1 Evropska konferenca naravnih zdravilišč in wellnessa ENSPA Skupnost slovenskih naravnih zdravilišč z organizacijo 1. Evropske konference naravnih zdravilišč in wellnessa ENSPA odpira priložnost za prenos znanja, izkušenj in dobrih praks na področju medicinske rehabilitacije, zdraviliškega zdravljenja, wellnesa, mednarodnih standardov za zdravilišča in wellness centre. Seznanila bo z najnovejšimi trendi na teh področjih in odgovorila na vedno aktualno vprašanje o razmerju med medicinskim in wellness turizmom. Klasična zdravilišča v Sloveniji in bližnji okolici so v zadnjem desetletju svoje storitve dopolnila tudi s številnimi preventivnimi programi in prav ta imajo v prihodnosti še velik razvojni potencial. Na konferenci ENSPA pričakujemo, da se bo zbralo do 100 strokovnjakov, predvsem iz slovenskih naravnih zdravilišč in term, ponudnikov wellness storitev in udeležencev sorodnih profilov iz tujine. V naslednjih letih je naš cilj, da konferenca postane še bolj mednarodno prepoznavna in priznana kot mesto spremljanja aktualnih, kot tudi iskanje in postavljanje novih trendov. Prepričani smo, da je konferenca že letos odlična priložnost za pozitiven dialog, nove poslovne priložnosti in navezovanje novih poslovnih stikov. Janez Kraševec Univerza v Mariboru, CIMRS, Inštitut za balneologijo in medicinsko klimatologijo, Slovenija Prikazan bo kratek zgodovinski oris razvoja balneologije od prazgodovine do danes. Prikazani bodo osnovni balneološki pojmi o termalnih vodah, peloidih in drugih naravnih dejavnikih v slovenskih naravnih zdraviliščih. Balneoterapija in znanost o balneologiji so medicinske vede, ki nujno potrebujejo svojo rehabilitacijo, tako v strokovnem kot v organizacijskem smislu. Slovenska naravna zdravilišča, skupaj z možnostjo talasoterapije v obmorskih krajih in možnosti klimatoterapije v sredogorju in visokogorju, predstavljajo potencialne razvojne resurse za razvoj zdraviliškega turizma. Restrikcijski programi in omejevanje zdravstvenega varstva, plačljivega s strani ZZZS, prisiljujejo ljudi, da začnejo pravočasno skrbeti za svoje zdravje, iskati preventivne ukrepe za preprečitev nastanka bolezni, za zmanjšanje posledic staranja in razvoja kroničnih bolezni. Zdravilišča so s svojimi potenciali najprimernejši resursi, kjer bi ljudje, ki iščejo tako preventivne kot kurativne storitve, lahko najhitreje dobili pravilen strokovni in pravočasni odgovor na svoje zahteve. Povezanost slovenskih naravnih zdravilišč s sodobno organizirano šolsko rehabilitacijsko medicino na eni strani, na drugi strani pa razvijanje novih oblik prevencije bolezni, je vizija razvoja zdravilišč. Če k temu prištejemo še ponovni vzpon alternativnih oblik zdravljenja, kjer predstavlja naravni dejavnik pomemben del, ter iskanje vedno novih oblik, prinesenih z Vzhoda ali Zahoda (akupunktura, ayurveda, fitoterapija, body-mind medicine), potem je možno dobiti holistični pristop zdravstvenih in rekreativnih storitev v slovenskih zdraviliščih. Zdravstvena prosveta je tista, ki bo znala ljudem predstaviti koristnost uporabe naravnih dejavnikov v prevenciji bolezni in ohranitvi zdravja. Na medicinskih strokovnjakih in menedžmentu slovenskih zdravilišč leži odgovornost najti pravi način strokovne povezanosti in poslovne odličnosti ter uporabe naravnih resursov, ki so nam v Sloveniji dani. Basic balneologic terms about thermal waters, peloids and other natural factors in Slovenian natural spas will be explained. Balneotherapy and the science on balneology are medical sciences that are in urgent need of rehabilitation themselves both in expert and organisational sense. Slovenian natural spas, including option of thalassotherapy in costal centres and option of climatherapy in highlands represent potential development resources for development of spa tourism. Restriction programmes and limits on health care paid for by The Health Insurance Institute of Slovenia (ZZZS) are forcing 9 people to start looking after their own health on time, to look for preventive measures for avoiding illness, to limit consequences of ageing and to avoid the development of chronic diseases. Spas are the most appropriate resources with their potentials, where people looking for both preventive and curative services could get the correct expert and timely answer to their needs. Connection between Slovenian natural spas on one side, with well organised school rehabilitation medicine, and on the other side the development of new forms of illness prevention is the vision of development of spas. If we add to this the upturn of alternative ways of healing, where natural factor is important and the search for ever new forms brought from East and West (acupuncture, ayurvedic medicine, phytotherapy, body-mind medicine) then we can have a true holistic approach to medical and recreational services in Slovenian spas. Health education will know how to present the value of natural factors in prevention of illnesses and keeping of good health. Responsibility lies on medical experts and on the management of Slovenian natural spas to find the right way of expert integration and business excellence as well as the use of natural resources that we have in Slovenia. Key words: balneology, climatology, medical tourism, medical rehabilitation Rehabilitacija v 21. stoletju Helena Burger Univerzitetni rehabilitacijski inštitut Republike Slovenije Maja 2001 je Svetovna zdravstvena organizacija sprejela Mednarodno klasifikacijo funkcioniranja, zmanjšane zmožnosti in zdravja (MKF), ki uveljavlja biospishosocialni model (1). Klasifikacija in model sta postala osnova fizikalne in rehabilitacijske medicine. Glavni cilj rehabilitacije je osebam z zmanjšano zmožnostjo omogočiti, da živijo kot bi same želele, kljub omejitvam, ki so posledica poškodb ali bolezni, ali njihovih osebnih dejavnikov. To najlažje dosežemo s kombinacijo ukrepov, s katerimi zmanjšamo ali zaobidemo okvare ter odstranimo ali zmanjšamo ovire v njihovem okolju (2). Rezultat je celostna rehabilitacija oseb usmerjena v vse vidike funkcioniranja posameznika. Rehabilitacija poteka v krogu prva ocena, postavitev kratkoročnih in dolgoročnih ciljev, priprava načrta rehabilitacije in izbor ustreznih metod, izvajanje posameznih terapevtskih postopkov ter ocena uspešnosti oziroma izida. Metode, ki jih uporabljamo morajo temeljiti na najnovejših dokazih kliničnih študij, testi za oceno pa morajo imeti dobre psihometrične lastnosti. Rehabilitation in the 21st century Helena Burger University Rehabilitation Institute Republic of Slovenia In May 2001 World Health Organisation launched International classification of functioning, disability and health (ICF), which introduced biopsychosocial model (1). Classification and model 10 became the basis of physical and rehabilitation medicine (PRM). The overall aim of rehabilitation is to enable people with disabilities to lead the life that they would wish, given any restriction imposed on their activities by impairments resulting from illness or injury as well as from their personal context. In practice this can be achieved by combination of measures to overcome their impairments and to remove or reduce the barriers in their environment (2). The result is comprehensive rehabilitation focused on all levels of individuals functioning. Rehabilitation is performed in a rehabilitation cycle. It starts with initial assessment, setting goals and preparing rehabilitation pan, choosing appropriate methods and their implementation and at the end evaluation of outcome. Methods used should base on use of current best evidence from clinical care research in the management of individual patients. Zdravstveni-wellness spekter obsega vse od kirurgije do zdravstvenega wellnessa, od zdravilnih, termalnih vod do lepotilnih salonov. Terminologija in tipologija postajata vedno bolj zapleteni, izvajalci in pacienti pa prepoznavajo, da je optimalno zdravje mogoče doseči samo s holističnimi pristopi. Predstavljeni bodo tudi rezultati mednarodne študije, ki je bila izvedena za potrebe globalnega vrha spa centrov („Global Spa Summit”) leta 2011. Razščiščeni