Reports: Clinical Data and System will not group report data from two Missing Labs Report facilities due to identical "Doing Business As" name. The CMS-2744 forms are completed once a year for all Medicare-certified dialysis and transplantation centers. To assist with meeting a CMS submission deadline, each ESRD Network set local due dates for facilities within their service areas. False. Users who manually enter data on the Clinical screens must log in to CROWNWeb and report the last lab values and other clinical data obtained for patients during each Clinical Month. Additionally, it is recommended that users who do not manage the manual data entry of clinical data due to an electronic data upload (“Batch”) verify the uploaded data’s accuracy. Some of the data on the Hemodialysis and Peritoneal Dialysis screens, and most of the data on the Vascular Access screen carry over from month-to-month, but they also include fields that must be updated each month for the carry over logic to continue. As part of it efforts, HMS randomly selects a number of Medicare-certified facilities throughout the nation. It then submits a formal request for items such as patients’ clinical data, and completed CMS forms. Please note that as part of its CMS-2728 form validation process that HMS will require medical records to support changes. Welcome to MyCROWNWeb.org Since the initial rollout of CROWNWeb, CMS has utilized http://www.ProjectCROWNWeb.org as the online communication and training resource for all CROWNWeb users. The Web address was ideal during CROWNWeb’s pilot phases. However, with the system being used by more than 6,000 Medicarecertified facilities, it is no longer in a “project” phase. With this in mind, in March 2015, CMS changed the CROWNWeb communication and training Web address to http://www.MyCROWNWeb.org. Users can access the same materials that were available via http://www.ProjectCROWNWeb.org at http://www.MyCROWNWeb.org.
