2015 Commitment to Wellness – $75 Gift Card Incentive
The IACT Medical Trust Board of Trustees approved the 2015 Commitment to Wellness
program as recommended by your IACT Medical Trust Wellness Committee. We are launching
the VISA gift card wellness incentive for the fifth year. This program is designed for municipal
members to promote wellness and disease management and for the participant employees to
recognize the importance of maintaining a healthy lifestyle. It is my hope every Trust municipal
member will participate in 2015. Whether you have 3 employees or 600+ employees, the group
effort and goal is to focus on wellness and reducing medical claims.
The IACT Medical Trust member municipalities are asked to support the following program and
encourage participation with completion by December 31, 2105.
Eligible participants include employees, dependent spouses and dependent
children ages 18-27. The dependents are required to participate in the city/town
sponsored wellness event to qualify for the gift card.
Role of City/Town Administration
a. The Trust asks for member City/Town Administration’s and/or Council’s support
of the “Commitment to Wellness” incentive by promoting and endorsing wellness
and disease management programs.
b. The Trust asks the member city/town to plan an employer sponsored wellness
event for eligible participants to qualify for a $75 gift card from the Trust.
c. The Trust asks all members to distribute the quarterly wellness communication
and newsletter to participant employees. The wellness newsletters are provided
by the Henriott Group to Trust Administrators.
Eligible Participant Requirements (to receive a $75 gift card)
d. The eligible participant must complete UnitedHealthcare’s online health risk
assessment through their myuhc.com account and the new Rally wellness portal.
e. The eligible participant must have a medical preventative exam (an annual
physical) with the defined age appropriate testing. This does not include a
dental or vision wellness exam.

The eligible participant must take part in at least one employer sponsored
wellness event. This could include a weight loss program, an employee walking
program, an annual health fair, a bio-metric screening, or a lunch and learn
series, etc. as determined by city/town leadership.
Eligible participants who complete d, e and f (above) by December 31, 2015 and submit the
completed verification form no later than Friday, January 8, 2016 will receive a $75 gift card
from the IACT Medical Trust.
The gift cards will be mailed to the member city or town’s Human Resources Director or Fiscal
Officer for distribution to the eligible participants. The Trust’s auditors require proof of
distribution by obtaining the employee’s signature acknowledging receipt of the gift card. The
signature sheet needs to be submitted to the IACT Medical Trust once the cards have been
distributed and confirmation signatures obtained. The gift cards will be issued for verification
forms submitted beginning October 1, 2015 through January 8, 2016. Please note that gift cards
will not be issued for verification forms submitted after January 8, 2016.
Supporting Information and Program Documents
1. The Employee Wellness Program Handout provides an explanation of the program
and can be distributed with the verification form to the employees or posted in city/town
hall as notification and encouragement to participate.
2. The Wellness Incentive Verification Form is required to be completed by the
eligible participant, verified by the employer and submitted to the Trust.
3. UnitedHealthcare informational handouts make it simple for employees to create
a myuhc.com account and complete the health risk assessment.

• After logging into myuhc.com, members should click on the Rally Health Survey
tab to create their new Rally wellness portal account.
Upon completion the verification forms are to be submitted by the Human Resource Director or
Fiscal Officer to the IACT Medical Trust via email, fax or mail. The Trust needs the signed
verification form only and asks the Trust members to keep the supporting documentation
proving the completion of the health risk assessment, physical and employer sponsored wellness
event.