bodo pojmi in definicije, prikazana bo raznolikost produktov zdravstvenega in wellness turizma. Dodatno bodo predstavljene analize poglobljenih raziskav, ki so bile izvedene v 12 državah, ki naj bi bile reprezentančne za svoje regije, s tem znanjem pa bo avtorica kombinirala tudi svoje predhodno znanje o različnih pristopih k zdravju in wellnessu, ki jih lahko prepoznamo v zahodni, srednji, vzhodni, severni in južni Evropi. To kaže na pomen regionalne zgodovine, geografije, resursov in kulture pri oblikovanju razvoja zdravstvenega in wellness turizma. Prezentacija se zaključi z ugotovitvijo, da so potrebne dodatne na dokazih temelječe raziskave, da bi ’dokazali’ učinkovitost številnih tretmajev, za katere trenutno velja, da so osredotočeni samo na wellness. To vključuje starodavne, tradicionalne in avtohtone zdravstvene prakse. Morali bomo tudi 11 prepoznati, da mentalni, psihološki in duhovni dejavniki lahko prav tako igrajo ključno vlogo pri pacientovem dobrem počutju, zmanjševanju stresa in okrevanju. Zdravstveni in wellness turizem torej ne moreta biti obravnavana kot popolnoma ločena, na kar kažejo novi trendi in nove ’modne’ besede, o katerih bo govora na koncu te prezentacije. The Relationship between Medical and Wellness Tourism Melanie Smith Budapest Business School, Hungary This presentation will consider the relationship between medical and wellness tourism, including definitions, typologies, national and regional developments and new trends. The medical-wellness spectrum runs from surgery to medical wellness to healing waters to thermal spas to beauty salons. However, terminology and typologies are becoming more and more complex as practitioners and patients alike recognise that optimum health can only be achieved through holistic approaches. The results of an international study undertaken for the Global Spa Summit in 2011 are presented, which clarify definitions and show the diversity of medical and wellness tourism products and tourist activities. In addition, an in-depth analyses of twelve countries considered to be representative of their regions is combined with the researcher’s earlier work on the different approaches to health and wellness which can be identified in Western, Central and Eastern, Northern and Southern Europe. This shows the significance of regional history, geography, resources, and culture in shaping the development of medical and wellness tourism. The presentation concludes that there is a need for more evidence-based research to ’prove’ the effectiveness of many treatments currently considered to be focused on wellness only. This includes ancient, traditional and indigenous health practices. Key words: medical tourism, wellness tourism, relationship, diversity of products Športna rehabilitacija v naravnih zdraviliščih Dr. Vlasta Brozičević Terme Selce and Biodex isokinetic reference center, Hrvaška Športna rehabilitacija vključuje postopke, ki so namenjeni maksimizaciji funkcionalnosti po poškodbi ali bolezni. Vključuje postopke za povrnitev športnikovega stanja na predhodno stopnjo funkcionalnosti v najkrajšem možnem času. Rehabilitacija se začne takoj po poškodbi, nadaljuje skozi akutno in subakutno fazo poškodbe ali kirurškega posega in se zaključi s športnikovim varnim povratkom med športnike. Rehabilitacija in povratek k športu sta kompleksna tako zaradi specifičnosti vsake poškodbe kot tudi zaradi pričakovanj samih športnikov, trenerjev in drugih vključenih v proces rehabilitacije in vrnitev poškodovanih športnikov. Z izboljšanjem življenjskih razmer se povečuje število posameznikov, ki se aktivno ukvarjajo s 12 športom. Kljub izboljšanim pogojem, športni opremi in treningu je še vedno velika pojavnost športnih poškodb. Učinkovit pristop pri rehabilitaciji športnih poškodb je multi-disciplinaren timski pristop. Ne glede na vrsto športa, starost in zapletenost poškodbe je pristop individualen. Potek rehabilitacije je odvisen od ustrezne prve pomoči, natančne diagnoze, izbranega tretmaja ali operacije, različnih metod rehabilitacije, predanosti športnikov in drugih sodelujočih. Cilj rehabilitacije je, da se športnika v čim krajšem času povrne k treningu, ne da bi tvegali nove poškodbe. Varen povratek zagotavlja konstantno spremljanje rehabilitacije. Zdravilišča, ki uporabljajo naravne dejavnike pri preventivi, diagnozi in rehabilitaciji lokomotoričnega sistema so idealno mesto za posameznike in klube za okrevanje po športnih poškodbah. It includes procedures designated to restore athletes to their previous level of function within the shortest time possible. Rehabilitation begins immediately after the injury, progresses through the acute and sub acute phases of injury or surgery, ending in safe return to sport. Rehabilitation and return to sports is complex, both because of specific injuries and the expectations of athletes, coaches and other participants in the process of recovery and return of injured athletes. By improving living standards there is growing percentage of those who are actively engaged in sports activities. Despite the improved conditions, sports equipment and training there is increasing number of the incidence of sports injuries. Effective approach in the rehabilitation of sports injuries is multidisciplinary team approach. Regardless of the type of sport, age and complexity of the injury approach is individual. The course of rehabilitation depends on the proper first aid, precise diagnosis, chosen treatment or surgery, differentiated methods of rehabilitation, dedication of athletes and other participants. Rehabilitation’s goal is to train athletes in a short time without the risk of new injuries. Safe return is guaranteed by continuous monitoring, rehabilitation’s evaluation and return to sport criteria. "Medical Spas" that use natural factors in prevention, diagnosis and rehabilitation of the locomotor system are the ideal place to individuals and clubs to recover after a sports injury. Human resources and continuous education, tradition, spatial abilities, technological equipment, climatic factors, geographic location, sports facilities, gastronomic offer, and other offer make such institutions an ideal place for the rehabilitation of athletes, and for a shorter or longer stay, for holiday, relaxation, preparation, education and sports events. Key words: sport rehabilitation, athlets, medical spas, recovery, preparation 13 Športna rehabilitacija po rekonstrukciji sprednje križne vezi (SKV) pri uporabi presadka kite pogačice in kit fleksorjev v Termah Zreče Doc. dr. Matjaž Sajovic, dr. med. Oddelek za ortopedijo in športne poškodbe, Splošna učna bolnišnica Celje, Slovenija Izhodišča Izbira presadka v primarni rekonstrukciji SKV je še vedno nedorečena. Za ugotovitev morebitnih razlik med obema presadkoma so potrebne kratkoročne in dolgoročne, prospektivne, randomizirane študije. Prav tako je potrebno ugotoviti, ali je pospešena rehabilitacija primerna za oba presadka. Metode Bolnike, ki so bili zaradi pretrgane SKV operirani med junijem 1999 in marcem 2000, smo naključno razdelili v dve skupini. V STG skupini z 32 bolniki sta bili kot presadek uporabljeni kiti mišic semitendinosus in gracilis, v PK skupini s 32 bolniki pa presadek iz dela kite pogačice. Vse operativne posege je opravil isti kirurg. Po-operativno so vsi bolniki izvajali enak pospešen rehabilitacijski protokol. Rezultati Po ocenitvenem sistemu Lysholm, ki smo ga pridobili 5 mesecev po operativnem posegu, je 85% bolnikov v STG skupini in 70% bolnikov v PK skupini doseglo odličen rezultat. Povrnitev polne gibljivosti je bila pogostejša v STG skupini (100 % versus 80 %). Manjši deficit iztegnitve ( 3°) je bil pogostejši v PK skupini (15 % versus 0 %). Patelofemoralne krepitacije so bile prisotne v 5 % STG skupine in v 30 % PK skupine. Testiranje s KT-1000 artrometrom pokaže 3 mm pri 80 % skupine PK (povprečje 1.8 mm) in v 85 % skupine STG (povprečje 2.0 mm). Izokinetično testiranje smo izvedli v 12 in 20 tednu. Ob 5 mesecu smo izmerili moč ekstenzorjev operiranega kolena (Power test: 60 stop/sec), ki je znašala 84 % v STG skupini versus 61 % v PK skupini. Izmerjeni deficit moči fleksorjev kolena je znašal 8 % v STG skupini in 4 % v PK skupini. 