Insurance benefits will terminate June 30, 2015 for anyone
who does not complete open enrollment. No Exceptions!
Open enrollment is your annual opportunity to review your insurance
benefits for the new school year. The only other time you can make
changes to your benefits is when you have a Qualifying Event. You have
30 days from the date of your qualifying event to contact ext. 7149.
To complete the online open enrollment process you must log in to Ivisions
and click on Employee Resources/Benefits/Benefits Enrollment.
Open Enrollment Meetings
Meetings are scheduled at every school, Food Service, Student Support
Center, and the District Office. Employees are welcome to attend any
meeting that fits their schedule. If you need assistance with the online
open enrollment process or need access to a computer, three additional
meetings are scheduled at the District office. The meeting schedule is
attached to this newsletter.
Resignations and Retirement
Employees resigning or retiring should not complete open enrollment. If
you indicated on your Letter of Intent that you would not be returning for
2015-16, your medical, dental and vision insurance will terminate June 30,
2015. All other benefits terminate on the last day worked.
For those who did not indicate on their Letter of Intent they would be
resigning or retiring will have their benefits terminate the last day of the
month their work calendar ends. All other benefits terminate on the last
day worked.
Plan Information
All plan documents, premiums, carrier websites and contact information is
posted on the Employee Benefits website. To access the website click on
the Benefits tab located on the District’s home page under the Human
Resources heading.
Medical
Insurance
Medical Insurance
United Healthcare provides the District’s medical insurance. The District
will continue to offer a health savings account with a high deductible
medical plan and a traditional PPO plan.
HSA2600 Health Savings Account w/High Deductible Health Plan
The HSA2500 Medical Plan will change to the HSA2600 Medical Plan.
This is a federally mandated change. The HSA2600 Medical Plan has a
$2,600 calendar year deductible for employee only coverage. The District
pays the employee’s premium for this plan. Dependent coverage is
available at the employee’s expense.
The District will contribute $500 to the employee’s health savings account
on or about July 1, 2015. Employees can earn an additional deposit of
$500 if certain requirements are met. The requirements have been slightly
modified from the current year. Please visit the Wellness webpage or the
Employee Benefits website for more information.
Preventive visits are covered at 100%. Prescriptions listed on the
Expanded Drug List are eligible for a copayment without having to meet the
deductible first. All other prescriptions are not eligible for a copayment until
the deductible is met. The copayments are $10 for Tier 1, $25 for Tier 2
and $50 for Tier 3.
Employees are not eligible for this plan if they are covered on a traditional
medical plan outside of the District. For more information about this plan
please review the Summary of Benefits and Coverage posted on the
Employee Benefits website.
Choice Plus 104 Medical Plan
The employee’s cost for the Choice Plus 104 Medical Plan is $60 per
month. Dependent coverage is available at the employee’s expense.
There are no changes to this plan’s coverage. For more information about
this plan please review the Summary of Benefits and Coverage posted on
the Employee Benefits website.
Both the HSA2600 and Choice Plus 104 Medical Plans offer zero-cost
contraceptives. This list is posted on the Employee Benefits website.
Vision
Insurance
Vision Insurance
United Healthcare provides the District’s Vision insurance. The District
pays the employee’s premium for the Vision plan. Dependent coverage is
available at the employee’s expense.
The copayment for an eye exam is $15, material (frames) copayment is
$30 and there is no copayment for standard single vision lenses or lined
bifocal and trifocal lenses.
Exams, frames and lenses are available every 12 months. Contact lenses
are available in lieu of glasses.
ID cards are not provided for the vision benefit. A temporary ID card can
be printed on the vision website, www.uhcvision.com. The Vision Plan
number is C0470.
Dental
Insurance
Dental Insurance
Delta Dental of AZ and Total Dental Administrators provide the District’s
dental insurance.
Delta Dental of AZ – the District pays the employee’s premium for the
Delta Dental of AZ Base Plan. The Delta Dental of AZ Base Plan provides
a $1,500 calendar year benefit for each covered person. The Buy-up Plan
provides a $2,000 calendar year benefit for each covered person and
$1,000 lifetime benefit for orthodontics for covered dependents under the
age of 18.
Both plans provide:

Routine cleanings every 6 months

$50 Deductible for single coverage and $150 deductible for Family

Same network of providers
Dependent coverage is available on both plans at the employee’s expense.
Dental cards will not be mailed unless you are new to the plan.
Total Dental Administrators - the District pays the premium for
employees and their dependents for this plan.
The A500S Plan has a Schedule of Benefits that lists the procedures
covered and the employee’s cost for each procedure. If a procedure is not
listed on the Schedule of Benefits, it is not covered. Employees must
choose a dentist from within the plan’s network.
The plan provides:
 Routine cleanings every 6 months
 No annual deductible
 No calendar year maximum
 There are no out-of-network benefits
Life
Insurance
Life Insurance
Voya Financial (formerly called ING) provides the District’s group life
insurance. The District provides a life insurance benefit equal to two (2)
times the employee’s base pay, at no cost to the employee.
Optional Life Insurance
Employees can elect optional life insurance coverage for their family.
Employees enrolling for the first time or requesting an increase must
complete the EOI form (Evidence of Insurability). If enrolling your spouse
or increasing your spouse’s coverage you must complete an EOI for your
spouse as well.
Completed EOI forms must be submitted to Human Resources by May 15,
2015.
The EOI form can be printed from the top of the benefit screen in Ivisions.
Disability
Insurance
Voluntary Short-term Disability Insurance
Assurant/Union Security Insurance Company provides the District’s
voluntary short-term disability insurance. Approved claims are payable on
the 8th day of an absence and are payable for up to 3 months. This
coverage pays in addition to sick leave.
Employees enrolling for the first time or requesting a benefit amount higher
than one level must complete the Health Questionnaire.
Completed Health Questionnaires must be submitted to Human Resources
by May 15, 2015.
The Health Questionnaire can be printed from the top of the benefit screen
in Ivisions.
FSA
Plans
Flexible Spending Accounts
BASIC provides the District’s flexible spending accounts. The maximum
election for the medical spending account is $2500, the maximum election
for the Limited spending account is $500 and the maximum election for the
dependent care spending account is $5000.
Debit cards for employees continuing their enrollment will be front-loaded
July 1, 2015. Debit cards for employees enrolling for the first time should
be received on or about July 1, 2015.
ARAG Legal Plan
Legal Plan
The ARAG Group provides the District’s legal plan benefit. Covered
services include divorce, bankruptcy, will preparation, real estate
transactions and minor traffic offenses.
Other . . .
Liberty Mutual Home, Auto & Renters Insurance
Liberty Mutual provides personal insurance coverage for your home and
auto needs. Employees can elect to pay their premiums with payroll
deductions.
Optional Retirement Savings Plans
Employees can initiate contributions to a tax sheltered annuity or deferred
compensation plan any time during the school year.
Employees must choose a company from the Approved Vendors List then
contact the company to initiate an account. Once the account is initiated
the employee can submit paperwork to Human Resources to start their
payroll deduction.
The District does not match contributions made to these plans. For more
information employees should contact a vendor of their choice.
Ivisions
When you complete the open enrollment process in Ivisions please read
each screen carefully. Your benefits are an important part of your
employment and the District wants to ensure you select the right coverage
for you and your dependents.
The meeting schedule and rate sheet for 2015-16 are attached to the
newsletter.
