School Supply Order Form 2015-2016

DISTRICT 102 SCHOOL SUPPLIES 2015-2016 SCHOOL YEAR
*Please note: This is a separate purchase from school registration!*
There are 2 easy ways to order school supplies for the upcoming school year.
1. Use the order form on the back of this page to PAY BY CHECK. You may use one order form
and write only one check for all of your children!
2. ORDER ONLINE using an easy secure website that is sponsored by D102 PTO
Go to: http://district102pto.revtrak.net
*Choose your child’s 2015-2016 school on the left side of screen under “Browse”
*Choose the PTO supply pack that corresponds to your child’s 2015-2016 grade
*Enter your child’s last name, first name and click “Buy Now”
*If your order is complete, proceed to Checkout OR if you would like to add another pack
for another child, click Continue Shopping
*Once all orders are complete and you proceed to checkout, you will need to select if you
are a new or returning customer
*New customers will need to enter an email address and password and create a profile.
*Returning customers just re-enter email and password
*Enter credit card information (both new and returning customers will need to enter
credit card information which is NOT stored)
*Verify info for final opportunity to change your order
*Once verified, you must enter COMPLETE ORDER
*Print copy of receipt for your records and you will receive an email confirmation as well
NOTE: School supply kits will be delivered directly to your child’s classroom for Pritchett, Tripp and
Meridian students. Aptakisic students will have a pick up date just before school starts (exact date will
be communicated to you over the summer).
All proceeds will benefit the District 102 Teacher Appreciation Fund.