11 let po operativnem posegu je bilo 22 bolnikov (81 %) v STG skupini in 18 bolnikov (72 %) v PK skupini še vedno tekmovalno aktivnih. Do ponovne raztrganine SKV je prišlo pri 2 bolnikih (6 %) v STG skupini in pri 4 bolnikih (12 %) v PK skupini. Rentgenski znaki degenerativnih sprememb rekonstruiranega kolena so bili prisotni v 84 % (21/25) bolnikov v PK skupini in 63 % (17/27) bolnikov v STG skupini (P = .008). Zaključki Kratkoročni rezultati obeh operativnih tehnik ne pokažejo signifikantnih razlik v po-operativni stabilnosti kolena, toda patologija odvzemnega mesta presadka igra odločilno vlogo pri funkcionalnosti operirane ekstremitete. Pospešena rehabilitacija je primerna pri uporabi obeh presadkov. 11 let po operativnem posegu oba presadka zagotavljata dobro subjektivno oceno in objektivno stabilnost kolena. Signifikantnih razlik v odpovedi presadka nismo našli. Rentgenski znaki degenerativnih sprememb so pogostejši pri bolnikih s presadkom iz dela kite pogačice. MD. Department for Orthopedic and Sports Trauma Surgery, General Hospital Celje, Slovenia Background There are still controversies about graft selection for primary ACL reconstruction. Prospective, randomized short-term and long-term studies are needed to determine the differences between graft materials. It is still not clear if rehabilitation program should be adjusted to graft choice. Methods From June 1999 to March 2000, 64 patients were included in this prospective study. A single surgeon performed primary arthroscopically assisted ACL reconstruction in an alternating sequence. In 32 patients, ACL reconstruction was performed with hamstring tendon autograft (STG group) while in the other 32 patients, ACL reconstruction was performed with patellar tendon autograft (PT group). Pooperatively, all patients underwent the same accelerated rehabilitation protocol. Results Lysholm scores were obtained at 5 month follow-up. 85% in STG group and 70% in PT group had excellent results. Return to full ROM was more frequent in STG group (100% versus 80%). A minor extension loss ( 3°) was more frequent in the PT group (15% versus 0%). Deterioration of patellofemoral crepitation developed in 5% of STG group and in 30% of PT group. KT-1000 arthrometer side-to-side difference 3 mm at 30 pounds was present in 80% of the knees of PT group (average 1.8 mm) and in 85% of those of STG group (average 2.0 mm). The isokinetic muscle tests were performed at 12th and 20th week. At 5 months follow-up, the mean peak extension torque value of the reconstructed knees (Power test: 60 deg/sec) was 84% in STG group versus 61% in PT group. The corresponding hamstring peak torque deficit was 8% and 4% respectively. At 11 years follow-up, 22 patients (81%) in the STG group and 18 patients (72%) in the PT group were still on their pre-injury level of activity. Graft rupture occurred in 2 patients from the STG group (6%) and in 4 patients from the PT (12%). The grade B and C abnormal radiographic findings were seen in 84% (21/25) of patients in the PT group and in 63% (17/27) of patients in the STG group (P = .008). Conclusion In short term follow-up, both operative techniques do not show significant differences in postoperative stability of the knees, but donor site morbidity plays a major role in functional outcome of extremity. The accelerated rehabilitation program is appropriated for both graft material. At 11 years, both STG and PT autografts provided good subjective outcomes and objective stability. No significant differences in the rate of graft failure were identified. Patients with patellar tendon graft had a greater prevalence of osteoarthritis at 11 years after surgery. Običajno so premeščeni v zdravilišče dva do tri tedne po nastanku možganske kapi. Osnovni namen rehabilitacije je obnovitev prizadetih motoričnih funkcij in osnovnih dnevnih aktivnosti. Obravnava bolnikov je skupinska, vodi jo specialist fizikalne in rehabilitacijske medicine, sodelujejo diplomirani fizioterapevti, diplomirani delovni terapevti, diplomirane medicinske sestre, zdravstveni tehniki in bolničarji. Motorično reedukacijo živčno mišičnega sistema izvajamo z različnim tehnikami fizikalne terapije. S pomočjo specifičnih nevroterapevtskih vaj in metod učimo bolnike dnevnih aktivnosti in nadaljujemo z obravnavo kognitivnih aktivnosti Rehabilitacijsko nego izvajamo na negovalnem oddelku neprekinjeno štiriindvajset ur. Trajanje rehabilitacije je omejeno na štirinajst oziroma enaindvajset dni. Omejeno je število in čas posameznih terapij. Post-Stroke Subacute Rehabilitation of Patients at Thermana Laško Dr Topolić Slavka, a specialist in physical medicine and rehabilitation Dr Silvester Krelj, a specialist in physical medicine and rehabilitation Thermana d.d., Laško At Zdravilišče Laško, a constituent part of Thermana, d.d., we admit patients with a stroke immediately after the completion of their hospital treatment. Usually, they are transferred to our health spa two to three weeks after the onset of a stroke. Rehabilitation’s basic goal is the restoration of affected motor functions and basic daily activities. There is a group treatment of patients led by a specialist in physical medicine and rehabilitation, 16 assisted by graduated physiotherapists, graduated occupational therapists, graduated nurses, medical technicians and paramedics. The motor retraining of the neuromuscular system is performed with a variety of physical therapy techniques. Using specific neurotherapeutic exercises and methods, we teach our patients how to perform daily activities as well as continue with the treatment of cognitive activities. Rehabilitation care is performed on the nursing ward 24 hours a day without a break. The duration of rehabilitation is limited to two or three weeks. Furthermore, the number and duration of therapies is also limited. Annually, we treat 250 to 300 patients suffering from a stroke, who have been transferred here directly from Slovenian hospitals. Key words: Subacute rehabilitation, health spa, stroke Konzervativni pristop k zdravljenju Ahilove tendinopatije Dr. Tanja Rauter Pungartnik Katja Skaza Terme Zreče, Slovenija Glavni vzrok za nastanek tendinopatij je preobremenitveni sindrom, ki se začne s postopnimi bolezenskimi spremembami. Očitni simptomi z vsemi neugodnimi posledicami za športnika se pojavijo sorazmerno pozno v obdobju izvajanja športne aktivnosti. Tipične značilnosti razvoja preobremenitve gibal: postopen in nezaznaven začetek okvare, težave so običajno povezane s tipično aktivnostjo, športniki pogosto začetnih znakov ne vzamejo dovolj resno in jih pripisujejo neogretosti, pretiravanju v obremenitvi in neugodnim zunanjim vplivom. Ahilova tendinopatija je boleče stanje, ki se pojavlja pri aktivnih in tudi malo manj aktivnih posameznikih. Bolečina se pojavi kot posledica nanovo nastalih žil s pripadajočimi živci. Osnovno zdravljenje se začne s tradicionalnimi neinvazivnimi terapijami, ki vključujejo: modifikacijo aktivnosti, ortoze (da se popravi nepravilen položaj stopala), raztezanje, dvig pete (da se sprosti napetost v kiti), masažo, termoterapijo, UZ, nesteroidna protivnetna zdravila. Prispevek je pregled fizikalnih modalitet s posebnim poudarkom na predstavitvi učinkovitosti ekscentričnih vaj s postopno obremenitvijo, s katerimi odpravljamo bolečine, ki se pojavijo pri pojavu tendinopatije. Program ekscentrične vadbe vključuje spuščanje stopala iz polozaja polne plantarne fleksije v dorzalno fleksijo s kolenom v iztegnjenem (za gastroknemius) in rahlo pokrčenem polozaju (za soleus). Vaje se izvajajo na stopnici in sicer tri serije po 15 ponovitev, 2x dnevno, vsak dan v tednu. Svetuje se izvajanje v obdobju 12 tednov, kjer se obremenjevanje progresivno stopnjuje. Najprej se vaja izvaja z lastno tezo, ko bolečina izgine, dodamo obremenitev preko nahrbtnika ali z dodajanjem prostih utezi v primeru potrebe po velikih obremenitvah. Tak pristop da pozitivne rezultate pri 80% pacientov. Obvious