Registered Practical Nurse Services to the First Nations’ Population 1 Permanent Full Time (Wallaceburg/Walpole Island) The Canadian Mental Health Association Lambton Kent Branch is a proudly merged community agency serving the mental health needs of Lambton County and the Municipality of ChathamKent. Our efforts promote community integration, build resilience, and support recovery from mental illness. This position is responsible for providing highly individualized and flexible support to persons with a mental illness or suspected mental illness. The incumbent will be required to develop, implement, and evaluate individualized treatment plans with clients on an on-going basis. The incumbent will work with the RN on complex assessments that require a RNs consultation. The incumbent will engage the client and families, providing education and support to them through the treatment and recovery process. The incumbent will also work collaboratively with community partners; the Community Health Team and Community Health Centre on Walpole Island; to provide services and where appropriate assist clients with access and linkage to other resources. Reporting to the Manager of Integrated Client Services, the incumbent will be responsible for the following functions: o o o o o o o Medication management including monitoring, delivery and administration of oral medications and health teaching. Provides discipline specific assessments to monitor physical and mental health Responsible for the secure storage of medications. Acting as a resource person or an advocate to assist individuals in obtaining information/accessing services/supports which are deemed needed Researching and preparing group resources and materials. Providing long acting injection to identified clients as per physician’s orders. Providing necessary assessment to clients receiving long acting injections and clozapine. QUALIFICATIONS: Registered Practical Nurse Diploma from a recognized post-secondary institution; certificate of competence from the College of Nurses of Ontario; and Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this population. Experience working with First Nations/Aboriginal People in a community based setting. A demonstrated knowledge, awareness and sensitivity to Aboriginal Cultures and Traditions. The candidate will work with other professionals using strong problem solving and communication skills. A valid driver’s license, and use of a vehicle, which will be used in conducting agency business, is required. Proficiency in both official languages will be considered an asset. SKILL REQUIRMENTS: Must have strong engagement and assessment skills; Non-Violent Crisis and Suicide Intervention training is an asset; First and CPR Training Extensive knowledge of the Mental Health Act, as well as relevant legislation specific to an individual with a serious mental illness; Planning and organization skills to facilitate advocacy and support coordination/service access; Working knowledge of metabolic syndrome and interventions; Extensive knowledge of the support/treatment services network in the community and of regional services commonly accessed by the program; Proven competence in the use of computer software and related applications (ex. Microsoft Office); and Well-developed written and verbal communication skills. Hours of Work: 35 hours/week. Flexible hours are required to meet the position requirements.
