove. And my daughter, Eva, who was around 18 or 20 back then, she just leaned into me and said, `What are you waiting for?’” “I didn’t feel some surge of activism,” notes Baxter. “I just had no defense against what she had told me. So I said to David, `There’s not gonna be a next time.’ I didn’t know where those words came from. He just kind of laughed at me, and the mockery started: `Oh, the little darling, are you feeling bad?’ You know, that kind of shit. But it started a conversation where somewhere I said, `I’m not happy.’” For all the obvious acrimony, Baxter at least attempts to shift the focus back to her own journey. “Yes, I had anger about what transpired. David is not someone whose process I respect because he treats people unkindly. However, I knew who he was before I went into that marriage, so how can I blame him? The main idea is that I cannot see myself as a victim. That’s a toxic position to be in, which is why I would go back and do it again.” Alcohol was a helpful crutch, but even getting clean wasn’t the answer to Baxter’s deeper problems. “I’ve been sober for 21 years, but I didn’t do any of the work the first ten years, because I really didn’t think I was an alcoholic. I thought everybody else had a problem. I thought, `I’ll stop drinking because it seems to be bothering people.’ My life got better, and my relationships with the kids were better, but it was still my thinking that was the problem. And my thinking was, `find something outside myself to solve my problems. And if it wasn’t gonna be alcohol, then it would be…a guy! I’ll marry this guy, `cause he seems to want me.’ And I’m thinking, `If you just love me, I’ll pretend that I don’t see that you lie, that you’re unfaithful’ – when we met, he was having an affair on his wife with me! So I knew this was a guy not to trust. That was my third husband.” When Baxter finally did the heavy lifting of therapy, Meredith Baxter continued she not only realized the vicious cycle she was on but learned something revelatory about her sexuality. “Look, I was attracted to men,” she explains. “I mean, you listen to Ricky Martin interviews – he had very happy and successful relationships with women. It doesn’t mean that you can’t; it’s just not where you want to be. As it happens, I like men much better now that the whole sexual component is out of the way.” And as for her partner, building contractor Nancy Locke, the actress says, “We’ve been together five years, though I’ve known her longer than that. I basically tortured her at first,” she laughs. “Because I was so distrustful of the way I thought, and I wanted to make sure I wasn’t lying to myself again. I was attracted to her, and I enjoyed being with her, but I didn’t trust me.” Though not overtly political, Baxter does believe she’s making a statement simply by being open about her homosexuality. “I’m not out there going, `GO GAY!’ Just because it’s right for me does not mean it’s right for anybody else. But if you’ve known me all these years, and you liked me all these years, then the fact that I’m gay doesn’t change anything.” Also on a happy note, Baxter, cancer-free since the late 1990s, has been a vegetarian for nearly 30 years. “I used to be a big-time runner, but because of back problems, I can’t do that anymore. Still, Nancy goes to the gym absolutely every day. I go with her probably three, four times a week. When does it happen that a girl goes from being a lithe young thing to a little-bit-older woman who has to hold her stomach in? I never appreciated how thin I was!” Baxter laughs. “I’m a healthy girl, though vegetarians can still eat a ton of ice cream, dontcha know!” ❖ David Lefkowitz co-publishes Performing Arts Insider (TotalTheater.com) and hosts Dave’s Gone By (davesgoneby.org) on UNC Radio, where he serves as programming director. There was a lovely family I knew out there on Jones Beach 25-30 years ago. They were friends of David’s, and we went sailing with them. Growing up in Los Angeles, I’d missed so much because I noticed these kids understood, from a very young age, the tides, the tables, the dangers of the water, and when it was good to take the boats out. It was very foreign to me and wonderfully attractive. We had a great time out there. The dream role you never got to play? Juliet. It’s such a winsome part, and I love the language. But then it was, `Well, I’ve wayyyy outgrown Juliet.’ Maybe I could do it at someplace like the Muny opera house in Kansas City that seats 10,000 people. I’d be small enough that they wouldn’t be able to tell how old I was. Your thoughts on celebrities being “outed”? I personally don’t believe in it. It’s none of anyone’s business. This country was based on freedom of religion, so why can’t it be the same for your sexuality? We have Methodists and Hindus and Baptists and Jews and Protestants, and we can all go to any church we want and think, `What dogma speaks to me? Oh, I like their music. Or, that priest? Ehh.. a little judgmental. I think I’ll try the Baptists. Oh, too much hellfire and brimstone – that doesn’t speak to me. Or the Buddhists. That’s very gentle; I like that.’ Where I am now speaks to me. I feel a comfort in the level of connection here. Last spring a dear friend told me, before she sent her teenaged daughter off to college, that she was going to treat her to a “real haircut.” That meant one of those $100 minimum Long Island salons. The same salon where “Mom” goes. Daughter came home looking great. Fast forward a few weeks. Mom and Daughter were in the car and Mom was laughing about that day’s “Ask Amy” newspaper advice column (the replacement for Ann Landers.) Mom said, “Can you believe the trouble this girl had when her Mom sent her daughter to her salon? Mom quoted the “Ask Amy” letter. “I am a 17-year-old girl who has gotten herself into a very awkward situation … (my Mom sent me to her stylist) … The stylist was extremely flirty. At the end of my appointment he asked me for my phone number. He’s 24. Mistake No 1: I loved the attention, so I wrote my phone number on his tip. When he actually called that night and asked me to meet him at a party in the city, I was totally taken aback and I told him I was busy. He asked me to call him this weekend to set something up. I’m freaking out because I have absolutely no desire to go out with a man more than six years older who seems to think it’s fine for a high school girl to meet up with him on a Thursday night in the city! I can’t tell my mom because he does her hair!” Mom was hysterical. The laughing kind of hysterical. Daughter was hysterical. The hysterical kind of hysterical. Seems young Daughter had also been feeling kind of grown up and flirty the day she went to her Mom’s salon. So when the stylist hit on her, well, she wrote her phone number on his tip. He called. Big party in the city. Freak out. You know the rest of the story. Daughter was so petrified she accessed the best advice around – a bunch of 17-year old girlfriends. Their experienced solution: write to “Ask Amy.” It’ll never get published anyway. Or will it? Today Daughter has a year of college under her belt. She’s learned a lot about the world – including that Mom is a great first stop for advice in crisis. Oh, and Daughter’s hair – she’s growing it out these days. Really. ◆ Daughter was so petrified she accessed the best advice around – a bunch of 17-year old girlfriends. Jerusalem Maiden by Talia Carner Life for a twelve-year-old girl at the beginning of the 20th century was difficult enough, but for young women who were members of an ultra-Orthodox sect, there were few choices and little to look forward to. As soon as she reached puberty, a “Jerusalem maiden” was expected to marry a man she had never met and begin a life of hard work, repressing rituals, frequent childbirth, and living as a second-class citizen. Esther Kaminsky dreads the day when her family marries her off because she is an artist who wants nothing more than to practice her craft. Creating art, however, is strictly forbidden by her religious sect, and although she is able to study in secret with a teacher who recognizes her talent, she knows that once she is married, those sublime hours will end. With Jerusalem Maiden, Talia Carner has written an exquisite tale that recreates a city and its people and beckons the reader to follow Esther through her adolescence in Jerusalem, married life in Jaffa, and a visit to Paris that transforms her life. Guest Reviewer: Helen Stefanidis, Marketing Executive The Warmth of Other Suns by Isabel Wilkerson This powerful book tells the epic story of a migration few people have heard of, but most have lived through. Between 1915 and 1970, six million African Americans migrated from the Jim Crow South to northern and western cities to change their lives—and eventually, the fabric of America. Wilkerson weaves the stories of 1,200 people she interviewed along with the three main characters of her story: a Mississippi sharecropper, a Florida laborer, and a Louisiana doctor. It’s a sweeping story, at times heartwrenching and cruel, at times uplifting and triumphant. The Great Migration hits close to home for all those who left their families, friends, language, and familiar landmarks behind to escape a society of cruelty, inhumanity and fear. As Wilkerson writes, “The actions of the people in this book were both universal and distinctly American. Their migration was a response to an economic and social structure not of their making ...They did what human beings looking for freedom, throughout history, have often done. They left.” To advertise: 516-505-0555 x1 or ads@liwomanonline.com Steven G.