symtomps with all negative consequences for the athlete appear relatively late in the period of activities. Typical features of the overuse syndrome are: the gradual and imperceptible starting of the symptoms, problems are usually associated with typical activity, athletes don't take seriously early symptoms. Achilles tendinopathy is a painful condition that occurs in active and less active individuals. Pain occurs as a result of newly generated blood vessels associated with nerves. Primary treatment begins with the traditional non-invasive therapies, which include: activity modification, orthoses (to correct the incorrect position of the foot), streching, heel raising (to release tension in the tendon), massage, thermotherapy, ultrasound, non-steroidal anti-inflammatory drugs. This article reviews the physical modalities with special emphasis on presentation the efficiency of eccentric exercises with progressive load to eliminate the pain, which occur in the tendon. The program of eccentric exercise involves lowering the foot from the position of full plantar flexion to dorsal flexion with the knee extended (for gastrocnemius) and in slightly bent position (for soleus). Exercises are carried out on the stairs and through three sets of 15 repetitions, 2 x day, every day of the week. The exercices should extend period of 12 weeks with the load progressively increasing. First, the exercise is carried out by its own weight and when pain disappears, we add the load over a backpack or by the addition of extra weights in the case of the need for high loads. Such an approach showed positive results in 80% of treated patients. Današnjo predstavo o zdravju povzemamo v smislu, da je zdravje moč realizirati, ga interpretirati, družbeno definirati in z njim tržiti. Zdravja torej ne smemo pojmovati samo kot odsotnost motenj v telesu, temveč kot sposobnost živeti z določenimi omejitvami. Takšno pojmovanje sovpada z velneškimi cilji, da je potrebno izkoristiti vsak zdravstveni potencial posameznika. Zakaj v zadnjih letih tako velik porast turizma, ki je bolj ali manj povezan z zdravjem? Odgovor je iskati v podaljševanju življenjske dobe ljudi in v iskanju kakovostnejšega življenja po meri posameznika. Temeljno poslanstvo velneških ponudnikov je v izobraževanju svojih uporabnikov za zdravi življenjski slog, ki ga naj udejanjajo v svojem življenju in ne samo na aktivnih počitnicah. V raziskavi smo analizirali nakupne motive turistov v izbranih hotelih v Sloveniji in Avstriji in jih primerjali z njihovo skrbjo za zdravje in dobro počutje, ki so ju izrazili z dvanajstimi trditvami. Proučevali smo, ali obstajajo statistično značilne razlike v pomembnosti teh dejavnikov med gosti v hotelih v Sloveniji in Avstriji in glede na sociodemografske spremenljivke ter jih primerjali z nakupnimi motivi. Ugotovili smo, da so za turiste, ki sami zelo intenzivno 18 preferirajo skrb za svoje zdravje in dobro počutje, med najbolj pomembnimi nakupni motivi s področja zdravja, športa in rekreacije ter počitka in sprostitve. Slavka Gojčič Go-spa S d.o.o., Slovenia Wellness tourism is a very important segment of the wellness activity (industry); its offer is based and combined on the promotion of health or the prevention of healthy living. Today’s perception of health can be summarized as health is achievable; it can be interpreted, socially defined and marketed. Health therefore can not be defined as “a lack of disturbances” (in a person’s body), but as being able to live with limitations. This definition coincides with wellness goals, like it is necessary to make the most of each individual’s medical prospective. Why is there such a strong increase of tourism in the past years that is more or less connected with health? The answer can be found in the extended lifetime of people and in the search for a higher quality of living for every individual person. The basic mission of wellness providers is educating its users how to live a healthier life, how to implement “wellness” into their everyday life (not only during their vacation and free time). In our research we analyzed buying motives in selected hotels in Slovenia and Austria and we compared their concern for health and wellbeing, which was stated in twelve hypothetical sentences. We tried to find out if there are statistical differences in the importance of these key factors between guests in Slovenian and Austrian hotels and socio-demographic variables and compared them to buying motives. We documented that tourist, who intensively care for their health and wellbeing themselves have high buying motives towards areas of health, sports, recreation as well as rest and relaxation. Nataša Artič Višja strokovna šola za gostinstvo Maribor, Slovenija Velnes je v slovenskem prostoru prisoten že skoraj desetletje in v tem času se je v stroki nenehno nadgrajeval. Danes lahko gledamo nanj ali z vidika velneške dejavnosti (podjetja) ali kot način življenja (posameznika). Leta 2010 je v Sloveniji stopil v veljavo prvi akreditiran, javno veljavni izobraževalni program za področje velneške dejavnosti, čeprav je že prej bilo mogoče zaslediti študijske programe, ki ponujajo predmete oziroma module z velneško tematiko. Ob dejstvu, da je Slovenija v preteklem desetletju investirala v razvoj velneške infrastrukture ogromna sredstva, je prišel čas, da se ob standardih na strani ponudbe vzpostavijo tudi standardi na področju izobraževanja za potrebe izvajanja velneške dejavnosti. S prenosom javno veljavnega nemškega programa usposabljanja na področju velnesa v Slovenijo govorimo o prvih zametkih postavljanja standardov velneške dejavnosti tudi na področju izobraževanja, kar je zagotovo pripomoglo k razvoju teorije in vsebin tega področja v prihodnje in ne nazadnje tudi k razvoju samostojnega javno veljavnega izobraževalnega programa za organizatorja velneške dejavnosti. V študijskem letu 2011/12 se v Sloveniji izobražuje po študijskem programu Velnes prva generacija vpisanih študentov. Istočasno smo v Sloveniji priča naglemu razvoju velneške 19 dejavnosti, uveljavljanju svetovnih trendov slednje ter povečanem povpraševanju po storitvah, povezanih s skrbjo za zdravje in zdravim načinom življenja. Ob tem se tudi izobraževalna sfera srečuje z novimi izzivi za nadaljnji razvoj usposabljanj in izobraževanj za potrebe velneške dejavnosti. The transfer of officially recognized German training program for wellness services to Slovenia set the first foundations of wellness service standards. In year 2010 there has been accredited publically valid study program for the wellness activity in Slovenia, even thou there has been some private program on the market before. In study year 2011/20 there is the first generation of wellness program students on the way to degree. At the same time Slovenia is facing with the fast development of wellness market, new trends and demand acceleration. The growing market is highly challenging for the further development of wellness education and training. Bistvene značilnosti velnes kulinarike so uporaba lokalnih sezonskih živil ter skrbno sestavljeni jedilniki, ki zagotavljajo zadostno količino antioksidantov, vitaminov in mineralov. Prav tako pa je iz izobraževalnega in animacijskega vidika velneškega gosta potrebno stremeti k ponudbi kreativnih jedi in pijač, ki očarajo vse čute in dovolijo gostu za mizo »varno« razvajanje. Tako imenovana »doživljajska kulinarika« ima svojo načela in posebnosti ter se lahko za potrebe velneške ponudbe Slovenije razvije na osnovi slovenske kulinarične dediščine. The main characteristics of wellness cuisine are local and seasonal products and sophisticated menu planning, which ensures needed amount of antioxidants, vitamins and minerals. From the wellness guest aspect of education and motivation, it is also important to look for creative food and drink offer which will charm all the senses and allow the guest “safe” enjoyment. So called adventure cuisine has its specifics and principles that can be adapted from Slovenian national cuisine for the wellness product of Slovenia. Therefore it is