**You may mail in your official tournament entry form and championship roster and payment to
the above address, you may also make your payment over the phone with a credit or debit card or
come into our office to register for this tournament. NO entry will be considered official until we
have received all official paperwork signed by your state commissioner. We will need either the
original or a faxed copy (928-373-5244) of the official entry form and championship roster to us by
the date of the draw Thursday September 10 at noon in order for your team to be officially entered
in the tournament.
Mandatory Manager’s Meeting and Lunch
Meeting will be held Friday, September 18, 2015 at 12:00 pm; at the Yuma Civic Center, located at 1400
W. Desert Hills.
One complimentary lunch ticket will be provided to each team, additional tickets can be purchased for
$10 each, limited amount of tickets will be sold.
