How to complete the Qualification Registration Form (QRF)
The Qualification Registration Form can be found at:
https://www.cisi.org/qrfnew
Candidates are required to complete this form prior to their examination.
The reasons for the QRF are:
1. To ensure the CISI holds correct and current candidate information such
as your address, email address and date of birth
2. For UK candidates it is an Ofqual requirement that we record the
qualification programme you are registering for. International candidates
are not required to supply this information.
To log into the QRF you will need your Customer Number and Password –
these can be found on the email sent to you when you registered with the
CISI:

Once you have entered your details click ‘Log in’
When logged in, the form will be presented as in the following screenshots –
for the purpose of this guide these have been split into sections with an
explanation of the information presented or required under each heading.
Personal Details
This section of the form contains the information you provided when you first
registered with the CISI. Please check this information carefully and notify the
Customer Support team by emailing customersupport@cisi.org if you notice
any errors in your details. Unfortunately, for security reasons, your name
cannot be changed online.

If you wish to amend your contact details, the fields can be altered and the
changes will take immediate effect once you have submitted the information.
1. Choose either your home or work address as your default contact
address. This will be the address used by the CISI for correspondence
(eg this is where your certificates will be sent etc.)
2. This is your preferred contact email address and will be used for order
confirmations and exam reminders. You will also need to use this to log
into secure areas of the website
3. Please ensure your password is something secure but memorable as
you will need this to log into candidate and member areas of the CISI
website and also to access your results online
4. It is advisable that you enter either a home or work address – but both
would be preferable. Please ensure that your postcode is input
correctly as errors can delay your deliveries
UK / International Candidates
Please tick the relevant box indicating whether you are a UK or International
candidate. Please note the explanation for this at the beginning of this guide
UK candidates are required to select the programme and qualification which
they intend to undertake. There are two drop-down menus – the first is for the
general programme and the second is for the specific qualification. For
example, a candidate taking the Certificate in Derivatives would first select
‘Certificate Programme’ and then select ‘Certificate in Derivatives’ (as below).
Below is a guide to the options you should select for the relevant qualifications
(please note that some qualifications are included in more than one
programme – please choose the most relevant):

The CISI collects data on candidates’ ethnic origin and disability status in
order to monitor the effectiveness of our Diversity and Equality Policy. All
information is confidential and if you do not wish to provide this then please
choose ‘I do not wish to register this information’ on the two drop-down
menus.
CISI Membership
This section of the form gives you the option to request further information on
how to upgrade to ACSI membership you have successfully completed a CISI
qualification. It also lists, for the avoidance of doubt, those awards which do
not lead to ACSI membership.
If you would like to receive further information on how to upgrade to ACSI
membership you should click on this option to select.
CISI Data Protection
This section of the form requests a Unique Learner Number (ULN) however
this can be left blank if you do not have one. This is a UK-based initiative
where all people (over the age of 14 years) are allocated a ULN when
undertaking UK education or training.
For candidates outside of the UK this can be left blank.
You also have the option here to opt out of sharing participation and
achievement data – tick the box.

If your exam is booked either by your firm or an Accredited Training Provider
(ATP), those parties would then (potentially) have access to your result in that
particular exam.
1. Tick this box if you wish to block the result of your exam from your
employer or ATP (if they booked the exam).
2. Tick this box if you wish to block your score (mark) from your employer
or ATP (if they booked the exam).
If you select both boxes your results and mark would then be blocked. Whilst
this does not affect your certificates from being issued, this would block you
from accessing your exam mark via the CISI website.
If you have paid for the exam directly yourself, it is worth noting that you
would be the only party to have access to your exam information. Results
would not be given to your firm or any other third party in this situation.
Declaration
You must tick this box to acknowledge that the information you have provided
is correct and you agree to the CISI’s examination regulations.
Once all the sections of the QRF are complete click ‘Submit’
