Brywood A National Blue Ribbon Elementary School Dear Parents, It is time to start making plans for the 2015-2016 school year. We’ve had a terrific year here at Brywood Elementary, and we truly look forward to working with your children again next year. To help us determine our staffing needs for the upcoming year, we need to project our upcoming student enrollment. If you have a Kindergarten-5th grade student(s), please fill out and submit the survey below as soon as possible (no later than February 20, 2015). It is very important that you complete one of these surveys for each of your children who currently attend Brywood Elementary. Do not complete this survey for your 6th grade child who will be moving on to middle school next year. In order to project reliable enrollment information, IUSD is also requiring an updated Residency Verification Form and Affidavit for each student - both forms are available to print online. If you have more than one student at the same school, you can list all your students on one completed Affidavit. Please return the completed IUSD Residency Verification Form and Affidavit along with two current proofs of residency to our school's front office by February 20, 2015. Any new resident who is new to the District as of January 1, 2015 does not need to submit new residency verification forms as you have just completed this process. th th Residency verification and this form are required for all Kinder through 5 Grade students. If your student is in 6 Grade Residency verification is the only document required.
