CHECKLIST FOR COMPLETING THE I-‐20/DS-‐2019 REQUEST PACKET ADMITTED INTERNATIONAL UNDERGRADUATE STUDENTS o Complete all applicable sections of the Student Information Sheet. Choose how you want to receive your I-‐20/DS-‐2019. Attach a copy of your passport identity/biographical page. o Provide dependent information, if applicable, and attach a copy of your spouse/children’s passport. o Complete the Certification of Finances page. o If you are attending Summer Academy: International before Fall 2015, you must add Summer Academy expenses to the total amount required to issue an I-‐20/DS-‐2019 for your program. o If you have an individual sponsor, check that they have authorized the amount and provided their signature and an official letter. o If you are receiving an award, scholarship, or assistantship, attach a copy of the award letter. o If you are already in F-‐1 or J-‐1 status in the U.S. and will transfer to UCSC from another U.S. school or university, complete the Transfer-‐In Verification Form. Make sure to inform the DSO/ARO at your current institution that you have been accepted to UCSC and to request the DSO/ARO release your SEVIS record to the University of California, Santa Cruz (F-‐1 Program: SFR214F00416000 / J-‐1 Program: P-‐1-‐03968). Before the deadline listed on the Student Information Sheet, upload all required documents for the I-20/DS-2019 Request Packet to the Admissions web tool (http://admissions.sa.ucsc.edu/coe/). You can only access the tool once you have accepted your offer of admission. All documents submitted must be legible. If a document is not legible you will be asked to submit it again. The earlier that this step is completed, the sooner you can receive an I-20 or DS-2019. ISSS will not be able to process your I-‐20/DS-‐2019 request until all of the required documentation has been received. It will then take approximately 10-‐15 business days to issue your I-‐20 or DS-‐2019. Please factor in time for mail delivery of your I-‐20 or DS-‐2019 to your overseas or local U.S. address before you make your visa appointment.
