THURS APRIL 23RD – SUN APRIL 26TH 2015 -‐ SENIOR WIND ENSEMBLE (GR 10-‐12) -‐ EXTRA-‐CURRICULAR CHOIR (9-‐12) HOLY CROSS REGIONAL SECONDARY SCHOOL TRIP CONSENT FORM As you are no doubt aware, hundreds of students from the Holy Cross Bands & choirs have participated in previous Whistler and Sunpeaks festivals. We are planning on taking the grade 10 concert band, grade 11/12 concert band, and concert choir this year to Nelson with a similar format to the other two festival locations. The students will be traveling by bus to Nelson, leaving from Holy Cross in the early morning on Thurs Apr 23rd – Sun Apr 26th. While in Nelson we will be staying at the Prestige Hotel. *Pending space availability which the travel company is currently looking into. The main purpose of the trip is for the students to perform at the Nelson Music Festival and participate in its associated activities including workshops and performances. A festival concert with professional musicians and featured groups will also be part of the festival. Optional activities will be offered closer to the time of the festival. Other fees may be required, and additional consent forms will be required. They will be provided once the optional activities have been decided upon. Although students will be provided with lists of suggested and required items to bring on the trip, it will be each individual student’s responsibility to bring whatever he or she needs on the trip. While in Nelson, the students will be supervised by at least 7 adult chaperones including teachers and parents. If you are interested in being a chaperone please let Mr Cabralda know. There is some cost involved with being a chaperone. Supervision will consist of adult presence in the hotel any time that any students are there. Furthermore, there will be supervision of the group during all transfers, and adult presence in any of the locations where students are taken for activities. For example, although adult supervisors will be present when the students are shopping in the village, the students will be free to stay in groups and enjoy the sights and shops without an adult being with them at all times. It is to be understood that the school’s expectations regarding student behaviour will apply during this field trip, and that the parent / guardian signing below assumes the responsibility for any costs caused by the student’s failure to abide by these expectations, including sending the student home. *A copy of the expectations and rules of conduct will be reviewed and distributed closer to the date of departure.* Accidents can be the result of the nature of any activity and can occur with or without any fault on either the part of the student, or the school board or its employees or agents, or the facility where the activity is taking place. By allowing your son/daughter to participate in this activity, you are accepting the risk of an accident occurring, and agree that this activity, as described above, is suitable for your child. The school does not provide any accidental death, disability, dismemberment, or medical expenses insurance on behalf of students. DRAFT ITINERARY Thursday April 23 SURREY | NELSON
