Thank you for participating in the 2015 Great Place to Work (GPTW) Trust Index Survey that
ended May 3. The results will provide crucial insights into how you perceive and experience
your workplace by measuring levels of trust, pride and camaraderie.
Our next step requires the same survey to be completed June 2-16 by approximately
2,000 randomly-selected team members. The results of this survey will be used as a part of
our application to be considered for the FORTUNE 100 Best Companies to Work For® list.

The online survey consists of 58 questions and takes about 15-20 minutes to complete.

Making the Most of Your Dental and
Vision Insurance
WellStar offers comprehensive dental and vision insurance coverage. If enrolled, are you
taking advantage of all that the plans offer?
Dental Insurance
MetLife is our dental plan administrator and
offers the Dental Access and Dental Choice
plan options.
Dental care is important to your overall health,
and early detection and prevention are vital to
minimizing your need for more serious dental
treatment. Both of WellStar’s plan
options cover key preventive/
diagnostic services, such as
oral exams, cleanings and
X-rays, at no charge to you.
For basic services, which
may include fillings, oral
surgery or non-surgical
gum treatment, provided
in-network after you meet
your deductible, the Dental
Choice option pays 80 percent and
Dental Access pays 60 percent.

Helpful Information Just a Click Away
EyeMed is our vision plan administrator and offers
the Essential and the Enhanced plan options.
By registering with www.eyemedvisioncare.com, you
will have online access to view your benefits, including
claims and explanation of benefits (EOB) documents.
You also can find a provider, access a library of vision
wellness articles and videos and print a copy of your ID
card. To begin, click on the “Member Login” box from
the EyeMed homepage.
Vision care should be a part of your annual
preventive care routine. In addition to correcting
any vision problems, an annual comprehensive eye
exam also can reveal the warning signs of more
serious undiagnosed health problems, such as
hypertension, cardiovascular disease and diabetes.
For an annual eye exam with an in-network
provider, you pay $10 in the Essential plan option
and pay zero in the Enhanced option. If you need
glasses, standard plastic lenses, including single
vision, bifocal, trifocal, lenticular and standard
progressive lenses, are available at no cost to
you through the Essential plan option when you
are in-network. The same options for lenses are
available through the Enhanced option for $10
when in-network. For frames with an in-network
provider, the Essential option has a $130 allowance
and 20 percent off the balance above $130. In the
Enhanced option, there is a $160 allowance and
20 percent off the balance above that price. For

You also can view your vision benefits on the go with
the EyeMed Members mobile app. You can search for
in-network providers, view a copy of your ID card, get
directions to your provider’s location and find answers
to common question. Search for “EyeMed Members” in
the App Store to download for Apple devices.
EyeMed also helps you save money while investing
in your eye health with a $50 certificate towards
non-prescription sunglasses from Sunglass Hut. You
and each dependent enrolled in a vision plan option
(Essential or Enhanced) are eligible for a savings
certificate. The one-time certificate expires Dec. 31,
2015, and may not be combined with any other offers
or discounts. To access the savings, sign up at www.
eyemedvisioncare.com/sunperks. You will receive an
email with a savings code that can be used in Sunglass
Hut stores or online at sunglasshut.com.
Next PTO Cash-In Deadline is May 22
WellStar offers the opportunity to convert PTO hours into cash for full-time and benefits
eligible part-time team members who have been at WellStar for at least six months. The
first PTO cash-in for 2015 will be paid on June 11.
You may cash-in PTO hours at 80 percent of the value for PTO that you have accrued
this year or in prior years. The deadline to submit the 80 Percent PTO Cash-In
form to the Payroll Department is May 22. The form, along with additional
cash-in information, is available on eSource. From the homepage, select
Departments>Accounting and Finance>Payroll>PTO Cash-In.
If you submitted a 100 Percent PTO Cash-In form to Payroll by Nov. 30,
2014, no action is required.
The next PTO cash-in will be paid on Nov. 12, 2015.
Please remember that 403(b) contributions will be
deducted from PTO cash-in. For questions, contact

