No one else gives homeless individuals and families the comprehensive care they need to recover their health AND the supportive services to regain safe and stable housing. • We are partners with our clients. At HCH, people who are too often overlooked, marginalized, and abused are treated with patience, dignity, and respect. • We run innovative programs to demonstrate that homelessness need not be permanent. We work with homeless individuals with persistent illnesses and give them rapid access to housing and the wrap-around supportive services critical to keeping them housed. Eighty-five percent (85%) of the people whom we have helped in this way remain housed for the long-term. How do we end homelessness? • • HCH is a force of 180 professionals who come together with homeless people and housed people to fight for affordable housing, comprehensive health care, and adequate incomes for everyone. .84 out of every dollar goes to direct care $1,647,350 (12%) $571,312 (4%) HCH provides comprehensive health services to 9,000 individuals annually through more than 80,000 patient visits across the State. Program Services General & Administrative $11,219,408 (84%) Fundraising • Of our clients, 50% have an addiction; 34% have a major mental illness; and 25% have both. • Clients have very high rates of high blood pressure, asthma, diabetes, HIV, and other preventable and treatable illnesses. • Our therapists, counselors, nurses and doctors provide: o pediatric, adolescent, adult, and geriatric medical care o state-certified addiction treatment o outpatient mental health services o social work services, including casework and case management o dental care o HIV testing, counseling, treatment, and referral services • In addition to providing on-site care, our staff assists over 2100 people on the street, and in shelters, encampments, soup kitchens, and transitional housing. • HCH provides care in Baltimore City, Baltimore County and through contracts with the City of Frederick, as well as Montgomery and Harford Counties.
