Project Bread is pleased to provide this 2015 Summer Food Service Program Site Grant Application packet to assist
you in applying for funding to ensure children 18 and under of all cultures and walks of life have reliable access to
nutritious food during the summertime. Please carefully read and follow the instructions on this page before
completing your grant application for 2015.
1. We strongly prefer that you use a computer to complete the application. Instructions for completing the
fillable electronic PDF version are found on page 3 of this package.
2. Applications must be complete. Please answer all questions that apply to your agency. Incomplete
applications will not be accepted.
3. Contact information must be accurate and spelled correctly. Please double check that the contact information,
mailing address, and name of the agency to which the check will be issued are accurate. If you receive funds,
checks will be sent to the mailing address you provide on page 5.
4. Providing accurate statistics is critical to your application. Statistics reported in this application are for the
duration of your SFSP program in 2014 (if you had one) as well as projected statistics for your SFSP program

5. All required documents must be included and sent as separate attachments (see page 4). Do not scan and
merge required documents into the application.

About Project Bread

Funding Criteria
• The purpose of Project Bread's Summer Food Service Program (SFSP) Site Grant is to ensure low-income
children in Massachusetts have access to nutritious, healthy food during the summertime.
• Grant requests are considered based on the need in the community, the impact on the targeted community
served, and the program's accessibility and efficiency in providing food to those in need.
• The program's assistance must promote the dignity and respect of those served.
Who is Eligible
Sites can apply for up to $2,000 in grant funds. Funds may be used for one or more of the grant categories
listed below.
a.
Creating a new site(s): Funds may be used to purchase required materials/equipment needed to start
an SFSP site. New sites must serve a minimum of 25 kids per day beginning in the summer of 2015. It
is expected that the site will continue to operate in future summers.

Introducing a nutrition education campaign: Grants will be available for sites interested in
developing a nutrition education campaign to increase consumption of fresh fruits and vegetables.
Activities may include taste tests, fruit and vegetable of the week promotion, nutrition-related games,
and/or other nutrition education programming. A portion of these funds (maximum: $400) may be
used to purchase fresh produce. A nutrition education toolkit will be made available to grant
recipients.
c.
Developing a teen outreach plan: These funds must be used for activities and outreach efforts that
specifically attract teens and preteens to the summer meal site. Specifically, funds may be used to
support creative outreach strategies or innovative programming, to purchase equipment for sports,
recreation, academic activities, and/or other program needs. All funds must be spent on items or
activities intended to draw teens and preteens to the summer meal site.
d.
Offering programming: Funds may be used to purchase recreation equipment as part of a
programming plan. Applicants may request funds to purchase new equipment for sports, arts and
crafts, academics, and other activities. Twenty percent (20%) of a request for funding in this category
may be spent on staffing, transportation, or fees provided such expenses serve a strictly recreational
purpose. A recreation toolkit will be made available to recipients of programming grants.
Grant Guidelines:
1.
Priority will be given to low-income communities where the percentage of students qualifying for free or
reduced-price school meals is 50% or higher and to programs that serve SFSP meals for at least 4 weeks.
2.
Priority will also be given to grant applicants requesting funds to purchase sustainable items, namely, items that
will be useful to the program not just in 2015 but also in subsequent years. These could include equipment for
food distribution, equipment for sports, recreation and academic activities, equipment for advertising and
promotion, and/or equipment to support other program needs.

Please give complete, specific responses to ALL questions.

For programming grants only, up to 20% of requested funds may be used for non-sustainable expenses (see
guideline #2), as long as these expenses strictly relate to providing programming. Priority will be given to
sponsors/sites seeking to create a memorable experience for children at their sites, whether that takes the form
of having a farmer, educational speaker, or performer visit on one or more days; or taking a field trip. These
funds are intended to enhance programming at sites and are not to be used for day-to-day operating costs.
Operation of the site must not depend on these funds.
5.
Grant funds must be spent by the last day of the grantee's SFSP.

Grant recipients will be required to return a grant award agreement, signed by the sponsor and business
manager. The agreement confirms an understanding that the grant funds must be spent as stated in the
application and by the allotted time frame.
7.
Along with the grant application, applicants must also complete and submit the Site and Budget Tracking
Spreadsheet, which also can be found on the CNOP website www.meals4kids.org/sfsp-grants.
8.
A sponsor and its site CANNOT both apply for funds from the same grant category.

For-profit entities are not eligible to receive grants.

The applicant will agree to complete an electronic grant reporting form to Project Bread by Friday,
September 4, 2015. Project Bread may conduct a site visit to the SFSP site(s) during the 2015 summer program.

To Open and Save Application
Save the attached file [2015 Summer Food Program Site Grant] to your computer. Please name the file with your
agency's name. For example, [Your Town] Boys and Girls Club rather than just 2015 Grant Application.
In most email servers you can click the right mouse button, select "Save As" and a box will pop up asking you where
you would like to save the file. Take note of where the file has been saved.
After saving to your computer:
■ Open Adobe Reader.
■ On the toolbar select File and then select Open.
■ Locate the saved file on your computer and double click. The 2015 application should open.
■ You can save your progress as you fill out the form by selecting File on the toolbar and then Save.

Along with this grant application, applicants must also complete and submit the Site and Budget Tracking
Spreadsheet, which can also be found on the CNOP website www.meals4kids.org/sfsp-grants. Please submit this
completed form in the same e-mail as this grant application.
Within this spreadsheet, there are two tabs. The first tab, "Site Information" asks for your site name(s), whether the
site is open or enrolled, the days of the week that meals are provided, the types of meals that will be served, and the
meal start and end times. If you oversee multiple site locations then please include information for each site.
The second tab, "Budget Tracking," asks you to list items to be purchased with the 2015 grant award from Project
Bread. Please list the items, quantity, and at what SFSP site(s) they will be used. Please follow the example at the
top.