While not a topic we like to think about, it is
important to create an estate plan, which includes a
will and other legal documents addressing health and
financial decisions for the future. The hardest part
often can be knowing how to begin and where to turn
to for information. Through WellStar, you can access
a variety of educational resources—and even create
your own legal documents for free. WellStar’s basic life
and accidental death and dismemberment insurance
vendor, Cigna, in coordination with ARAG, offers these
resources, and more.

and complete the online form.
One of the form items is a checkbox to indicate
whether you have basic life or accidental death
and dismemberment insurance coverage through
Cigna. Full-time and benefits eligible part-time team
members who meet eligibility requirements are
automatically enrolled in both programs at no cost.
Once the form is complete and you register as a new
user, you can immediately start building your will
and other legal documents.

EAP is Another Estate Planning Resource
WellStar’s employee assistance program (EAP)
administrator, GuidanceResources®, also offers free
education and resources for estate planning.
• Will – Details what is to be done with your property
when you die, names the executor of your estate
and allows you to name a guardian for your minor
children
Additionally, if you decide you would prefer to
create your will and other legal documents through
a lawyer, the EAP can provide a list, based on your
search criteria, from its extensive database. Once
you select a lawyer to work with, you will pay a fee to
create your estate planning documents. Remember,
you can always contact the EAP for free consultations
and information via the phone with the program’s
in-house attorneys.
• Living Will – Outlines your wishes regarding the
use of life support or other life-sustaining medical
treatment
• Healthcare Power of Attorney – Allows you
to grant someone permission to make medical
decisions if you are unable to make them yourself

National Health &
Fitness Month
HealthStart will continue celebrating National
Health & Fitness Month throughout May with
activities to encourage physical fitness and
healthy eating. From Lunch and Learns to
walking classes to the Eat Well Program and
more, there will be something for everyone!

Schedule Your Biometric
Screening for HealthStart
If you are a Program Year 4 participant in HealthStart, your
next step in achieving the financial incentive is to complete
a Biometric Screening at a medical provider’s office. This
screening must be completed by Sept. 30, 2015.
When you schedule your appointment, request a “wellness visit
with biometrics.” If you have group medical coverage, this should
be free of charge—unless you are examined for a medical
condition or illness during your visit, or you have already had a
wellness visit within the current plan/calendar year.
One wellness office visit is covered at 100 percent under the
Affordable Care Act (ACA) for most group-sponsored medical
plans, including the WellStar Employee Medical Plan, each
plan/calendar year. Under the ACA, the preventive Biometric
Screenings also are covered at 100 percent.
Prepare for Office Visit and Results
You will need to take a hard copy of an Alternative
Means Screening (AMS) form to your visit, and leave this
form at your provider’s office. The form is available on
wellstarhealthstart.org. Once your lab results are in, you
must collect your completed and signed form, and fax or
mail it to HealthFitness.

Reminder: Making
Benefit Changes
During the Year
If you experience a qualifying life event—some
examples include marriage, divorce, birth of a
child, change in a dependent’s student status or
change in your employment status—you may
be eligible to make changes to your benefits.
You first must submit documents to support
this change to Human Resources. You need to
enroll in or make changes to your coverage
levels within 31 days (beginning on the event
date) by accessing wellstarmybenefits.org.

WellStar’s Employee Assistance
Program (EAP) is a trusted resource for
information and support for personal
issues, but did you know you can seek
help in other areas? Our EAP, which is
administered by GuidanceResources®,
also provides assistance with legal,
financial and work-life issues.
One of the newest features is helping you find a summer camp
for your kids. Just provide details about what you are looking
for—day camp, overnight or a specialty camp such as drama,
sports or music—and the EAP will provide you with three to five
listings that match your request. Each screened referral includes
a program description, contact information and details about
camp tuition. There is no fee for the referral assistance.