necessary for local quality products to be consistently included in modified local recipes as well as the design of complete gastronomic offer that will beside the taste also include ethnographic origins of Slovenia. In theoretical part of the report we will present the fundaments of national cuisine that will be used as a starting point for the modification. Furthermore there will be presented the basics of healthy diet in wellness offer and its transformation for the adventure cuisine. Specialistka fizikalne medicine in rehabilitacije, Slovenija Večinoma razmišljamo na osnovi stereotipov o lepoti in ne o kvaliteti počutja - vse pač povežemo s tem, kar vemo o anti-aging postopkih: modne smernice, kozmetična sredstva, kirurgija, vitkost za vsako ceno, glajenje gub in še kaj v povezavi biti lep in mlad. V svetu medicina uporablja bioidentične hormone za izboljšanje kvalitete počutja. Uporabljajo se v zadnjih letih, ko vse večje število zdravnikov ne zdravi le očitnih pomanjkljivosti in izvidov, temveč tudi mejne. Sodoben pristop, ki spominja na preventivo, pa temelji v glavnem na bolnikovih težavah in telesnih znakih – terapija se predpiše tudi takrat, ko so bolnikove vrednosti (morda) še v mejah normale, oseba pa že ima določene težave. Poseben poudarek je na bolnikih, ki imajo tipične znake določenega pomanjkanja npr. hormonov, njihovi laboratorijski izvidi pa so še v mejah normale. Najbolj nazoren primer je perimenopauza, obdobje pred in v začetku mene, ko imamo ženske številne težave, vrednosti hormonov pa so velikokrat v mejah normale. Največ težav v tem obdobju povzroča pomanjkanje progesterona, za njim je estrogen, padec ostalih hormonov. Nizek progesteron povzroča slabo počutje, bolečine in napetost v sklepih in mišicah, razdražljivost, jeza, znižan prag bolečine, nespečnost – v tem obdobju so pogoste ločitve. Bioidentični hormoni (BiH) imajo popolnoma enako zgradbo kot hormoni, ki se po naravni poti izločajo v človeškem telesu. Nasprotno pa je struktura modificiranih (ne-bioidentičnih) sintetiziranih hormonov (SH) drugačna in različna od tiste, ki jo imajo naravni hormoni. Popravljanje vrednosti na višjo raven zavaruje ženske (in moške) pred staranjem, da nov zagon za življenje, prenavlja duševne in telesne energije. Popravljen nivo hormonov ščiti tudi pred Alzheimerjevo boleznijo in pred pretiranim gubanjem, pomaga tudi pri težavah multiple skleproze, pri otrocih in še kje. Terapije, ki jih izvajamo, delujejo resorbtivno, stimulativno, preventivno, podpirajo rekonvalescenco in rehabilitacijo. Obravnavane so indikacije za ginekološko balneologijo, kot so kronična vnetja, sterilnost in infertilnost, hormonske motnje, rehabilitacija po zdravljenju malignomov in rehabilitacija po komplikacijah po ginekoloških operacijah. Pomembno je, da vse te terapije izvaja ustrezno izobražen kader pod vodstvom zdravnika ginekologa in fiziatra ob upoštevanju kontraindikacij za posamezne terapije. Tradition of healing gynecological diseases in Terme Dobrna Dr. Marija Ocvirk Terme Dobrna, Slovenia In article is given brief hystorical survey of development of healing gynecological diseases in Terme Dobrna, and professional organisation of our work and effect of use of natural factors in medical programs and comprehensive influence on the body. Therapies used are acting resorbtively, simulative, preventive and are supporting reconvalescence and rebilitation. The main indications for gynecologic f are chronic infflammations, sterility and infertility, hormones disregulations, rehabilitions after treatments of malignoma, rehabilitations for complications after gynecologic operations. It is of great importance that gynecologic therapy is applied by professionally educated team under the supervision of gynecologist and physiatrist, having in mind of the indications and contraindications for applied therapies. Terme Olimia, Slovenija Dinamična nevromuskularna stabilizacija (DNS) je razmeroma nov koncept za konzervativno obravnavo pacientov z vertebrogeno patologijo. Metodo je razvil v devetdesetih letih 20. stoletja 23 v Pragi prof. Kolář, učenec prof. Lewita in pokojnega prof. Vojte, katerega metoda je uveljavljena na področju nevrorehabilitacije otrok. DNS temelji na korekciji gibalnih vzorcev in funkcionalnih odnosov med mišicami in sklepi, tako kot jih definira razvojna kineziologija, s ciljem doseči boljšo kakovost in časovno usklajenost aktivacije stabilizacijskih mišičnih verig, korigirati smer vleka mišic in doseči boljšo centracijo opornih segmentov telesa. V predavanju bodo podrobneje opisani dinamični testi, ki se uporabljajo za oceno stabilizacijske funkcije hrbtenice po metodi DNS, praktična načela terapevtskega pristopa in rezultati zdravljenja, pri pacientih po operaciji hernije medvretenče ploščice - spremljani z lestvico ´Back Pain Index´ v Termah Olimia. Treatment of patients with vertebrogenic pathology in Terme Olimia using the method of Dynamic Neuromuscular Stabilization Peter Kurila, dr.med. Terme Olimia, Slovenia Dynamic neuromuscular stabilization (DNS) is a relatively new concept for conservative treatment of patients with vertebrogenic pathology. The method was developed in the nineties of the 20th century in Prague by prof. Kolář, a disciple of prof Lewit and late prof. Vojta, whose method is established in the field of neurorehabilitation of children.DNS is based on correcting movement patterns and functional relationships between the muscles and joints, as defined by developmental kinesiology, with the aim of achieving better quality and timing of activation of stabilizing muscle chains, to correct the direction of muscle pull and achieve better centration of supporting segments of the body. The presentation will further describe the dynamic tests which are used to assess the stabilizing function of spine according to the DNS method, practical principles of the therapeutic approach and results of the treatment in patients following surgery due to herniated disc – monitored by ´Back Pain Index´ scale in Terme Olimia. Terme Olimia, Slovenija Posamezniki, ki jih je prizadela huda osteoartroza kolena, se srečujejo z znatnimi funkcionalnimi omejitvami, zmanjšano mišično močjo spodnjih udov, manjšo telesno vzdržljivostjo in upadom kakovosti življenja. Potreba po rehabilitaciji, ki sledi artroplastiki (AP) kolena, temelji na dokazih, da se nastali funkcionalni izpadi ne popravijo samodejno po operaciji. Čeprav AP kolena sledi hitro in bistveno zmanjšanje bolečine, 37 % bolnikov beleži eno leto po operaciji le omejeno izboljšanje funkcije. Najpogostejše omejitve pri teh bolnikih so počasnejša hoja, težave pri hoji po stopnicah in nezmožnost povrnitve k športom, s katerimi so se ukvarjali pred operacijo. Šibkosti mišice kvadricepsa je bilo posvečeno dovolj znanstvene in raziskovalne pozornosti in številne študije so ocenjevale vlogo kvadricepsa pri funkcionalnih 24 omejitvah po AP kolena. Glede na raziskave, objavljene lani, vloga kolčnih abduktorjev pri funkcionalni omejitvi po AP kolena zna biti ravno tako pomembna ali še bolj pomembna od vloge kvadricepsa. Zmanjšano moč iztegovalk kolena po AP, ki zmanjšuje funkcionalno delovanje, vsaj delno povzroča otekanje kolena v zgodnji pooperativni dobi. To je nov in pomemben podatek. Poleg tega je zmanjšano funkcionalno delovanje po AP kolena povezano z omejeno močjo mišic iztegovalk in tudi z omejeno gibljivostjo kolena v smeri upognitve. V moji predstavitvi bodo poudarjene novosti v rehabilitaciji in funkcionalni rezultati rehabilitacije po AP kolena, spremljane z lestvico Knee Society Score pri pacientih med stacionarnim zdraviliškim zdravljenjem v Termah Olimia po AP kolena. Novelties in rehabilitation after total knee arthroplasty Mojca Kranjc, PT Terme Olimia, Slovenia People affected by severe knee osteoarthritis experience significant functional limitation, reduced lower limb muscle strength, aerobic fitness and quality of life. The need for rehabilitation following total knee arthroplasty (TKA) is based on the evidence that these deficits do not spontaneously resolve after surgery. Although