Please ensure that you have completed your daughter’s Care Monkey Medical Profile and
Permission Form for camp for Year 7 prior to Friday, 24 November, 2014.

Care Monkey
At Firbank, we have Care Monkey which is a health and safety application. This is an innovative, parent
controlled electronic medical form. In essence, it is an electronic version of the forms such as excursion
permission forms. This system reduces the burden on parents to complete many forms throughout the
year.
Parents can access and complete the forms using a PC, laptop, tablet or mobile phone to enter details
such as: emergency contacts, medical information including asthma and allergies and notes and care
instructions which relate to the student.
You will receive an email from Care Monkey on Thursday, 6 November inviting you to enter the details for
your child on the electronic form. This information is owned and controlled by you (the parent) but visible to
the School.
We would appreciate your assistance in completing the forms as this will ensure that we always have
current information about your child.
If your child is new to the School in Year 7, you will need to complete Care Monkey by Friday 24,
November. No student is permitted to go on an excursion or camp unless Care Monkey has been
completed and permission has been given by a parent.
Textbooks, Stationery and Equipment Requirements







Lodge booklists with Campion Education as per instructions.
Stationery items are available from the Firbank Bookroom throughout the year.
The Bookroom will close on 12 December, 2014 and will re-open on Tuesday, 27 January, 2015.
Holiday Hours: 9.00 a.m. till 4.00 p.m. Closed between 12.00 p.m. and 1.00 p.m.
School Term Hours: 8.15 a.m till 4.15 p.m. Closed between 12.00 p.m. and 1.00 p.m.
The Bookroom is located at 23 Outer Crescent, adjacent to the Hospitality Centre.

Technology Requirements
Students entering Years 7, 8, 9 and 10 will be required to purchase an iPad. We recommend purchasing a
64-bit iPad Air.

Boarders only can order their books through the School and collect their books from the Bookroom
when School resumes. Parents of boarders will be charged on their School fee account for the textbooks.

Uniform
The Firbank Uniform Shop supplies all your daughter’s uniform requirements (with the exception of shoes).
A good range of new and second-hand uniforms is available. Payment is by cash, cheque, Mastercard,
VISA or EFTPOS.
The Uniform Shop is run by the Manager, Mrs Judy Kemp. Parents of new students are asked to
telephone Mrs Kemp on 9591 5140 for an appointment to outfit their daughter. Please allow approximately
one hour. The Uniform Shop is open during the School term each Tuesday, 12.30 to 5.00 p.m. and each
Thursday, 8.30 a.m. to 2.30 p.m. Please refer to the Information Package for additional opening dates.

Absentees for the Senior School
The absentee line is 9591 5198 which is available 24 hrs. There is also an email address if you prefer,
absentees@firbank.vic.edu.au
A note from parents/guardians is required for students who are required to leave the School for planned
appointments. A Note explaining the reason should also be written for students who are late for school.
For longer periods of absence or absence at the end or beginning of a term an application for Leave of
Absence must be requested in writing, to the Principal, one term in advance. Such leave will only be
granted in exceptional circumstances.
School term times are published well in advance and holidays should not impact on term time.
Lost Property
At the Senior School, small and/or valuable items of property lost by students may be handed in to
reception. Please note that any large lost property will be placed for safekeeping in the PESC (Physical
Education Sports Complex) Office. Please remember to name all items of clothing etc. so they can be
returned to their owner.
Students in Year 7 to 11 will be working from an E Diary, which will be web-based application, which will
be available upon commencement.
