Commissioner _______________________________________________________________ nyc.gov/health Introduction This directory aims to provide New York City hospitals, healthcare facilities, and city response agencies with a comprehensive resource of pediatric critical care services offered by NYC hospitals that includes information and instructions on admission and/or transfer of critically ill pediatric patients to hospitals with Pediatric Intensive Care Units (PICU). This guide is intended to provide information for daily referrals and inter-hospital transfers as well as to assist healthcare providers to care for pediatric patients during a disaster when their typical referral choices may not be available. Data for this project was collected by the New York City Department of Health and Mental Hygiene Healthcare Emergency Preparedness Program during 2006-2007 as part of the Pediatric Resources and Emergency Preparedness Initiative (PREP-I) Project. This project was funded by the Department of Health and Human Services Office of the Assistant Secretary for Preparedness and Response Grant Number U3RHS07565. Must be staffed 24 hours a day by a board certified/board eligible neonatologist either in-house or readily available by phone. Must have the ability to provide 24 hour cardiorespiratory monitoring and mechanical ventilation of neonatal patients within the neonatal intensive care unit. Neonatal Patient: A patient ranging in age from 0 to 28 days after birth. Pediatric Emergency Department: A separate emergency department (ED) for pediatric patients ranging in age from 0 to at least 17 years old but not routinely more than 21 years at time of presentation. Pediatric Inpatient Unit: A physically separate inpatient unit for pediatric patients (including general medical/surgical beds) with patients ranging in age from 0 to at least 17 years but not routinely more than 21 years at time of admission. Each patient must be assigned to a board certified/board eligible pediatrician, family practice physician or pediatric subspecialist and must be covered 24 hours a day by one of the same who is either in-house or readily available by phone Pediatric Intensive Care Unit (PICU): A high acuity inpatient unit with patients ranging in age from 0 to at least 17 years but not routinely more than 21 years at time of admission. Pediatric Transport Team: A team of health care workers with some additional pediatric training consisting of a minimum of trained Emergency Medical Service (EMS) workers who travel with, stabilize, and manage pediatric patients during interhospital transfer. The team may also include health care workers with more extensive pediatric training (pediatric attending, pediatric intensive care attending, residents, nurse practitioners, nurses with various levels of training who are experienced in caring for pediatric patients, physician assistants). Must be under the medical direction of a pediatric provider who directly (by being physically present) or indirectly (by phone or by protocols communicated to the team before leaving for transport) oversees the transport. Must be subject to periodic quality assurance Transport Team: A team of health care workers that stabilizes, manages, and travels with patients during interhospital transfer. The team may or may not be dedicated to a particular hospital or a particular inpatient unit within a hospital. Well Baby Nursery: An inpatient unit consisting of newborns that require only routine infant care with the exception of phototherapy therapy for hyperbilirubinemia and that is only utilized post-delivery or transfer from the NICU.
