Mark Bush of Dick White Referrals was recently
presented with a case involving multiple metacarpal
bone fracture in a 1.7kg Chihuahua.
Management of fractures in dogs such as the Papillon
and Chihuahua weighing less than a kilo is very
challenging. Even 1.5mm and 2.0mm screws are too
large. Metacarpals in even some medium size patients
require very small implants.
To deal with fractures in tiny bones we have introduced
our exclusive range of 1.0mm implants and
equipment.
The range includes T-plates, compression plates
and 5 different cuttable strips. Due to the small size,
dedicated instrumentation is required.

1.0mm Plates gives flexibility for new management
options for a number of different fracture scenarios.
All of our Cuttable Plates have round holes. The
range is suited to fractures of the small bones of the
pes and manus, such as the metacarpal bones, as
well as other fine bones such as the ulna and fibula.

long bone fractures on very small animals as well as

Standard DCP or Round Hole Plates are held into
position by being trapped between the head of the
screw and the bone. Tightening the screw pulls the
bone up to the plate. When the screw is tight the
threads of the screw pull against the bone holding the
plate in position.
The situation with locking screws and plates is very
different.
Although the threads of the screw shaft engage the
bone, the interface between bone and screw is not
related to attachment of the plate. In locking plates
and screws it is the interface between the screw head
and the plate which attaches the plate to the screw.
As the screw is tightened the bone maintains its
position relative to the plate, it is not drawn up to it.
The screw head engages with the plate.

TTA Rapid NEW
Technique - No Maquet
Hole
The TTA Rapid techniques create an incomplete
osteotomy, freeing up the tibial tuberosity, but
leaving sufficient cranial cortex and tibial shaft to
be structurally sound. Levering the tibial tuberosity
cranially and then inserting and securing a cage into
the osteotomy gap to maintain the advancement. An
incomplete osteotomy reduces stability less than the
complete osteotomy seen in the original TTA technique
and is subjectively more comfortable for patients.
There are now two recognised TTA Rapid techniques.
The original involves drilling of a ‘Maquet’ hole at
the distal extent of the osteotomy. This provides a
fixed location point for the guides used when making
the osteotomy and in theory helps force distribution
around the circumference of the hole making cracking
around this point less likely. The technique of drilling
a hole at the end of a ‘crack’ has been common and
accepted practice in various engineering applications
for decades.

A body of surgeons believes the Maquet hole
is directly responsible for tibial shaft fractures,
especially where it is over-sized in proportion to the
tibial shaft diameter, so a second technique has
evolved that doesn’t use a Maquet hole at all. Avoiding
the use of a Maquet hole leaves more bone stock
behind, which may leave it stronger.

C-Lox (Patent Pending)
Developed over several years by Rita Leibinger Medical in cooperation with Professor Dr. Franck Forterre, Bern
University (Switzerland), C-Lox is gaining quite a following amongst European spinal surgeons for intervertebral
distraction fusion.
Manufactured in pure titanium using the same advanced selective laser
sintering (SLS) 3D printing technology as TTA Rapid cages, a trabecular
structure approaching the same Young’s modulus as cancellous bone
is created. In theory this reduces the risk of subsidence commonly seen
with many of the more basic metallic implants. The trabecular structure
also encourages early vascularisation and bone through-growth.
C-Lox cages feature integrated spikes that stand proud of the bearing
surfaces and engage with adjacent vertebral end-plates. Additional
locking screw fixation provides for a secure primary fixation.
Technique Summary
1. Preliminary cage size is selected following imaging
2. From a ventral approach, the affected intervertebral disc space is
identified and exposed
3. Drill adjacent vertebral bodies using a 2.5mm drill-bit and distract the
intervertebral disc space using the C-Lox Distractor
4. Perform laminectomy
5. Use the C-Lox Spinal Disk Broaching Curette to remove all remnants
of the nucleus pulposus and to create a bed for a C-Lox cage
6. Use trial cage (template, no spikes) to confirm cage selection
7. Insert final cage
8. Mark start point for screws using dedicated guide and drill-bit
9. Insert special C-Lox self-drilling, self-tapping titanium locking screws
A multi-centre European C-Lox study is currently in progress headed by Dr Franck Forterre and Günter Schwarz.
With early results showing great promise, the decision has been made to make C-Lox available to experienced spinal
surgeons outside of the trial group and not to deny a greater patient population access to C-Lox surgery.
The full C-Lox kit contains all the C-Lox instrumentation and drill bits, 1 of each trial cage, 1 of each implantable cage
and 5 screws of each screw length. The full kit is available for purchase, but UK customers may like to start by hiring

Advancements of just 3mm are required for most
cats and many smaller dogs.
For these smaller patients, the TTA Rapid Petite
system has been developed.
The system features dedicated instrumentation and
a dedicated selection of both cages and screws
which the surgeon will find are better tailored to
those smaller patients.
FLEXIPATH Trocars are sterile single-patientuse instruments that consist of a mechanical
obturator and a flexible sleeve.
When activated, the mechanical obturator
expands the flexible sleeve, which becomes
secured to the mechanical obturator for
placement. The flexible sleeve incorporates
holes (for sutures or skin staples) to secure it
to the skin.