there is a rapid and substantial reduction of knee pain after TKA, 37% of patients have limited functional improvement one year after the surgery. The most common limitations of these patients are diminished walking speed, difficulty in ascending and descending stairs, and inability to return to sports played prior to the surgery. The weakness of the quadriceps muscles has received a good amount of research attention, and several studies have tested its contribution to functional limitations after TKA. According to the studies revealed last year, the contribution of hip abduction strength to functional limitations in patients with TKA may be as relevant as, or even more relevant than the contribution of quadriceps muscle strength. The well-known finding of decreased knee-extension strength after TKA, which decreases functional performance, seems to be caused in part by postoperative knee swelling in the very early postoperative phase. This is a new and important information. Moreover, decreased functional performance after TKA seems to be related to both decreased knee-extension strength and decreased knee-flexion ROM. In my presentation, novelties in rehabilitation and functional outcomes of rehabilitation after TKA will be emphasized, using a Knee Society Score at in-ward patients, treated in Terme Olimia after TKA. Key words: bio energetic nutrition, energy value, water, wood, fire, earth, metal, cool, fresh, neutral, warm, hot Redno uživanje naravne mineralne vode Radenska Classic ugodno vpliva na rast mišic in zmanjševanje telesne teže – Pilotni primer Martin Trapečar1*, Mario Gorenjak2, Lea Zbontar Zver3, Avrelija Cencic1,2 1*Katedra za biokemijo in nutricistiko, Univerza v Mariboru, Medicinska fakulteta, Slovenija 2Katedra za mikrobiologijo, biokemijo, molekularno biologijo in biotehnologijo, Univerza v Mariboru, Fakulteta za kmetijstvo in biosistemske vede, Slovenija 3Radenska d.d., Oddelek za raziskave in razvoj, Slovenija Sodobno življenje je označeno z vse manjšo fizično aktivnostjo, močnim porastom stresa ter slabih prehranjevalnih navad. Iz tega razloga današnji potrošniki čedalje pogosteje iščejo načine, 27 kako se spopasti z izzivi, ki jih predstavlja naš čas. Naravna zdravilišča in uživanje naravnih mineralnih vod z visoko vsebnostjo mineralov, ki potrošniku zraven prehrambene vrednosti ponujajo tudi dodatne za zdravje koristne lastnosti, so pomembna alternativa. Takšna voda je mineralna voda Radenska Classic, ki jo tradicija že več kot 140 let povezuje s številnimi pozitivnimi učinki. V naši nedavni študiji smo ocenjevali biološko dostopnost Radenske Classic in njen potencialni vpliv na rast mišic v In vitro 3D celičnem modelu črevesja. Poleg tega smo povezali naše ugotovitve s klinično študijo, v kateri je sodelovalo 44 prostovoljcev, ki so redno uživali Radensko Classic skozi obdobje treh mesecev. V 3D modelu smo pokazali, da v primerjavi s kontrolno vodo Radenska Classic spodbuja rast skeletnih mišic in njihovo metabolno dejavnost. Ti rezultati so v skladu z ugotovitvami klinične študije, kjer so posamezniki, ki so uživali Radenska Classic, pridobili v povprečju 1 kg več mišične mase in izgubili 1,4 kg več telesne maščobe kot placebo skupina. Posledica te transformacije je splošni upad indeksa telesne mase. Značilnost visoke fizične aktivnosti je povečano delovanje muskulature in povečana izguba elektrolitov. Ob tem dejstvu predstavlja Radenska Classic kot dostopen vir ionov, ki hkrati spodbuja delovanje in rast mišičevja, odlično prehransko dopolnilo visoko aktivnemu delu prebivalstva kot tudi manj aktivnemu. Today’s consumers are therefore increasingly looking for ways how to cope with these challenges. Natural spa treatments and the consumption of natural waters with high mineral content, which provide the consumer with additional health beneficial attributes beside their nutritional value, represent an important alternative. Among such waters is the mineral water Radenska Classic that has been linked with widely recognised health related properties for over 140 years. In our recent study we have assessed the bioaccessibility of Radenska Classic and its potential to influence muscle growth in a novel In vitro 3D model of the gut. Further, we have linked our findings with a clinical study with 44 volunteers that have regularly consumed Radenska Classic for a period of three months. 28 The In vitro model showed that when compared to the control water Radenska Classic is able to enhance muscle proliferation and metabolic activity. These results are in line with the findings of the clinical study where subjects consuming Radenska Classic have gained in average 1 kg more muscle mass and have lost 1,4 kg more body fat as the placebo group, resulting in a overall decrease of the body mass index. With our work we have developed an approach that provides long time overdue means to fully exploit the potential of natural mineral water. Primerno je tako za posameznike po poškodbi hrbtenjače kot tudi za ljudi z duševnimi in fizičnimi omejitvami, kot so cerebralna paraliza, živčno-mišična obolenja, multipla skleroza, astma, amputacije, diabetes, slepota in gluhost. V določenih segmentih je potapljanje primerno tudi za starejšo zdravo populacijo, ki želi obnoviti, ohraniti ali izboljšati kakovost svojega življenja. Od leta 2002 je IAHD Adriatic razvil široko paleto programov, v katere so bile vključene in skrbno spremljane s strani zdravstvenih in potapljaških strokovnjakov iz celega sveta vse omenjene skupine. Programi se izvajajo v bazenih kot del moderne hidroterapije in v odprtih vodah kot osnovno in napredno usposabljanje za potapljače. V zadnjih desetih letih smo opazili, da so bili vplivi potapljanja majhni, vendar zelo koristni. Vpliv hidroterapije je namreč zaradi redne telesne aktivnosti, izboljšane socialne interakcije ter psiholoških prednosti v veliki meri pripomogel k izboljšanemu zdravstvenemu stanju in k večji kakovosti življenja invalidnih potapljačev. Kljub dejstvu, da invalidi, ki so do sedaj vključeni v naše programe, niso imeli omembe vrednih zdravstvenih zapletov, smo prepričani, da je še naprej potrebno nameniti veliko pozornost kakršnim koli spremembam. Sistematično zbiranje, dokumentiranje in obdelava pridobljenih podatkov povečujejo naše znanje in prispevajo k temu, da je potapljanje postalo dostopnejše in bolj varno za vse več ljudi s posebnimi potrebami. The benefits of scuba diving for disabled in modern hydrotherapy Branko Ravnak, Alenka Fidler IAHD Adriatic and Terme Zreče, Slovenia Scuba diving can be a part of rehabilitation and recreation for groups of people with disabilities. It is suitable for individuals after Spinal Cord Injury – SCI and also for others with mental and physical restrictions like cerebral palsy, nerve and muscular disorders, sclerosis multiplex, asthma, amputations, diabetes, blindness and deafness. In some parts can be used for the healthy older population who needs to recover, preserve or improve their quality of life. 29 Since 2002 IAHD Adriatic developed a wide range of programs in which all the mentioned groups were included and carefully monitored by medical and diving specialists worldwide. Programs are performed in swimming pools as a part of modern hydrotherapy and in open water as basic and advanced diving training. In last ten years we noticed that immediate effects of diving were minor and mainly beneficial. However, improved health and quality of life of disabled divers could be the effect of hydrotherapy, regular physical activity, improved social interactions, psychological benefits of achievements and fulfilled life. In spite the fact that until now disabled participants did not have any noteworthy medical complications, we believe that vigilance must be maintained. Systematic data collection, their analysis and documentation of experiences increased our knowledge and contributed to dive safety for people with disabilities. V zadnjih desetih letih se je po podatkih IPK International za takšno obliko preživljanja dopusta odločilo 69 odstotkov več ljudi. Zaradi navedenih