MSHSL Eligibility Statement Health Questionnaire Emergency Contact Information Co-Curricular Transportation Waiver Sports Physical Exam – valid for 3 years Participation Fee: $250 Damage Deposit: $100 May not practice without red registration card Impact Testing - Nate Tellers / OSR Transfers – see the AD Participation Fees $250 Registration online at MWHS website Refunds – 100% refund for the first 7 days of participation or until tryouts are complete 50% refund from 2nd day of 2nd week until the 1st competition; Use or consume, have in possession tobacco; or, Use or consume, have in possession, buy, sell or give away any other controlled substance or drug paraphernalia Chemical Eligibility Suspensions 2 weeks/2 contests – whichever is greater 3 weeks/6 contests – whichever is greater 4 weeks/12 contests – whichever is greater Academic Eligibility New Quarter requirements – if a student fails 1 class in a quarter, he/she is ineligible for contests for two weeks or two contests, whichever is greater, following the official posting of grades.
