Season Greeting
Dear Friends,
We are thankful to our members for giving us tremendous support in all
our activities, which encourage us to do it better and better.
Hope everyone enjoyed a festival of color, 'HOLI', happily and now
days of winter are almost over and summer is trying to establish with the
interruption of rain in between. Our medical fraternity is dealing with 'Swine
flu' a real devil of present time. Ahmedabad Medical Association had also
join hands to fight against 'Swine flu' and organized a public seminar on it
and gave important tips to public about how to prevent it. We also organize
Scientific Program No. 4 on Respiratory Update. Ahmedabad Medical
Association Senior Citizen Club had organize a program on Old age
Ophthalmic Problems. Our Ahmedabad Medical Association Woman
Doctor Wing had join in women bike rally on World Women Day.
In coming month AMA is going to organize a scientific program in
which all faculties are invited from Mumbai. AMA Senior Citizen club is
going to organize a program on Organ donation.
This year we are going to organize our local conference AMACON th
2015 on 10 May, 2015. To make it grand success we request our member to
enroll as an early bird. This year we are also inviting Scientific Paper/Poster
from resident doctor and our member. Our whole organizing team working
very hard to make this conference a memorable one with lots of scientific
session, Entertainment, Lucky draw, scientific paper and many more.
This month is full of exam, so we wish for better result & grand success
in exam to our member's children.

Subject : Fwd: Esophagus-One more curable cancer
According to recent Globocan data the incidence of Esophageal cancer is
on the rise in India. The reported number was about 41000 & odd numbers
of new cases in our country in 2012. This is likely to go upto about 52000 by
2020 & about 68000 by 2030.
Symptoms of Esophageal cancer often include trouble swallowing and
weight loss. Other symptoms may include pain with swallowing, a hoarse
voice, enlarged lymph nodes around the clavicle ,a dry cough, and possibly
coughing up or vomiting blood.
The two main sub-types of esophageal cancer are squamous-cell carcinoma
which is more common in the developing world and adenocarcinoma
.
which is more common in the developed world In our experience 70% is
squamous & 30 % is adenocarcinoma. A number of less common types also
occur. Squamous-cell carcinoma arises from the skin cells that line the
esophagus. Adenocarcinoma arises from glandular cells present in the
lower third of the esophagus, often where they have already transformed to
intestinal cell type(a condition known as Barrett's esophagus) The most
common causes of the squamous-cell type are: tobacco, alcohol, very hot
drinks, and a poor diet. The most common causes of the adenocarcinoma
type are smoking tobacco, obesity, and acid reflux.
The disease is diagnosed by biopsy done by an endoscope. Apart from
routine CECT, PETCT is increasingly used in this cancer for the staging.
Endoscopic Ultrasonography(EUS) is also used in specialised centers for
further information of mediastinum in selected cases.
Treatment is based on the cancer's stage and location, together with the
person's general condition and individual preferences.
Endoscopic mucosal resection (EMR) is the removal of small tumors by
endoscope, that only involves the mucosa or lining of the esophagus (stage
T1a).
Small localized squamous-cell cancers (T2N0) may be treated with surgery
alone with the hope of a cure.
At HCG Cancer Center, Ahmedabad we treat all patients of esophageal
cancer beyond T2 & N+ by Preoperative Chemo-Radiotherapy. The
protocol we use is 25# of radiotherapy by a very high end radiation machine
Rapid Arc. It radiates in 360 dgree in every fraction & that is how 95% of
patients complete the treatment without interruption. The chemotherapy
given during this treatment is Paclitaxol & Carboplatin every week for 5
weeks. There after there is a recovery period of 4-6 weeks & than all patients are
subjected to Investigations to stage the disease.

Thereafter all patients with abdominal oesophageal cancers are treated by IvorLewis operation & all thoracic oesophageal cancers are treated by thoracoscopic
mobilisation of oesophagus so no need of thoracotomy at all. Than
oesophagogastric anastomosis is done in left neck. We do all anastomosis by hand
sewn methods. Because of this minimally invasive technique sometimes we have
been able to discharge the patient as early as 6th post operative day. The
pulmonologist is always the part of our team & has resulted in significantly
reducing pulmonary complications.
In recent times with the concerted efforts morbidity of oesophageal surgery has
gone down to <20% & mortality 2-5%.
The pathologists will report all the specimens in details giving us the information
about the effect of chemoradiation. In about 30% of patients there is no rasidual
tumour in the specimen. We expect this patients are likely to have long term
survival.
With this kind of approach five-year survival rates can be upto 40%.
In the presence of extensive disease or if the affected person is not fit enough to
undergo surgery, palliative care is often recommended. Outcomes are related to the
extent of the disease and other medical conditions, The modern approch to
advanced cancer is to put the self expanding metallic stent by endoscope which
definitely solves the problem of dysphagia & particularly blessings for the patients
suffering from tracheoesophageal fistula where in past they were miserable from
aspiration pneumonitis & intractable cough.
Some rare form of treatments are:
Laser therapy is the use of high-intensity light to destroy tumor cells; it affects only
the treated area. This is typically done if the cancer cannot be removed by surgery.
The relief of a blockage can help to reduce dysphagia and pain. Photodynamic
therapy, a type of laser therapy, involves the use of drugs that are absorbed by
cancer cells; when exposed to a special light, the drugs become active and destroy
the cancer cells. Radiofrequency ablation is a new treatment modality for the
treatment of Barrett's esophagus and dysplasia, and has been the subject of
numerous published clinical trials.
Prevention includes avoiding tobacco & alcohol.