dejstev želi prispevek analizirati enega ključnih elementov velneške ponudbe, in to je varnost v velneških centrih. Prispevek na sistematičen in analitičen način analizira vse zakonske temelje v Sloveniji, ki predstavljajo okvir za varno izvajanje dejavnosti v velneških centrih ter s tem tudi okvir za varnost gostov teh centrov. Sistematičnost pri obravnavi problematike prispevka nakazuje tudi dejstvo, da se v analizi prepleta tako zdravstvena varnost gostov, njihova osebna varnost ter varnost premoženja na eni strani, kot tudi varnost njihovih osebnih podatkov. Prav slednja omogoča zaupanje gosta, saj se z njo gostu zagotavlja osebna integriteta. Janez Mekinc Faculty of Tourism Studies Portorož – Turistica, Slovenia The importance of health-related holidays is coherent with the ESA (European Spas Association) fact, that yearly thermal spas in Europe are being visited by 20 million of guests. According to the IPK International in the last ten years 69 percents more of people decided to spend their holidays in thermal spas. Given those facts the article aims to analyze one of the key elements of wellness offer in spas that is security in wellness centers. The article systematically and analytically analyses legal foundations in Slovenia, that are presenting the legal frame for safe conduct of activities in wellness centers and by doing this also frame for guests security in these centers. Systematic approach to discuss the problem is shown by the fact that analyze aims to deal with health security of guests, their personal security and security of their property on one side and on the other side personal data protection. Personal data protection is enabling trust of the guests, since it is being foundation for their personal integrity. Nataša Artič Višja strokovna šola za gostinstvo in turizem Maribor, Slovenija Prispevek v okviru teoretičnih izhodišč predstavlja etiko v poslovnem svetu in tako na kratko opredeljuje razvoj pravil etike vedenja skozi čas, povezavo etike s pravom, razliko med etiko, moralo in omiko, načela etike, neetično delovanje in podobna teoretična izhodišča, ki so botrovala k oblikovanju etičnega kodeksa za velneško dejavnost. Nataša Artič Vocational College for Catering and Tourism Maribor, Slovenia Contribution to the theoretical study represents ethics in the business world and also briefly identifies the development of ethical rules of behaviour over time, the relationship of ethics with the law, the difference between ethics, morality and civilization, the principles of ethics, unethical action and similar theoretical frameworks that have fostered the creation of Code of Conduct for wellness activity. This paper describes a draft of such a code of ethics as the first germ of such rules 31 in the field of wellness activities in the Slovenian territory, which represents a new challenge for tourism operators who already have their peer code of ethics. In the final part of the paper presents and confirms the importance of compliance with codes and expectations of the users of wellness facilities in the area of ethics and presents opportunities for further development of ethical rules in the field of wellness activities. Rudi Rumbak Višja strokovna šola za gostinstvo in turizem Maribor, Slovenija Zdraviliško zdravljenje se v Sloveniji v pretežni meri še vedno financira iz javnih zdravstvenih sredstev. Plačnik te dejavnosti določa nabor, normativni obseg ter nivo kakovosti storitev, hkrati pa tudi pogoje za izvajanje dejavnosti, časovne normative ter ceno, torej standardizacijo zdraviliškega zdravljenja. Z vidika možnosti uvajanja medical wellnessa v ponudbo naših zdravilišč bi bilo zato morebiti najbolj enostavno te standarde prilagoditi povpraševanju po wellness storitvah, podobno, kot so bili nekdanji termalni programi, preventivni programi ter programi oddiha in sprostitve preoblikovani v novo wellness ponudbo. The payer of this activity provides selection, normative scope and level of service quality, but also the conditions for an activity, time benchmarks and price, thus standardizing the health resort treatments. From the viewpoint of the possibility of introducing medical wellness into our range of offers, they should therefore just adapt these standards to the demands for health resort treatments, as were former thermal programs, preventive programs and programs for rest and relaxation redesigned into new wellness. In this paper we want to draw attention to certain specificities of medical wellness as compared to conventional medical rehabilitation, but also to find any common ground between the already established standards in the field of medical wellness abroad. Učitelj ima danes na voljo veliko virov, pisnih in elektronskih. Internet ponuja skoraj neskončne možnosti za simulacijo nalog iz resničnega življenja. Študente spodbuja pri razvoju bralne in slušne kompetence in jim je v pomoč pri osvajanju besedišča. Prav tako lahko internet študentom služi kot vir pri kritičnem selekcioniranju in vrednotenju informacij kakor tudi pri razvijanju medkulturne kompetence. Kot predavateljica na Višji šoli za gostinstvo in turizem v Mariboru opažam, da moji študenti potrebujejo precej podpore in časa, preden začnejo uporabljati internet kot vir informacij. Predavanja in vaje načrtujem tako, da so študenti pri svojem delu čim bolj samostojni. Moje gradivo vsebuje veliko internetnih povezav, ki so izhodišče nalogam povezanim z bodočim poklicem naših študentov. Veliko pozornosti posvečam tudi samovrednotenju, ki je del mojega poučevanja. Travel agents and hotel owners should be able to meet the needs of modern travellers. That is why the basic goal of ESP learning is to develop the communicative competence. When we take into consideration ESP we have to be aware that it should serve as a support to other subjects and fulfil certain functions, such as formal writing, dealing with complaints, making phone calls, taking orders etc. Themes and topics should 34 be authentic and as close to real life needs as possible. There are a number of sources available nowadays, from written to electronic ones. The internet offers almost unlimited possibilities to explore the real life. It encourages students to develop their reading and listening skills and thus extend their vocabulary, to select the information and develop a critical approach towards it and last but not least to understand and be tolerant towards other cultures. As a teacher of English at the Vocational College for Catering and Tourism in Maribor I observe that students need quite some time to get used to use the internet as a source of information and real life tasks. I try to use my lessons and stimulate them to work on their own. My teaching materials contain many internet links which serve as a starting point for classroom activities and lead students toward real life tasks connected to their future profession. Dopoldan stresna služba, v privatnem življenju številne obveznosti, hobiji, družina, itd. Čedalje več ljudi se počuti izčrpane ali izgorele. Nekaterim se zdi, da je to le modna muha in s tem namigujejo, da to sploh ni bolezen, temveč le opravičilo za druge težave, ki jih imajo ljudje, kot so morda depresija, da želijo samo prekriti lenobo, ali podobno. Dejstvo je, da čedalje več ljudi izpolnjuje kriterije za izgorelost, da komaj obvladujejo vsakodnevne obremenitve, so tako fizično kot čustveno izčrpani in da živijo v notranji stiski. Ker za izgorelost nimamo neke laboratorijske preiskave in določene vrednosti, tudi nimamo zanesljivih statističnih podatkov o njeni dejanski razširjenosti v družbi. Nimamo niti univerzalnega zdravila za izgorelost. Dejstvo pa je, da se vse več ljudi sooča s tem pojavom, zato tudi mediji temu namenjajo čedalje več pozornosti. Znano je dejstvo, da izgorelost povzroči nekatere kronične bolezni. Kako se torej izviti iz pasti in kako ukrepati, saj gre po navadi za igro več dejavnikov. Najboljše napotilo, ki ga lahko damo, so gibanje, sprostitev in regeneracija telesa. Že Hipokrat je učil: »da je preprečevanje boljše kot zdravljenje«! Zato je