Hanover County Public Schools Open Enrollment for 2015 October 27, 2014 – November 14, 2014 Effective Date of Coverage Changes: January 1, 2015 Please read for important information about enrolling or making changes to your health, dental, supplemental plans, and flexible spending accounts benefits. All disclosure notices, forms, rates, and informational booklets will be available on the website by October 27, 2014. Open enrollment is a time during which benefit-eligible employees may make changes in coverage or enroll in certain benefits. If you choose not to enroll or make changes during open enrollment, you must wait until the next open enrollment unless you have qualifying event that allows you to make changes within 30 days of an event. Open enrollment occurs only once per plan year. You are encouraged to attend an open enrollment meeting to obtain more information. You will have the opportunity to ask questions directly to the benefit vendors. This is the only meeting where a US Bank representative will be present. Employees may contact US Bank if they have questions. Important Reminders: Employees who wish to enroll or make changes to their healthcare, dental and/or AFLAC supplemental insurance plans must complete an enrollment/change form. NO action is needed if you are not making changes to your healthcare, dental and/or AFLAC supplemental insurance coverage. Employees who wish to enroll or re-enroll in a Flexible Spending Account MUST complete a new Flexible Spending 2014 Election Form. NO action is needed if you do not wish to participate in flexible spending for the 2015 calendar year. The Health Savings Account (HSA) will be available to all benefit eligible employees beginning January 1, 2015. You MUST be enrolled in the New High Deductible Health Plan to participate in the HSA program. Employees may enroll in EITHER the Medical Reimbursement Flexible Spending Account OR the Health Savings Account BUT may NOT be enrolled in BOTH (this includes spouses). Employees MAY enroll in the Dependent Care Flexible Spending account AND the Health Savings Account. All forms and plan information may be found at http://hcps1/ or http://www.hcps.us/human_resource. Most payroll deduction changes will take effect 12/15/14. FSA and/or HSA payroll deductions for 2015 will begin 1/15/15. If you do not have access to the internet, please contact the Benefits Office for assistance at 365-4590. All forms must be received in the HR Benefit Office no later than 5:00PM on Friday, 11/14/14. Anthem HealthKeepers Health Insurance We currently offer two traditional plans through Anthem: Core and Value. We will be adding a third option this year - a High Deductible Health Plan (HDHP) coupled with a Health Savings Account (HSA), this plan will be called HDHP + HSA. For plan comparisons, please view the Anthem Booklet and Benefit Comparison on our website. The Core and Value plans will provide the same level of coverage as the 2014 plan year. All three plans: • Are open access, which means no referrals are required for specialist visits. • Have out-of-network benefits as well as prescription and Blue View Vision benefits. • Have in-network preventive care services covered at no cost. • Require a Primary Care Physician (PCP) be listed on the enrollment form for each participant. To find out if your doctor is currently participating with Anthem visit www.anthem.com or call 1-800-421-1880. The HDHP + HSA option is different compared to the other “traditional” benefit plans, but may be very appealing to many employees. The plan has a $3,000 up-front deductible for individuals, and $6,000 deductible for families that applies to all services except preventive visits (which are covered at 100%). The plan is designed to offer a lower cost option per pay period, and also allows the participant to open a Health Savings Account. This is a bank account that you can set aside tax-free dollars to help pay for the cost of the deductible. For the 2015 Plan Year, Hanover County will be contributing $1200.00/Employee Only and $2400.00/Employee + Spouse, Child(ren) or Family to the HSA if you enroll in the High Deductible Health Plan with a Health Savings Account. If you don’t use all the money by the end of the year, you KEEP IT. (This is not a use-it-or-lose-it account and it is portable if you leave HCPS employment). More details on this new plan will be provided during the Open Enrollment meetings. HEALTH INSURANCE OPT-OUT CREDIT Benefit-eligible employees who elect to waive coverage under the HCPS health insurance plan are eligible for an annual OptOut Credit. Full-time employees will receive $100 over the course of the year and part-time employees will receive $50 over the course of the year. Eligible employees who will not be participating in our health insurance plan and wish to receive the Opt-Out Credit must complete the ”Waiver of Group Health Benefits and Notice of Special Enrollment Rights” form found on our website. Employees currently receiving this credit will NOT be required to complete a new form. Delta Dental Insurance We currently offer three coverage options through Delta Dental of Virginia: Delta Premier, Delta PPO (Preferred) and DeltaCare. For plan comparisons, please view the Delta Dental Booklet on our website. The plans will provide the same level of coverage as the 2014 plan year. AFLAC Supplemental Insurance Plans We will continue to offer voluntary insurance with American Family Life Assurance Company (AFLAC). Group Critical Illness, Group Accident, Individual Cancer Protection plans will be available for enrollment. These supplemental plans are designed to help fill the gap left by expenses your medical coverage may not pay. You may also make changes to your current AFLAC plans during open enrollment. Flexible Spending Accounts (FSAs) Through payroll deduction, employees may defer up to $2,500 annually for a Health FSA and up to $5,000 annually in a Dependent Care FSA. Our FSA program is currently administered by AFLAC through “takecare” by WageWorks. More information on FSAs may be found on our website. All employees wishing to participate for calendar year 2015 MUST complete a new Election Form even if you are currently participating in this program. The form is available on our website. No action is needed if you do not wish to participate for calendar year 2015.