bil razvit tretman, imenovan »PREVENTATIO®«, v katerega se vključuje relaksacija, regeneracija in tudi zdrava prehrana. We are faced with stressful days in the office, many obligations in private life, hobbies, family, etc. More and more people feel exhausted or even burned out. Some people might think, that being ‘burn out’ is just a fad and not a real ailment, but that people are just trying 35 to find an excuse for other problems, like for example depression, perhaps to hide laziness, or similar. The fact is that more and more people do fall under the burnout syndrome, all the struggling to cope with everyday stress exhausts them physically and emotionally and they actually are in a distress stage. Since we have no burnout laboratory tests and so special values confirming the syndrome, we do not have a reliable statistics about its actual prevalence in society. We neither have a universal medicine for the burnout syndrome. But the fact remains, that more and more people are faced with this phenomenon, so the media devote more and more attention to it. It is also known that burnout causes some chronic diseases. The question is how to break from these pitfalls and how to act in such cases, because usually there are several factors in question. Movement, relaxation and regeneration help our body and that is the best recommendation that can be given. Even Hippocrates taught that "prevention is better than cure"! Today the WELLNESS STARS logo stands for personally tested quality, for a highly qualified and well-trained staff offering effective treatments in hotels, Thermal Spas and medical wellness facilities alltrough Germany as well as in Switzerland and Northern Italy. The quality criteria of WELLNESS STARS are based upon intensive guest surveys and comparisons between all existing qualifying methods for the spa and health tourism in Europe. An advisory board consisting of hotels, clinics and hot springs is continuously evaluating and adapting these criteria to the current situation. Mag. Alenka Šik Adriatic Slovenica zavarovalna družba, d.d., Slovenija V Sloveniji že nekaj časa govorimo o potrebnosti zdravstvene reforme, ki jo javnost pričakuje z velikim zanimanjem. Ideja o ukinitvi dopolnilnega zavarovanja je sicer mnogim slišati simpatično, po drugi strani pa ni države, ki bi lahko v zdravstvu preživela samo z javnimi sredstvi. Slovenija je z razmerjem 70:30 med javnimi in zasebnimi sredstvi za zdravstvo v povprečju držav EU, ravno tako v deležu BDP, ki ga namenja za zdravstvo. Ukinitev dopolnilnega zavarovanja bi ta razmerja porušila, če ne najdemo ustreznega nadomestila z drugo obliko zasebnih sredstev - z novimi zavarovanji ali direktnimi plačili iz žepa. V okviru kratkoročnih fiskalnih ukrepov je vlada že ponudila nekaj predlogov. Svoj predlog po vzoru nizozemskega zdravstvenega modela so predstavile tudi zavarovalnice. Doplačila za zdraviliško zdravljenje v okviru javne košarice pravic se sedaj gibljejo od 15 odstotkov (nadaljevanje bolnišničnega zdravljenja) do 90 odstotkov (nadomestno zdraviliško zdravljenje), pri čemer zavarovalnice letno prispevajo za zdraviliške storitve 66 odstotkov sredstev (23,3 mio EUR), ZZZS pa 33 odstotkov (12 mio EUR). Reforma predvideva izključitev nadomestnega zdravljenja iz javne košarice pravic, tako da bi bili pacienti v celoti samoplačniki za tovrstno rehabilitacijo. Kratkoročni fiskalni ukrepi predvidevajo tudi zvišanje doplačil za zdraviliško zdravljenje, ki je nadaljevanje bolnišničnega zdravljenja, za 5 odstotkov in znižanje vseh bolniških nadomestil za 10 odstotkov. Kratkoročno gledano bodo torej ljudje morali sami plačevati za več zdraviliških storitev kot sedaj, v času bolniške odsotnosti pa bodo imeli na razpolago še manj denarnih sredstev. Finančna varnost bo nižja, zato zavarovalnice predvidevajo, da se bo potreba po zavarovalniških produktih zaradi večje finančne varnosti povečala, med drugim tudi na področju zdraviliškega zdravljenja. Zdravilišča nudijo številne medicinske in nemedicinske storitve, ki so lahko potencialni predmet zavarovanja. Škodna zdravstvena zavarovanja so tista, pri katerih zavarovalnica povrne stroške zavarovanih storitev neposredno izvajalcu - na primer da krije stroške za specialistične preglede, ambulantno diagnostiko, fizioterapijo, zdravstvene posege ali za nadstandardno bivanje v zdravilišču. Vsotna zavarovanja so tista, pri katerih dobi zavarovanec izplačilo na svoj račun. S tem denarjem po lastni izbiri krije stroške okrevanja, rehabilitacije, dodatnih zdravil po prebolevanju srčnega infarkta, možganske kapi, operacijah srca, po različnih boleznih in poškodbah gibalnega sistema. To so vse storitve, ki jih kakovostno in v zadostnem obsegu nudijo tudi naša zdravilišča. Odprtje trga zdravstvenih storitev v letu 2014 lahko pripelje v zdravilišča tudi dodatna sredstva domačih in tujih zavarovalnic. 38 Ključne besede: zdravstvena reforma, finančna varnost, zdravstveno zavarovanje, okrevanje, rehabilitacija. Adriatic Slovenica zavarovalna družba, d.d., Slovenia In Slovenia we have been talking for some time now about requiring a health reform that the public expects with great interest. The idea of terminating the co-payment health insurance sounds good to many, but on the other hand there is no country in the world that could survive only on public resources in health care. Slovenia is average among the EU countries with its 70:30 ratios among public and private resources for health care as well as with its GDP share for health care. The termination of co-payment health insurance would demolish these ratios, if we fail to find an appropriate compensation with another form of private resources – with new forms of health insurance or out-of-pocket payments. The government has already offered some of the prepositions under the short-term fiscal measures. The insurance companies also introduced the preposition of the model following the example of the Dutch health care model. The surcharge for spa treatment under the public basket of rights is now moving from 15 per cent (continuation of hospital treatment) to 90 per cent (compensational spa treatment) where the insurance companies contribute 66 per cent of financial resources (EUR 23.3 m) for spa treatments, the Health Insurance Institute of Slovenia (HIIS)) contributes 33 per cent (EUR 12 m). The reform anticipates the exclusion of compensational treatment from the public basket of rights, so that the patients would have to pay for the whole rehabilitation of this type themselves. The short-term fiscal measures anticipate the increase in surcharges for spa treatment as well, which represents the continuation of hospital treatment by 5 % and decrease in all hospital compensations by 10 per cent. In a short-term aspect the people will have to pay for a larger number of spa services as they have paid so far and will have, during the sick leave, fewer finances at their disposal. The financial safety will be weaker; therefore, the insurance companies anticipate that the demand for insurance products due to the higher financial safety will also increase in the field of spa treatment, as well as among other fields. The spas offer numerous medical and non-medical services that can prove to have potential for insurance. The insurance company can pays health care costs directly to the health care provider - for example the costs of out-patient services, physical rehabilitation, superior accommodation at the spa, or it pays the so called lump sum directly to the bank account of the insured person. With this sum he or she can freely choose what to do with received amount - for the recovery, rehabilitation, for supplemental medicines, after recovering from a heart attack, stroke, heart surgical operations, after various diseases and injuries of sceletal-muscle system. These are the services that are also offered by the Slovene spas and are of high-quality and on a sufficient scale. The opening of the health care services market in 2014 can bring additional resources from Slovenian as well as from foreign insurance companies to spas.
